[Treatment of perforated duodenal ulcers].

Final surgical treatment of perforated duodenal ulcer is not admitted by all authors. The aim of this study is to evaluate final treatment of perforated duodenal ulcer among 110 patients. 74.5% were aged less than 41 years. 107 patients underwent surgery. Treatment was based on: truncal vagotomy with pyloroplasty (99 cases), truncal vagotomy with closure of duodenal perforation and gastroentero-anastomosis (5 cases), truncal vagotomy with antrectomy (1 case) and simple closure of the duodenal perforation (2 cases). Mortality of truncal vagotomy with pyloroplasty was about 1%. According to the …

Influence of obstructive jaundice on acute gastric ulcer, intragastric pH and potential difference in rats.

To elucidate the influence of obstructive jaundice on the process of acute ulceration of the stomach under stress loading and to investigate the preventative effect of vagotomy, a series of experimental studies were carried out using SD rats. Animals were divided into four groups: Control, jaundice, vagotomy and jaundice with vagotomy. Each group was subjected to the water immersion-restraint stress and changes in the potential difference of gastric mucosa (PD), the intragastric pH (pH) and the score of ulcer index at sacrifice (UI) were measured. The process of acute ulceration under the stre…

[Unsolved problems and prospects in the surgical treatment of duodenal ulcer].

Different variants of vagotomy in combination with pyloroantrumectomy or operations draining the stomach are regarded to be expedient for the treatment of duodenal ulcer instead of the classical resection of the stomach. The selective proximal vagotomy vagotomy is thought to be perspective when used without a drainage of the stomach as an intervention preventing the threat of the development of complications with a chronic course of the disease. In the surgery of complicated duodenal ulcers, specially with high operative risk, it is quite justifiable to use sparing operative interventions such…

Emergency operation for upper gastrointestinal hemorrhage.

From January 1973 through December 1977, 580 patients presented with 624 episodes of upper gastrointestinal hemorrhage at the University fo Pittsburgh Health Center Hospitals. Ninety-one patients (15%) underwent operation for uncontrollable hemorrhage. Operative mortality was 30 per cent for all patients and 21 per cent for patients with gastroduodenal bleeding (duodenal ulcer, gastric ulcer, erosive gastritis). In patients with gastroduodenal bleeding, seven of 15(47%) with preoperative hypotensive shock (systolic b.p. less than or equal to .02). Twenty-five patients had vagotomy and pyloropl…

Dragstedt, gastric acid and duodenal ulcer.

Dragstedt believed that basal hypersecretion of gastric acid was the root cause of duodenal ulcer, that the hypersecretion was due to an increased vagal stimulation, and that vagotomy would therefore cure duodenal ulcer. He introduced vagotomy and demonstrated that the operation was successful in curing most patients of their duodenal ulcers. This article reviews how further research in the succeeding half century has demonstrated that it is the effect of vagotomy on stimulated, rather than upon basal secretion that cures duodenal ulcer and that the apparent basal hypersecretion of patients wi…

[Atropine resistance and its significance in the selection of the method of surgical treatment of duodenal ulcer].

Medical vagotomy with atropine allows two types of patients with duodenal ulcer to be distinguished--atropine-resistant and atropine-sensitive. A complicated course of the disease, high values of nocturnal gastric secretion before the operation, and positive Hollander's insulin test after vagotomy are encountered most frequently in atropine-resistant patients. The values of gastrin are significantly higher in these patients than in atropine sensitive patients. In 3-year follow-up periods, recurrence of the ulcer after vagotomy was revealed in 6.5% of atropine-resistant patients but in none of …

[Operative treatment of massive hemorrhage from duodenal and gastric ulcers (author's transl)].

Considering the different pathogenesis of gastric and duodenal ulcers, it does not seem proper to speak of "gastroduodenal ulcer" and to postulate that bleeding gastic ulcer must be treated by resection only and the bleeding duodenal ulcer by vagotomy. Certainly our catamnestic enquiry shows that the B II gastric resection is favorable with regard to relapse and mortality rates in bleeding gastric ulcer, and the principle of vagotomy is good in bleeding duodenal ulcer; but we are of the opinion that after the introduction of controlled, selective, proximal vagotomy, only this procedure should …

Effect of ionizing radiation on the release of cholecystokinin in the hypothalamus of the rat.

