Do Implantable Cardioverter-Defibrillators Prevent Sudden Cardiac Death in End-Stage Renal Disease Patients on Dialysis?

Chronic kidney disease patients appear to be predisposed to heart rhythm disorders, including atrial fibrillation/atrial flutter, ventricular arrhythmias, and supraventricular tachycardias, which increase the risk of sudden cardiac death. The pathophysiological factors underlying arrhythmia and sudden cardiac death in patients with end-stage renal disease are unique and include timing and frequency of dialysis and dialysate composition, vulnerable myocardium, and acute proarrhythmic factors triggering asystole. The high incidence of sudden cardiac deaths suggests that this population could ben

Benefits from catheter ablation in patients with pulmonary hypertension: Recent advances.

Supraventricular tachycardia (SVT) is a frequent complication of pulmonary hypertension (PH). The most prevalent SVTs are atrial fibrillation (AF) and typical atrial flutter (AFL), followed by focal and macroreentrant atrial tachycardia (AT) and nodal arrhythmia (AV nodal reentry tachycardia or AV reentry tachycardia). SVT is frequently associated with functional deterioration and right ventricular failure in PH patients. According to some data, reestablishing sinus rhythm is associated with clinical improvement. Catheter ablation of typical AFL, nodal tachyarrhythmias, or other less complex f

A Case of Kawasaki Disease Accompanied by Encephalitis and Several Kinds of Arrhythmia during the Acute Phase.

Although central nervous system complications occasionally accompany during the acute phase of Kawasaki disease, clinically problematic arrhythmia is quite rare. We report a case accompanied by encephalitis and several kinds of problematic arrhythmia. Following the diagnosis of Taussig-Bing anomaly and coarctation of the aorta, the patient underwent aortic arch reconstruction, an arterial switch operation, and ventricular septal defect closure. No significant arrhythmias were observed. At the age of 5 years, the patient presented with a fever, rash, conjunctival hyperemia, and redness of the l

Vernakalant: a new drug to treat patients with acute onset atrial fibrillation.

Vernakalant is a new antiarrhythmic drug that acts selectively in the atrium, targeting atrial specific channels: the Kv1.5 channel which carries IK(ur), and the Kir3.1/3.4 channel which carries IK(Ach). Vernakalant can also work to block Ito, late Ina, with minor blockade of IKr currents. Vernakalant is available in both intravenous and oral forms. In phase II and III trials, intravenous vernakalant has been shown to be effective in terminating acute onset atrial fibrillation whose duration is >3 hours and <7 days (∼50% efficiency vs. 10% for placebo). It does not appear to be effective for a

[Cardiothyrotoxicosis in the young adult in Marrakech. A report of 36 cases].

Thyrotoxicosis is underdiagnosed because of its low occurrence in series from Africa. The aim of this study was to evaluate the frequency, the demographic data, and the etiological aspects of thyrotoxicosis among hyperthyroidy. Thirty-six patients with thyrotoxicosis (group I) gathered during a period of four years was analysed, as well as 180 hyperthyroidy cases (group II). Cardiothyrotoxicosis was observed with a frequency of 16.6%. The mean age was respectively of 44.5+/-13.3 versus 32.8+/-11.4 years (p<10(-6)). Cardiothyrotoxicosis was related to multinodular goitres (18 cases), a Basedow

Atrial pacing therapies for prevention of atrial fibrillation in patients with implantable defibrillators.

Atrial fibrillation (AF), atrial flutter and atrial tachycardia (AT) occur frequently in patients following implantation of an implantable cardioverter defibrillator (ICD) for the treatment of ventricular tachyarrhythmias. Some new generation ICDs have incorporated atrial antitachycardia pacing therapy (ATP) and atrial pacing algorithms designed specifically for the prevention of AF. In the GEM III AT clinical evaluation, atrial ATP efficacy for termination of AF and AT was assessed. Overall ATP efficacy for AF/AT, based on device classification, was 40% when adjusted using the Generalized Est

[A new ICD morphology criterion for differentiating supraventricular and ventricular tachycardia].

The high incidence of inappropriate therapies due to supraventricular tachycardia remains a major unsolved problem of the implantable cardioverter defibrillator. A new morphology discrimination (MD) algorithm has been introduced to improve specificity of ICD therapy without loss of sensitivity. It was the aim of this study to systematically analyze sensitivity and specificity of the MD criterion in combination with the enhanced detection criteria sudden onset and rate stability in the detection of ventricular and supraventricular tachycardia. After ICD implantation in 259 patients, 787 detecte

[Modern therapy of paroxysmal supraventricular tachycardia].

