Double-chambered right ventricle in a patient with pulmonary atresia and ventricular septal defect.

Double-chambered right ventricle is a rare form of right ventricular outflow tract obstruction caused by anomalous hypertrophy of muscle bundles in right ventricle. Cases most often occur in children and rarely in adults. Most cases (80-90%) are associated with ventricular septal defect. We describe a case of pulmonary atresia and ventricular septal defect with double-chambered right ventricle. The interesting clinical findings, ECG, echocardiography and angiocardiography features are described here.

Subaortic obstruction and complete atrioventricular block in Behçet's disease.

Left ventricular outflow tract obstruction may be dynamic, most commonly associated with hypertrophic cardiomyopathy, and, uncommonly, by congenital anomalies such as discrete subaortic stenosis. We describe a patient with Behçet's disease, presenting with a systolic murmur, fever, and syncope, in whom a diagnosis of subaortic obstruction caused by a pseudo-aneurysm dissecting the interventricular septum and associated with a complete atrioventricular block was made.

Progressive Right Ventricular Obstruction Caused by a Double-Chambered Right Ventricle Resulting in Shunt-Reversal via a Concomitant Congenital Ventricular Septal Defect and Subsequent Erythrocytosis in a Dog.

A 3-year-old Chihuahua was presented because of exercise intolerance, respiratory distress, and syncopal episodes. At the age of 10 weeks, the dog was diagnosed with a congenital small left-to-right shunting ventricular septal defect and a mild right ventricular outflow tract obstruction via echocardiography. At that time, the dog was asymptomatic, but the breeder's veterinarian heard a murmur. Both cardiac defects were judged to be clinically non-relevant at that time. However, at 3 years of age, echocardiography revealed a severe right ventricular obstruction, known as a double-chambered rig

CCTA of Pediatric Congenital Right Heart Obstructive Lesions: A Pictorial Review.

Pediatric congenital right heart obstructive lesions encompass a spectrum of diseases that obstruct blood flow from the right atrium to the pulmonary artery. Right ventricular inflow obstructions include tricuspid valve abnormalities, such as Ebstein anomaly, tricuspid valve dysplasia, and tricuspid atresia. Right ventricular outflow obstructions include pulmonary valve stenosis, pulmonary atresia, and tetralogy of Fallot. Cardiac computed tomography angiography (CCTA) is a valuable tool for the diagnosis, treatment planning, and follow-up of these lesions. In this pictorial review, we highlig…

Surgical repair of double-chambered right ventricle with or without ventriculotomy.

Double-chambered right ventricle is an uncommon congenital anomaly consisting of obstruction of the trabecular zone of the ventricle by an elevated, hypertrophied moderator band, with or without additional muscle bundles(s). During an 18 month period (August 1984 to February 1986) 12 patients underwent repair of double-chambered right ventricle. Their ages ranged from 13 months to 12 years (mean 4.4 years). Associated lesions were ventricular septal defect in 11 patients (three closed spontaneously), atrial septal defect in two, left superior vena cava in three, aortic valve prolapse in one, a

Mortality and complications in 3495 children with isolated ventricular septal defects.

BACKGROUND: Ventricular septal defects (VSDs) are the most common congenital heart defects (CHDs). Previous studies indicate an increased risk of endocarditis, aortic regurgitation, left ventricular outflow tract obstructions, pulmonary hypertension, arrhythmias and sudden death in patients with isolated VSDs. The present nationwide cohort study reports mortality and cardiac complications requiring hospitalisation or intervention in children with isolated VSDs. METHODS AND RESULTS: Medical information concerning all 943 871 live births in Norway in 1994-2009 was retrieved from the Medical Bir

Complex management of Fabry cardiomyopathy: a case report on the use of alcohol septal ablation and chaperone therapy.

BACKGROUND: Cardiac involvement in Fabry disease (FD) usually manifests as a concentric left ventricular hypertrophy with rare cases developing left ventricular outflow tract obstruction (LVOTO), symptoms varying from fatigue and exercise associated dyspnoea to angina or arrhythmias. CASE SUMMARY: We present the case of a 54-year-old man with cardiovascular risk factors and aggravated exertional dyspnoea in the past year, in whom the echocardiography showed hypertrophic obstructive cardiomyopathy (HOCM). Cardiac magnetic resonance was used as differential diagnosis tool between sarcomeric HOC

Obstructive accessory mitral valve tissue in an adult patient: a case report.

BACKGROUND: Accessory mitral valve tissue is a rare congenital anomaly that is commonly diagnosed in early childhood and rarely in adulthood. It is usually asymptomatic. However, it may cause left ventricular outflow tract obstruction in a way that mimics various other causes of obstruction. A 72-year-old Caucasian man complained of chest discomfort and exertional dyspnea for 3 months. There were no specific findings from a physical examination except systolic murmur. Transthoracic echocardiography demonstrated a mass on the mitral valve extending to the intraventricular septal, raising the pr

Left Anterior Mini-Thoracotomy for Adult Complicated Subarterial Ventricular Septal Defect.

