[Evaluation of factors of female urinary incontinence and therapeutic indications].

A good understanding of the factors involved in female urinary incontinence is essential for the therapeutic management of incontinent women. Urinary stress incontinence involves three variables which participate in the pressure equilibrium during effort: mobility of the urethrovesical junction, urethral resistance and the intensity of effort. The most appropriate treatment for each patient can only be selected by investigating all three of these factors and by determining their respective roles in incontinence. Urge incontinence is also multifactorial, but, very often, no cause can be found o…

Intravaginal maximal electrical stimulation in the treatment of urinary incontinence.

Urinary incontinence imposes a large economic burden, estimated at $10 billion per year. As the cost of health care continues to rise, conservative therapeutic measures are becoming more attractive. Anecdotal reports suggest that electrical stimulation may be up to 87% effective in the treatment of urinary incontinence. Investigators use different stimulation devices and protocols and make a comparison of results difficult. The true efficacy of electrical stimulation for the treatment of urinary incontinence is unknown since there have been no controlled clinical trials. Within our referral-ba…

[The role of urodynamic study in female patients with urinary incontinence].

To determine the type of urinary incontinence in 96 female incontinent patients, we performed a comparative study between the clinical evaluation and the results of urodynamic studies. Their complaints, physical examination, laboratory examinations including routine urological X-ray and ultrasonic studies, revealed that 15 to 30% of the patients had another type of incontinence than those with stress incontinence, urge incontinence and those in whom diagnosis was made from urodynamic studies. Urodynamic studies disclosed that another urinary incontinence type was observed in 7 to 18% of the pa…

Parity as a correlate of adult female urinary incontinence prevalence.

STUDY OBJECTIVE: The aim was to investigate the possible association between parity, as indicated by the number of childbirths, and prevalence of urinary incontinence in an adult female population sample. DESIGN AND SETTING: A sample of 3114 women aged 30-59 years was selected at random from the population of Aarhus, Denmark, and mailed a self administered questionnaire on urinary incontinence and, among other things, parity. PARTICIPANTS: A total of 2631 questionnaires was returned (85%) with a slight but significant decrease in respondency by age. MAIN RESULTS: The 1987 urinary incontinen…

[Diagnostic and therapeutic approach to stress urinary incontinence in women].

This work concerns twenty eight cases of urinary stress incontinence treated between 1977 and 1987. The high incidence of previous perineal operations and multiparity are noted. The diagnosis has been established clinically in all cases. Different types of surgery for urinary stress incontinence have been used: endoscopic bladder neck suspension in 17 cases, and retropubic colpopexy in 9 cases. The results are good in respectively 88% and 78%. Correct diagnosis and operative indication are the main elements of treatment of urinary stress incontinence. Each procedure permitting reposition of th

Vaginal diaphragm rings in the treatment of stress urinary incontinence.

To assess the effectiveness of wearing vaginal diaphragm rings in the treatment of stress urinary incontinence, 10 women with genuine stress incontinence were studied. Patients underwent urodynamic evaluation and perineal pad testing before and after fitted diaphragm rings were in place. Patients kept symptom diaries for 1 week without the ring and then for 1 week with the ring. They also gave an overall subjective evaluation of their experience. Urodynamic findings were essentially unchanged by wearing diaphragm rings. Trends toward improvement were observed in the results of pad tests and sy…

Colposuspension for urinary stress incontinence in premenopausal and postmenopausal women.

Sixty-nine premenopausal and 53 postmenopausal women had a colposuspension operation for urinary stress incontinence. A significant postoperative reduction (p less than 0.001) of symptoms of frequency, nocturia, urgency and urge incontinence was obtained in both groups. Postoperatively, 88.4 per cent of the premenopausal women were found to be dry compared with 66 per cent in the postmenopausal group (p less than 0.01). No differences were found preoperatively and postoperatively in the cystometric values or in the urethral pressure profiles at rest in both groups and between the groups. The p

[Clinical effect of terodiline hydrochloride on pollakisuria and urinary incontinence].