This study was designed to identify the mechanisms underlying the reduction in food intake in rats. Measurements were made of the release of cholecystokinin (CCK) stimulated by potassium chloride in the hypothalamus after (a) gamma irradiation (60Co), (b) treatment with the CCK-A and CCK-B antagonists L-364,718 and L-365,260 with and without radiation, (c) bilateral abdominal vagotomy, and (d) vagotomy with and without radiation and with and without L-364,718. The concentrations of CCK in hypothalamus perfusate were measured by a radioimmunoassay. Exposure of rats to 1, 3, 5 and 10 Gy (1 Gy/mi…

The cephalogastric phase of the pancreatic response to food in the dog.

We studied post-meal pancreatic secretion and gastrin release in conscious dogs with duodenal Thomas cannulas. Normal dogs were tested in physiological conditions and with an i.v. infusion of atropine 20 micrograms/kg/h or secretin 0.5 CU/kg/h. The responses were also studied after antral and truncal vagotomy. In the early phase (0-20 min) of the response, before gastric emptying started, antral vagotomy reduced fluid and protein outputs, and truncal vagotomy reduced them still more. Atropine reduced only the protein response. Gastrin release reached a peak after 20-25 min. After antral and tr…

[Surgical treatment of 100 patients with perforated pyloric ulcer. Immediate and follow-up results].

The aim of this study was to assess the results of different surgical treatments in 100 patients admitted from 1972 to 1984 with perforated pyloric ulcer. Forty-six patients were treated with simple suture, thirty-two patients underwent high selective vagotomy with pyloroplasty, 13 patients were submitted to truncal vagotomy with pyloroplasty and 9 to gastrectomy. This study has shown that high selective vagotomy and pyloroplasty for perforated pyloric ulcer can be performed as safely as simple closure. The overall clinical results according to the Visick classification were recorded as excell…

Sump tube drainage as a source of bacterial contamination.

There is well documented evidence indicating in-appropriately high basal gastrins in patients with duodenal ulcer disease. After stimulation by protein meals, calcium infusion, and insulin-induced hypoglycemia there appears to be an exaggerated release of gastrin in patients with duodenal ulcers compared to control subjects. Vagotomy in general tends to increase serum gastrin by decreasing acid secretion and allowing less inhibition for antral gastrin release. This increase appears less with selective vagotomy and parietal cell vagotomy compared to truncal vagotomy, suggesting vagal inhibition…

Reflex control of the tracheal vasculature of sheep.

Arteries to the cervical trachea were perfused at constant flow in anesthetized sheep. Perfusion pressures (PP), blood pressure (BP), and changes in tracheal smooth muscle tone (Ptr) were measured. Stimulation of pulmonary C-fiber receptors decreased PP (-6.5%) and BP (-16.8%) and increased Ptr (+61.5%), changes prevented by vagotomy and therefore reflex. Stimulation of cardiac receptors and slowly adapting pulmonary stretch receptors decreased PP (-7.9%) and BP (-21.0) and increased Ptr (+19.0%), changes reversed by vagotomy and therefore reflex. Stimulation and inhibition of slowly adapting …

[Experimental studies on the gastroduodenojejunal motility and gastric emptying after duodenectomy with preservation of the total stomach and pylorus].

It is of interest to know whether or not the removal of the pyloric branch of the vagus nerve (subsequently referred to as vagotomy) is associated with the early stagnation of gastric contents supervening upon duodenectomy with preservation of the total stomach and pylorus (innervation of pylorus group 5, denervation of pylorus group 5). In the present study preservative duodenectomy was performed on dogs to determine the effects of vagotomy on gastroduodenojejunal motility and gastric emptying. The group of typical strong preprandial contraction waves (subsequently referred to as phase III) d…

Vagal nerve and the gastric mucosal defense.

