Durability of the Arterial Switch Operation: Long-Term Survival and Structural Sequelae in a Multicentre Cohort.

BACKGROUND: The arterial switch operation (ASO) is the standard repair for complete transposition of the great arteries (TGA), but long-term data across large contemporary cohorts remain limited. In this study we evaluated 30-year survival, reintervention, and structural sequelae after ASO and identified factors that influenced late outcomes. METHODS: A total of 1125 patients, 652 with TGA with intact ventricular septum, 389 with TGA with ventricular septal defect, and 84 with double-outlet right ventricle with Taussig-Bing anomaly who underwent ASO between 1990 and 2015 at 10 tertiary centre

Surgical approach to critical pulmonary valve stenosis in infants less than six months of age.

During the past 7 1/2 years, 36 infants (less than or equal to 6 months of age) underwent surgical relief of critical pulmonary valve stenosis. The results of closed and open (cardiopulmonary bypass, CPB) procedures were compared. Closed procedures included Brock valvotomy (two patients) and normothermic inflow stasis with transpulmonary valvectomy (10 patients). Twenty-two patients had open procedures with valvectomy alone (five patients) and right ventricular outflow tract (RVOT) reconstruction (17 patients). Two additional patients had staged operations (shunt followed by open procedure at

Late assessment after biventricular repair for isomerism heart.

BACKGROUND: Although biventricular repair is theoretically ideal for the treatment of isomerism heart, the long-term outcome is unknown. We assessed the outcome of biventricular repair for atrial isomerism. METHODS: From 1984 to 2002 in our surgical database, 10 of 67 patients with atrial isomerism received biventricular repair. The age at operation was 7.7 +/- 9.1 years. Preoperative ventricular volume was normal in all patients. Left ventricular ejection fraction was 62% +/- 8%. Intra-atrial rerouting was required in 8. The atrioventricular septal defect with double-outlet right ventricle w

Taussig-Bing anomaly and arterial switch: aortic arch obstruction does not influence outcome.

OBJECTIVE: Aortic arch obstruction is a commonly associated problem in the Taussig-Bing anomaly. Between 1983 and 1995, 28 consecutive patients with Taussig-Bing anomaly underwent arterial switch operation with baffling of the left ventricle to neoaorta. Group A: 11/28 had associated aortic arch obstruction. Group B: 17/28 had isolated Taussig-Bing anomaly. We assessed whether the coexistence of subpulmonary ventricular septal defect and aortic arch obstruction pose an incremental risk factor. PATIENTS AND RESULTS: Group A: Mean age and weight were 1.4+/-1.3 months and 3.5+/-0.4 kg. The aorti

Exposure to Endocrine-Disrupting Chemicals and Congenital Heart Diseases: The Pooled Results Based on the Current Evidence.

The relationships between maternal exposure to endocrine-disrupting chemicals (EDCs) and congenital heart diseases (CHD) are not elucidated yet. The exposure levels of EDCs are generally estimated based on self-reported questionnaires or occupational exposure evaluations in the literature. Therefore, a study based on epidemiological data from human biospecimens is required to provide stronger evidence between maternal exposure to EDC and CHD. Embase, Pubmed, Scopus, and the Cochrane Library databases were searched for related research which provided risk estimates regarding the relationships b

Congenitally Corrected Transposition of the Great Arteries: Fetal Diagnosis, Associations, and Postnatal Outcome: A Fetal Heart Society Research Collaborative Study.

Background Fetal diagnosis of congenitally corrected transposition of the great arteries (ccTGA) has been increasingly reported; however, predictors of clinical outcomes remain underexplored. We undertook a multicenter, retrospective study to investigate natural history, associated anomalies, and outcomes of fetal ccTGA. Methods and Results Fetuses with ccTGA diagnosed from January 2004 to July 2020 within 20 North American programs were included. Fetuses with severe ventricular hypoplasia thought to definitively preclude biventricular repair were excluded. We included 205 fetuses diagnosed wi

Metastatic Renal Cell Carcinoma Presenting as an Intracardiac Tumour Without Involving the Inferior Vena Cava.

Renal cell carcinoma (RCC) is an aggressive tumour, with 25% of the cases presenting with distant metastases at the time of diagnosis. Approximately 33% of the patients with RCC eventually develop metastatic spread. RCC can metastasize to various sites including the lung, liver, bone, brain, adrenal gland, and more. Cardiac metastasis is rare in RCC, but even rarer in the absence of inferior vena cava (IVC) involvement. This case report presents a 60-year-old male patient who was referred by his general practitioner due to breathing difficulties. An initial echocardiogram revealed a right vent

[Valvular diseases of the right heart].