Lack of consensus remains for the minimally invasive surgical repair of adult subarterial ventricular septal defects, and the technique has rarely been reported for cases complicated by other cardiac anomalies. We describe a left anterior mini-thoracotomy through the third intercostal incision in an adult woman for combined subarterial ventricular septal defect repair and right ventricular outflow tract obstruction relief. We consider this minimally invasive technique a balanced approach for correction of a complicated subarterial ventricular septal defect in adults.

Extended aortic root replacement with aortic allografts.

Complex left ventricular outflow tract obstruction after operation for subaortic stenosis or with hypoplastic aortic anulus remains a challenge for pediatric cardiac surgeons. We have recently applied a new technique of extended aortic root replacement using a cryopreserved aortic allograft to treat two patients who had previously been operated on for subaortic stenosis and a third who had aortic stenosis with a hypoplastic aortic anulus. This new procedure combines the concept of aortoventriculoplasty with aortic root replacement and coronary artery reimplantation. The valved aortic homograft

Obstruction within the right ventricular body: two-dimensional echocardiographic features.

Real-time 2-dimensional (2-D) echocardiograms were performed in 4 patients, all with considerable obstruction within the right ventricular (RV) body, in 3 patients due to muscle bundles and in 1 due to hypertrophic cardiomyopathy (HC). None had obstruction at the pulmonic valve level or immediately below. Echocardiographically, obstruction presented in 3 patients as localized bulging of the RV free wall in the subinfundibular region such that the RV cavity presented a typical "hourglass" appearance. Associated ventricular septal defect (VSD) was detected in 2 patients by echocardiography. In t

Aneurysm of the membranous ventricular septum in transposition of the great arteries.

In patients with transposition of the great arteries (TGA), both the D- and L- forms, an aneurysm of the membranous ventricular septum (AMS) produces subpulmonic stenosis due to the higher right ventricular pressure which forces the aneurysm to protrude into the left ventricular, i.e., subpulmonic, outflow tract. The clinical signs and symptoms, hemodynamic findings as well as surgical results were analyzed in eight patients with TGA and AMS. The presence of an AMS should be suspected from hemodynamic data consisting of a combination of elevated left ventricular pressure, gradient across the l

Tetralogy of Fallot with subarterial ventricular septal defect. Diagnostic and surgical considerations.

Since 1964, 77 patients underwent repair for tetralogy of Fallot with subarterial ventricular septal defect. Median age at operation was 5 years. Cyanosis was commonly mild, and hypoxic episodes were infrequent. Accordingly, only 12 patients (15.58%) needed either palliative or corrective operations before the age of 2 years. Characteristic angiographic and echocardiographic features were observed, which allowed this entity to be differentiated from either classic tetralogy or other types of double-outlet right ventricle. The earliest series of operations (in which no right ventricular outflow

Dynamic left ventricular outflow tract gradient resulting from Takotsubo cardiomyopathy ameliorated by intra-aortic balloon pump counterpulsation: a case report.

BACKGROUND: Takotsubo cardiomyopathy is a variant of acute coronary syndrome with characteristic acute left ventricular apical ballooning. Uncommonly, there can be associated left ventricular outflow tract (LVOT) obstruction causing cardiogenic shock refractory to inotropic support. The use of afterload-reducing mechanical support such as intra-aortic balloon pump (IABP) counterpulsation is not routinely employed in instances of this kind. CASE SUMMARY: In our case report, we describe a 66-year-old female with acute Takotsubo cardiomyopathy and associated LVOT obstruction which failed to resp

Morphologic determinants of hemodynamic state after ventricular septal myotomy-myectomy in patients with obstructive hypertrophic cardiomyopathy: M mode and two-dimensional echocardiographic assessment.

To define the morphologic features of the left ventricle after ventricular septal myotomy-myectomy and to elucidate the structural changes associated with a postoperative reduction in the pressure gradient, 28 patients with obstructive hypertrophic cardiomyopathy were studied with M mode and qualitative and quantitative two-dimensional echocardiography. Nine patients with a marked reduction in the pressure gradient (no or small, less than or equal to 25 mm Hg, residual basal gradient) demonstrated a marked reduction in septal thickness after surgery (23 +/- 6 to 13 +/- 4 mm; p less than .01),

Repeated Systolic Anterior Motion of the Mitral Valve After Double Outlet Right Ventricle Repair.

We herein report a rare case of double outlet right ventricle repair complicated by repeated systolic anterior motion of the mitral valve after weaning from cardiopulmonary bypass and postoperatively. Even in patients without obvious preoperative left ventricular outflow tract obstruction, systolic anterior motion and mitral regurgitation may still occur from functional factors, such as intraoperative or postoperative inotropic drug use, tachycardia, or intraventricular volume loss in patients with severe ventricular septal hypertrophy and left ventricular hypertrophy.

Fabry's disease with LVOT obstruction: diagnosis and management.

A 46-year-old male was diagnosed with hypertrophic cardiomyopathy (HCM) and severe left ventricular outflow tract (LVOT) obstruction. Genetic testing revealed that this was a "mimic of HCM" and the true diagnosis was Fabry's disease, although there were no other clinical features of Fabry's. Despite maximal medical therapy he remained symptomatic from the outflow tract gradient and required surgical myectomy for symptom relief. This case illustrates the diagnosis of an HCM mimic by genetic testing.
