Epidemiological, Clinical, and Virological Characterizations of Yezo Virus Infections, an Emerging Tick-borne Orthonairovirus Disease in Japan.

BACKGROUND: Febrile disease caused by infection with Yezo virus (YEZV), a tick-borne orthonairovirus, was first reported in Japan in 2021 and subsequently identified in China. While recent studies suggest a broader endemic area of the emerging YEZV infections, its clinical and epidemiological characteristics remain poorly elucidated due to the limited number of reported cases. METHODS: We conducted retrospective and prospective surveillance of patients with suspected tick-borne febrile illnesses in Hokkaido, Japan, between 2013 and 2024. Serum samples were analyzed by quantitative reverse tra

Post-infection immune response in adults with COVID-19 with and without nirmatrelvir-ritonavir treatment and virologic rebound.

BACKGROUND: Nirmatrelvir-ritonavir (N-R) reduces morbidity and mortality from COVID-19 in high-risk individuals; however, N-R use has been associated with risk of SARS-CoV-2 virologic rebound. The mechanisms contributing to virologic rebound after N-R treatment are currently unknown. One plausible mechanism is that antiviral treatment may alter the development of immune responses to SARS-CoV-2 infection, thereby contributing to rebound after cessation of therapy. METHODS: We profiled immune responses in a case-ascertained, longitudinal, prospective cohort of ambulatory individuals with COVID-

Dynamics of virological suppression and systemic immune-inflammatory response following antiretroviral therapy in pediatric HIV infection.

BACKGROUND: Persistent systemic inflammation remains a hallmark of treated HIV infection and is not reliably resolved by virological suppression alone. The Systemic Immune-Inflammation Index (SII), a composite hematologic index [(neutrophil × platelet)/lymphocyte count], has emerged as a candidate inflammatory biomarker. Longitudinal SII data following antiretroviral therapy (ART) initiation in pediatric HIV are lacking. METHODS: This single-center retrospective paired longitudinal study included 30 pediatric patients with HIV with laboratory data at baseline and 12 months post-ART. The prima

Initiation of Long-Acting Antiretroviral Combinations Achieve Sustained Virologic Remission in Early-Life SIV Infection.

Long-acting (LA) antiretroviral agents offer a promising strategy to overcome adherence challenges in HIV prevention and treatment. However, their efficacy in early life has not been evaluated. Using a highly translational newborn macaque model of simian immunodeficiency virus (SIV) infection, we assessed the preventive and therapeutic potential of LA lenacapavir (LEN), alone or in combination with cabotegravir (CAB). Despite its efficacy in adult pre-exposure prophylaxis setting, early LEN monotherapy failed to prevent viral acquisition following intravenous SIV exposure at birth and resulted

Liver reprogramming toward hepatocellular carcinoma following hepatitis C sustained virologic response: a narrative review.

BACKGROUND AND OBJECTIVE: Despite direct-acting antiviral (DAA) therapies achieving sustained virologic response (SVR) rates exceeding 95%, hepatocellular carcinoma (HCC) risk persists in cured hepatitis C virus (HCV) patients, particularly those with advanced fibrosis or concurrent metabolic dysfunction-associated steatotic liver disease (MASLD). This clinical paradox implies that HCV induces durable oncogenic molecular alterations independent of active viral replication, fundamentally challenging the notion that virologic cure equates to biological liver cure. This review aims to delineate t

A prediction model for virologic failure in adolescents living with HIV in Uganda: Findings from the Suubi+Adherence study.

OBJECTIVES: Adolescents living with HIV (ALHIV) have low viral suppression levels, with 1 in 3 ALHIV experiencing virologic failure, calling for more efforts to reverse these trends. We developed and validated a model that predicts the risk of virologic failure (VF) among ALHIV. STUDY DESIGN: Cross-sectional study. METHODS: We used baseline data from 702 ALHIV enrolled in the Suubi + Adherence cluster-randomized clinical trial. Participants were aged 10-16 years, living with HIV and aware of their HIV status, and are living with a family. We developed a risk-prediction model for VF (viral lo

Genetic alterations in hepatocellular carcinoma after sustained virological response in relation to the molecular characterization of metabolic diseases.