Recently, the development of diagnostic and operative interventional techniques has contributed to the successful treatment of congenital valve lesions of the right heart. The clinical picture, diagnostic approaches, and therapeutic outcome are presented. In pulmonic valve stenosis the fusion of leaflets is most common. Stenosis severity corresponds to the intensity of the systolic murmur. Pressure gradients can be quantitated by Dopplersonography. Balloon valvuloplasty is the method of choice for treatment. Residual gradients (24-36 mmHg) are not significantly different from surgical results.

11-Year follow-up of percutaneous pulmonary valve implantation (PPVI) in central South Africa.

BACKGROUND: Excellent mid-term outcomes are well-established for percutaneous pulmonary valve implantation (PPVI) in treating right ventricular outflow tract (RVOT) dysfunction. Despite this success, long-term data assessing PPVI performance are limited, especially within middle- to low-income country contexts. OBJECTIVE: To report on the patient characteristics and long-term outcomes after PPVI. METHODS: This retrospective study describes outcomes for 31 PPVI patients treated from 2012 to 2023 at the paediatric cardiology department, Universitas Academic Hospital, Bloemfontein, South Africa

Combined cox maze procedure, septal myectomy, and mitral valve replacement for severe hypertrophic obstructive cardiomyopathy complicated by chronic atrial fibrillation.

Atrial fibrillation (AF) has been reported to be an important prognostic indicator for clinical deterioration particularly in patients with hypertrophic obstructive cardiomyopathy (HOCM). A 66-year-old female complained of severe exertional dyspnea and tachycardia, which were resistant to medical treatments. Doppler echocardiography demonstrated a peak left ventricular outflow tract (LVOT) gradient of 117 mmHg at rest. A catheter examination revealed left ventricular end diastolic pressure of 34 mmHg, a cardiac index of 1.94 L/minute/m(2), and a peak LVOT gradient of 70 mmHg at rest. A transao

Diagnostic challenge of mitral regurgitation caused by concomitant coronary obstruction and dynamic left ventricular outflow tract obstruction after transcatheter aortic valve replacement: a case report.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) is an established therapy for severe aortic stenosis (AS), but rare complications can rapidly become life-threatening, particularly in anatomically high-risk patients. CASE SUMMARY: A 94-year-old woman with very severe AS underwent transfemoral TAVR using a self-expanding valve. After transcatheter heart valve implantation, she developed haemodynamic collapse with right coronary artery obstruction and severe mitral regurgitation (MR). At that time, the aetiology of the severe MR could not be clearly identified. Although intra-aortic ba

A Ball Valve Thrombus in a Young Obese Male With Hyperlipidemia.

A ball valve thrombus is a common entity in rheumatic mitral stenosis where a large clot forms in the left atrial appendage, the clot detaches from the left atrial appendage, swirls in the left atrium in a clockwise manner, obstructs the mitral inflow producing cyclic acute pulmonary edema, and likely causes left ventricular outflow tract obstruction producing transient syncope or embolism to the brain producing dense hemiplegia. Besides rheumatic mitral stenosis, ball valve thrombus has been described in post-mitral valve replacement patients and restrictive cardiomyopathy. Left atrial myxoma

Complete percutaneous repair of Tetralogy of Fallot in adult: a case report.

Tetralogy of Fallot (TOF) is the most common congenital cyanotic heart disease and is characterized by an antero-superior deviation of the infundibular septum with a consequent large malaligned ventricular septal defect (VSD) and a pulmonary and sub-pulmonary (infundibular) stenosis. Surgical repair has been the cornerstone of treatment that is electively performed early in their lives between 3 and 6 months of age. With advancements in transcatheter interventions, the complete percutaneous repair of TOF, a complex disease with multiple treatable lesions, is becoming a conceivable possibility.

A Redundant Tricuspid Valve Leaflet in a 23-Year-Old Female: A Report of a Rare Case and a Literature Review.

Accessory tricuspid valve (ATV) is a rare cardiac anomaly often associated with other complex cardiac disorders, most commonly tetralogy of Fallot (TOF) and ventricular septal defect (VSD), but it can also be an isolated entity. It is usually diagnosed in childhood. It can be asymptomatic and get diagnosed incidentally, but it can also lead to cardiac complications such as right ventricular outflow obstruction (RVOTO) or tricuspid valve regurgitation. Here, we present the case of a 23-year-old woman with an isolated ATV diagnosed on an echocardiogram without symptoms after a physical exam note

Combined semilunar valve stenoses in neonates: management approaches and literature review.

Combination of right and left ventricular outflow tracts obstruction is extremely rare. Neonates with combined aortic stenosis (AS) and pulmonary stenosis (PS) present in critical condition and required urgent treatment. The management approach is not well defined. We report five female neonates with combined AS and PS presented to our institute in the last 5 years, age (1-18 days), weight (2.2-3.4 kg). Two had associated muscular ventricular septal defects. The mean Doppler gradient across the aortic valve (AV) was 73 mmHg (53-105 mmHg) and across the pulmonary valve was 62 mmHg (44-76 mmHg).