Paroxysmal supraventricular tachycardia (SVT) may have numerous electro-physiologic mechanisms. The most common type of SVT is AV-nodal reentry tachycardia (60%) followed by the bypass tract-mediated SVT (preexcitation. 30%) and a smaller group (10%) comprising paroxysmal atrial flutter or fibrillation and atrial ectopic tachycardia. In persons with otherwise normal hearts symptoms are usually mild and include palpitations or an uneasy feeling in the chest. But some describe precordial pain. Weakness, dizziness, nausea, vomiting, and even syncope. Whenever possible a 12-lead-ECG during an epis

The effect of magnesium versus verapamil on supraventricular arrhythmias.

Magnesium has previously been used in the treatment of various arrhythmias, but few randomized and prospective studies are available. In a single-blind study, the efficacy and safety of intravenous magnesium sulfate (bolus doses of 5 + 5 mmol followed by infusion of 0.04 mmol/min) versus verapamil (5 + 5 mg followed by 0.1 mg/min) was evaluated in 57 patients with supraventricular arrhythmias (supraventricular tachycardia, atrial fibrillation, and atrial flutter) of recent onset (less than 1 week). Fifteen (58%) of the patients receiving magnesium (n = 26) converted to sinus rhythm within 4 h,

[Dysfunction of Carpentier-Edwards porcine mitral bioprosthesis causing a musical systolic murmur: a report of two cases].

Two patients with disruption of Carpentier-Edwards porcine mitral bioprosthesis presenting with a musical systolic murmur were reported. The two patients noted sudden development of a loud musical murmur 4 (Case 1) and 6.5 (Case 2) years after surgery. Case 1 had associated heart failure. Phonocardiogram disclosed a loud musical systolic murmur with regular vibrations at 230-240 Hz in Case 1, and 250-260 Hz in Case 2. In Case 1, echocardiography demonstrated fine fluttering of the porcine mitral valve at the same frequency as the murmur. Doppler echocardiography revealed characteristic Doppler

Loss of His-bundle and Right Ventricular Septal Capture Following Transcatheter Aortic Valve Replacement-A Case Report.

Conduction abnormalities following transcatheter aortic valve replacement (TAVR) are common. High-grade atrioventricular block (AVB) and new-onset left bundle branch block remain the most reported. These often require the placement of a permanent pacemaker (PPM). His-bundle (HB) pacing is increasingly being utilized as the preferred mode of ventricular pacing due to its more physiologic ventricular activation. In this case report, we present a case of a patient who developed loss of HB capture and experienced an increase in the local right ventricular (RV) capture threshold after TAVR that led

Safety and risk/benefit analysis of ibutilide for acute conversion of atrial fibrillation/flutter.

Safety data were reviewed from several controlled clinical trials of ibutilide, a new class III antiarrhythmic drug recently approved for the acute interruption of atrial fibrillation and flutter. Noncardiovascular adverse effects of ibutilide were similar in frequency to those with placebo. Cardiovascular adverse effects occurred in 24.9% of 586 ibutilide-treated patients as compared with 22.2% of 108 sotalol-treated patients, and 7.1% of 127 patients who received placebo. Polymorphous ventricular tachycardia, diagnosed as torsades de pointes, was more common with ibutilide than with placebo

Cavotricuspid isthmus dependent flutter is associated with an increased incidence of occult coronary artery disease.

AIMS: Atrial flutter (AFl) and atrial fibrillation (AFib) share many clinical risk factors and potential mechanisms with atherosclerosis. Despite this, an association between stable coronary artery disease (CAD) and atrial arrhythmias has not previously been documented. To investigate this hypothesis we measured the incidence of occult coronary atheroma on coronary angiography inpatients undergoing radiofrequency ablation procedures. METHODS AND RESULTS: Consecutive coronary angiograms performed on patients with no history or symptoms of CAD undergoing elective ablation of arrhythmias were an

In vitro and in vivo evaluation of the thermal patterns and lesions of catheter ablation with a microwave monopole antenna.

BACKGROUND: Radiofrequency (RF) catheter ablation is an effective treatment for supraventricular tachycardia. The effectiveness of the technique is at times limited by the small lesion size produced by RF energy delivery. Previous reports have indicated that microwave energy is capable of producing a larger volume of heated tissue than radiofrequency energy, raising the possibility that microwave energy may offer a potential alternative energy source to radiofrequency for the substrate ablation of certain arrhythmias such as ventricular tachycardia or atrial flutter. METHODS: The present stud

Clinical review of radiofrequency catheter ablation for cardiac arrhythmias.

Clinical tachycardias are a major cause of morbidity with detrimental effects on quality of life, physical activity and health care costs. Catheter ablation delivered by radiofrequency energy (RFA) has gradually expanded as a therapeutic modality for cure or palliation and is being adapted to address the most difficult to treat tachycardias. The purpose of this paper is to inform decision makers about the current evidence base of RFA through a comprehensive literature review. Of the 968 citations identified through the literature search strategy, 111 studies (11%) met the inclusion criteria. O