Efficacy and safety of terodiline hydrochloride were studied by treating 38 patients who complained of pollakiuria and incontinence with this drug. Terodiline hydrochloride in a dose of 24 mg once or 12 mg twice a day was administered to these patients for 4 weeks and the conditions of the patients before and after the treatment were evaluated. According to the subjective symptoms, pollakisuria during the daytime and at night, incontinence, anischuria at night and frequency of urination were all reduced significantly. Objective symptoms detected after the treatment a tendency of increase (p le

[Construction of a decision tree for preoperative diagnosis in urinary incontinence].

In order to analyse the decisions used in non-urodynamic preoperative diagnosis of female urinary incontinence we constructed a decision tree, consisting of decision nodes and probability nodes. Each branch of the decision tree leads to an end point (terminal node), to which an utility must be assigned. We describe the different steps in the construction of the decision tree: 1. listing of all possible decisions (questionnaire, clinical stress test, both, no preoperative tests), 2. assignment of probabilities to the different events (e.g. cure, failure, operation in the absence of genuine stre…

Diagnosis and management of female urinary incontinence in general practice.

In response to an invitation sent to women who had complained previously of regular incontinence, 65 women with regular incontinence were seen by their general practitioner. A diagnosis was made using a personally administered questionnaire and appropriate examination. Patients were placed in one of three diagnostic/management categories--stress, urge or stress/urge incontinence--and were given an appropriate treatment programme. Fifty six women were recruited as matched controls from non-responders while attending the surgery for other reasons. They underwent identical entry procedures but we

The Aldridge sling procedure in the treatment of urinary stress incontinence.

The long-term results of the Aldridge sling procedure for the treatment of recurrent urinary stress incontinence was assessed in 50 women most of whom had had previous failed vaginal surgery; 78% of patients were cured of all symptoms and a further 20% were symptomatically improved--a 2% failure rate. Only one patient reverted from a cure to improved during the lengthy follow-up. The procedure is not difficult to perform and is a good surgical option in patients with failed vaginal surgery.

The surgical management of total urinary incontinence in the female patient.

A total of 14 female patients with complete urinary incontinence underwent urethral reconstruction by a variety of techniques. Included were 2 patients with acquired urethral defects, 5 with congenitally short urethras, 2 with epispadias, 3 with remnant ureteroceles and 2 with cloacal anomalies. Complete continence was restored in 10 patients, 3 were improved markedly and there was 1 failure owing to obstruction with overflow incontinence. In several patients full control was acquired slowly and was hastened by bladder retraining. Urodynamic studies proved to be extremely valuable for the post…

[Treatment of urinary incontinence of neurologic origin in children and adolescents].

Recent advances in the treatment of urinary incontinence of neurological origin in children are discussed. The etiology of the neurological changes is mainly congenital (myelomeningocele, sacral agenesis and hidden dysraphism) other causes such as myelitis, radiculitis or cord injuries being very rarely observed. Two fundamental elements can be evaluated by radiologic and urodynamic investigations of the lower urinary tract: the capacity for compliance of the detrusor muscle and the degree of cervico-urethral resistance. The presence of a cervico-urethral obstruction is no longer an indication…

A proposed scoring system for the management of female urinary incontinence.

The management of urinary incontinence is frequently ineffective. Inappropriate surgical intervention can convert medically treatable incontinence into a more serious and less remediable condition. To allow a more scientific approach to management, we have developed a scoring system for female incontinence based on three primary controlling factors: (1) the direction of the urethra, (2) the urethral vesical angle and (3) the urethral pressure profile. For each factor a score of 1 to 3 was assigned according to the deviation from normal anatomy or function. Scores ranged from 3 to 9. Patients s…

Detrusor instability score in the evaluation of stress urinary incontinence.

The major cause of failure in the surgical treatment of stress urinary incontinence (SUI) in women is an occult detrusor instability. In order to detect detrusor instability and its degree, urological histories were standardized by scoring the replies to ten specific questions with O (indicative of SUI), 1 or 2 (slightly and markedly indicative of detrusor instability, respectively). The sum of the scores was termed the "detrusor instability score" (DIS). The DIS was calculated for 134 patients both preoperatively and 2 years after operation, and 112 of these women were also evaluated by bead-…
