The development of ovarian follicles was studied ultrasonographically in 21 cycles in eight infertile patients with the diagnosis of ovulatory dysfunction in whom conception did not occur after the defect had apparently been corrected. Pelvic ultrasound demonstrated three patterns in 20 of the 21 cycles studied: ovarian hyperstimulation in seven cycles; follicular atresia before ovulation in three cycles; persistent unruptured follicular cysts in 10 cycles. Recognition of these patterns has been helpful in modifying therapy in infertile patients.

The ultrasonographic features of multicystic renal dysplasia in 15 patients (14 children, 1 adult) are described with emphasis on differentiating this condition from hydronephrosis. Ultrasonography is a reliable method in the detection of free fluid in the abdomen. In 2 patients with severe pre-eclampsia, maternal ascites was diagnosed by ultrasound. Both babies were delivered prematurely by Caesarean section, indicated by deteriorating maternal condition. One of the babies died in the early neonatal period. The sonologist should be aware of the possible presence of peritoneal free fluid in patients with severe pre-eclampsia, and be able to demonstrate this accordingly in the ultrasonogram.

End-stage renal cystic disease and severe hydronephrosis may reveal similar ultrasonographic findings. A "blind analysis" of 13 cases selected on the basis of smiliar extensive end-stage disease (little or no renal parenchyma visualized) was performed. The identification of a dilated renal pelvis was the most reliable ultrasonographic indicator of hydronephrosis. Ultrasonic irradiation of the labyrinth through the round window approach is a minimally destructive method of surgical management for Meniere's disease. Results of 48 procedures, followed from six months to eight years, are reported. Relief from vertiginous symptoms was obtained in 70 percent of the patients. Hearing was improved in 14 percent; hearing was also significantly worse in 14 percent. The ultrasonic appearance of the normal and abnormal fetal spine before 20 weeks' gestation was reviewed in 121 pregnances. A model was constructed to establish the mechanism of production in the various configurations seen. The relative roles of ultrasonography, alpha fetoprotein assay, and fetoscopy in the diagnosis of spina bifida are discussed.

Recognition of thrombosis of a Björk-Shiley aortic valve prosthesis 4 years after insertion in a patient was based upon sudden clinical deterioration, loss of prosthetic sounds, and development of new stenotic and regurgitant murmurs. Thrombotic fixation was confirmed by diagnostic alterations on the echocardiogram. All manifestations reverted to normal after successful surgical debridement of the prosthesis. Echocardiography is a valuable noninvasive adjunct in the differential diagnosis of prosthetic valve malfunction.

A new ultrasonic equipment for ophthalmological diagnosis has been developed. Manual compound scanning using water immersion made acurate representation of cross-section of the orbital structure possible. A large, focused transducer brought resolution and sensitivity. Gray scale display of the ultrasonogram increased information concerning intensity of echoes. Ultrasonograms of retinoblastoma, vitreous hemorrhage, melanocytoma, optic disc coloboma and unilateral exophthalmos are presented.
