Labyrinthine fistula in chronic mastoiditis.

A series of 79 otic capsule fistulae occurring in 792 consecutive cases of chronic mastoiditis undergoing mastoidectomy was analyzed. The incidence (10%) is the same as in other large series, the primary difference being the location of the fistula. In all series cited, 90% of fistulae were in the lateral semicircular canal whereas in this series the comparable incidence was 75%. The 20 (25%) fistulae located in areas other than the lateral semicircular canal enabled us to document a method of "reading" the fistula test preoperatively to establish its precise location. Eye responses to the tes

Laser stepedotomy for otosclerosis.

The argon laser microscope recently developed by the author is used to vaporize the stapes tendon, the posterior crus and a rosette of holes in the stapes footplate in the surgical treatment of otosclerosis. An autogenous vein--stainless steel piston assembly is used to reconstruct the stapes portion of the ossicular chain. The surgical technique and results in a preliminary series of 11 patients are reported. Rationale and advantages over conventional stapedectomy are discussed.

Impact of an Endolymphatic Sac Tumor on Subjective and Objective Audiovestibular Function.

OBJECTIVE: This study reports subjective and objective audiovestibular function in patients diagnosed with an endolymphatic sac tumor (ELST), to evaluate the clinical implication of vestibular testing in ELST patients. STUDY DESIGN: Retrospective cohort study. SETTING: Tertiary referral center/university hospital. PATIENTS: Five patients diagnosed with unilateral ELST. INTERVENTION: Subjective and objective audiovestibular tests; pure-tone average; speech discrimination score; the Video Head Impulse Test; the cervical vestibular evoked myogenic potential test; the caloric irrigation test;

The effect of unilateral posterior semicircular canal inactivation on the human vestibulo-ocular reflex.

The responses to rapid, passive, unpredictable, low amplitude (10-20 degrees), high acceleration (3,000-4,000 degrees/s2) head rotations were used to study the human vestibulo-ocular reflex (VOR) in pitch and yaw plane after unilateral posterior semicircular canal occlusion (uPCO) in 10 subjects. The results from these 10 uPCO subjects were compared with those from 18 normal subjects. The VOR gains at a head velocity of 200 degrees/s in the uPCO subjects were: pitch upward = 0.62 +/- 0.06, pitch downward = 0.87 +/- 0.11, yew ipsilesion = 0.78 +/- 0.06, yaw contralesion = 0.79 +/- 0.10 and in n

A multimodal data-based diagnostic model for predicting vestibular migraine: a retrospective study.

OBJECTIVE: Vestibular migraine (VM) is a common neurological disorder characterized by recurrent vertigo and migraine symptoms. Due to its h … 1 VM patients and 147 non-VM controls. Multimodal data were collected, including clinical features, vestibular function tests, hematological indicators, contrast transthoracic echocardiography, and psychological assessments. Logistic regression was used to construct the prediction model, and its performance was evaluated using receiver operating characteristic (ROC) curve …

[Vestibular function of middle-aged persons exposed to 30-day head-down tilt hypokinesia].

Vestibular responses of 15 men (aged 42-50) to 30-day head-down tilt (-8 degrees) were investigated. The test subjects showed atherosclerotic symptoms and neuro-circulatory dystonia of the hypertensive type. They were exposed to the Fitzgerald-Hallpike caloric test, indirect otolithometry (eye counter-rolling reflex according to the method of successively presented images), and motion sickness according to Bryanov. In the pretest period vestibular changes were seen at the level of labyrinths and central formations (change in the nystagmic pattern, dissociation of the components of the caloric

Treatment of acute vestibular neuronitis with glucocorticoids.

OBJECTIVE: To report the effects of glucocorticoid treatment of acute vestibular neuronitis on recovery of vestibular function and length of hospital stay. STUDY DESIGN: Prospective, consecutive case series compared with historic controls. SETTING: Secondary referral academic hospital. PATIENTS: Patients with acute vestibular neuronitis. One group treated with glucocorticoids within 3 days after symptom onset (n = 33) and 2 historic untreated control groups (n = 41 and n = 67). INTERVENTIONS: Oral prednisolone 50 mg/d for 5 days with tapering of doses for the next 5 days, alternatively wit

The effects of cochlear implantation on vestibular function.

OBJECTIVE: Determine the risk posed by cochlear implantation (CI) to the labyrinth. STUDY DESIGN: Prospective cohort study. SETTING: Academic tertiary referral center. PATIENTS: Thirty-six ears belonging to 35 adult CI candidates (mean age, 46 yr; range, 23-69 yr). INTERVENTION: Cochlear implantation. MAIN OUTCOME MEASURES: Vestibular function was assessed using the quantitative 3-dimensional head impulse test (qHIT), clinical head impulse test (cHIT), post-head shake nystagmus, caloric electronystagmography, vestibular-evoked myogenic potentials, dynamic visual acuity, and Dizziness Hand

[Application of 3D-Flair MRI and vestibular function assessment in profound sudden sensorineural hearing loss patients].

Objective: To analyse the 3D-Flair MRI manifestations of the inner ear, vestibular function status, and their correlation with hearing treat … tter than those with dizziness/vertigo (P<0.05). Conclusions: The 3D-Flair MRI of the inner ear and vestibular function tests have reference value for the prognosis assessment of patients with severe sudden sensorineural hearing loss. Abnormal 3D-FLAIR MRI of the inner ear, especially absorption high signal, is associated with high incidence of vestibular dysfunction and d …