An essential role for an intact vagal nerve has been proven in the development of gastric mucosal cyto- and general protection. On the other hand, chemically-induced (ethanol, HCl, indomethacin) gastric mucosal damage is enhanced after acute surgical vagotomy. The aims of this paper were to study the possible mechanisms of the vagal nerve in the development of gastric mucosal defense. The following questions were addressed: 1) effect of surgical vagotomy on the development of ethanol- (ETOH), HCl-, and indomethacin (IND)-induced gastric mucosal damage: 2) changes in the gastric mucosal defense…

Elective surgery for peptic ulcer: a five-year review.

A prospective study of surgery for peptic ulcer was undertaken to assess the current results of surgery in the so-called "post-cimetidine" era. Over a five-year period, 125 patients had undergone surgery; 82 parietal cell vagotomies, 10 vagotomy and drainage procedures, 10 Billroth I gastrectomies, 7 Billroth II gastrectomies, and 15 vagotomy and antrectomy procedures were performed. There were no postoperative deaths. Overall, the outcome was satisfactory in 92 patients, less than satisfactory in 13, and was considered a failure in 14, either because of recurrent ulceration (8%) or because of…

[Effect of the antrum and vagus nerve on the stimulation of the rat stomach by betazol in the perfusion test].

In the perfusion model on Wistar rats the action of a single s. c. injection of betazol (Histalog) in a dose of 50 mg/kg b. w. in dependence on various types of stomach operations was studied. The following effects were observed: 1. the vagotomy shortly before the perfusion test did not reduce the effect of betazol. 2. If the vagotomy dated back at least 4 weeks, a reduction of the stimulated maximum secretion to half of the controls was seen. 3. The combination of the vagotomy with a pyloroplasty led to a reduction of the stimulated maximum acid secretion to half the amount. Futhermore, the b…

Operations for gastric ulcer: a long-term study.

This study assesses the long-term results of operations for benign gastric ulcers. Three-hundred forty-nine patients operated upon between 1950-1979 have been followed over the past 20 years with a mean and median follow-up of 11.8 and 11.1 years. Fifty-five per cent of the patients had a gastric resection without vagotomy; 19.8 per cent had gastric resection with vagotomy; and 20.3 per cent had vagotomy, pyloroplasty, and wedge excision or biopsy of the ulcer. Operations were selected based on the type of ulcer (Types 1-4), whether the surgeon suspected cancer preoperatively, whether the oper…

Surgical management of gastric ulcer.

The charts of 139 patients operated on for benign gastric ulcer between 1976 and 1980 were reviewed. Indications for surgery included failure of medical management, bleeding, perforation and inability to differentiate benign from malignant disease. Surgical management included hemigastrectomy 29%, vagotomy with antrectomy 27%, vagotomy with pyloroplasty 13%, wedge resection 7% and highly selective vagotomy 4%. Eighty-four patients (60%) were available for a minimum 4-year follow-up. Recurrence rates were highest in those treated by highly selective vagotomy (33%) and wedge resection (30%). The…

Lack of evidence for impaired preload or Bezold-Jarisch activation during brief umbilical cord occlusions in fetal sheep.

Impaired cardiac preload secondary to umbilical cord occlusion (UCO) has been hypothesized to contribute to intrapartum decelerations, brief falls in fetal heart rate (FHR), through activation of the Bezold-Jarisch reflex. This cardioprotective reflex increases parasympathetic and inhibits sympathetic outflows triggering hypotension, bradycardia, and peripheral vasodilation, but its potential to contribute to intrapartum decelerations has never been systematically examined. In this study, we performed bilateral cervical vagotomy to remove the afferent arm and the efferent parasympathetic arm o…

An audit of peptic ulcer surgery in Ibadan, Nigeria.

BACKGROUND: Several operations are useful for peptic ulcer surgery, these include Vagotomy and pyloroplasty, Vagotomy and antrectomy, Vagotomy and gastrojejunostomy, and Highly Selective Vagotomy to name a few. Utilising any of these procedures may be due to the operator's preference or more importantly the suitability of the operation to the individual patient. This study was carried out to see the surgical presentation of chronic peptic ulcer disease patients and the form of ulcer-surgery utilised in the University College Hospital Ibadan, Nigeria. STUDY DESIGN: A retrospective study of all…