AIM: The mechanism of hepatocarcinogenesis after sustained virological response (SVR) in hepatitis C virus (HCV) patients is unclear. We compared gene profiles of hepatocellular carcinoma (HCC) between HCV-SVR, steatotic liver disease (SLD), and HCV-non-SVR patients. METHODS: This study analyzed 126 resected HCCs from patients with HCV and SLD, classifying them as HCV-SVR (n = 22), HCV-non-SVR (n = 56), and SLD (n = 48). Deep sequencing of 2910 hotspots in 55 cancer-related genes was conducted to examine mutations and copy number variations in both cancerous and background liver tissues. RES

Hepatic Ectopic Adrenal Gland in a Patient with Hepatitis C Virus Who Achieved a Sustained Virological Response.

Hepatic ectopic adrenal glands are rare and can mimic hepatocellular carcinoma (HCC). We herein report a 60-year-old woman with a history of chronic hepatitis C who achieved a sustained virological response. A 23-mm hepatic lesion was detected and initially diagnosed as HCC. However, laparoscopic resection and histopathology confirmed the presence of an ectopic adrenal gland. Given the location of the posterior liver, a biopsy is challenging. Adrenal scintigraphy may aid in the diagnosis and reduce the need for invasive procedures. As more cases are reported, improved imaging-based diagnoses m

Characterizing treatment interruptions in the OPERA cohort and virologic outcomes after resumption with bictegravir/emtricitabine/tenofovir alafenamide.

BACKGROUND: Despite advancements in antiretroviral therapy (ART) for people with HIV, barriers to adherence remain, potentially leading to long-term gaps in adherence known as treatment interruptions. These treatment interruptions are associated with viral rebound and can impact the effectiveness of the subsequent regimen and the long-term health of the individual. We aimed to characterize unplanned ART treatment interruptions in the OPERA® cohort and investigate virologic outcomes among individuals who resumed treatment with bictegravir/emtricitabine/tenofovir alafenamide (B/F/TAF). METHODS:

Analysis of Repeat Hepatitis C Viremia After Sustained Virologic Response in a Large Cohort of US Veterans.

BACKGROUND: Chronic hepatitis C virus (HCV) infection affects >1% of the US population and a larger percentage of US veterans. Direct-acting antiviral (DAA) medications achieve viral cure, termed sustained virologic response (SVR), but repeat viremia after SVR is recognized. Prior work has been limited by electronic medical record data. We aim to better understand repeat viremia in the DAA era through detailed chart review. METHODS: We identified 1129 individuals from the Veterans Health Administration (VHA) who achieved SVR using DAA therapy but subsequently had detectable HCV viremia. A ran

The clinical challenge of managing patients after sustained virological response with direct-acting antivirals for Hepatitis C.

PURPOSE OF REVIEW: This review highlights the critical considerations for monitoring patients who achieve sustained virological response (SVR) after direct-acting antiviral (DAA) therapy for hepatitis C virus (HCV) infection. Despite the remarkable success of DAAs, challenges persist in managing long-term risks, including hepatocellular carcinoma (HCC), liver decompensation, and extrahepatic manifestations, necessitating a tailored follow-up approach. RECENT FINDINGS: Recent studies emphasize that SVR does not eliminate risks for complications, particularly in patients with advanced fibrosis

Analysis of high-risk factors for hepatocellular carcinoma after sustained virological suppression of chronic hepatitis B.

Chronic hepatitis B (CHB) patients who have sustained virological suppression (SVS) still develop hepatocellular carcinoma(HCC), and the risk factors for developing HCC in these patients are not fully understood. This study included a total of 5234 patients who achieved SVS. After SVS, the incidence rate of HCC was 1.9% in 1-8 years and 1.33% in 9-16 years. There was a significant difference between the two periods (P < 0.001). After 1-8 years and 9-16 years of SVS, after multivariate analysis and IPTW adjustment, the factors related to the occurrence of HCC were men, hypertensive patients, di
