Vitamin D and bone status in elderly women.

Elderly women are the fastest growing segment of the population. Because of their increased longevity, they are more likely to develop chronic bone diseases such as osteoporosis. The maintenance of a proper balance of calcium, vitamin D, and parathyroid hormone (PTH) is essential to sustain healthy bones throughout life. Women in northern climates may be particularly susceptible to the development of osteoporosis due to their lack of exposure to sunlight, which ultimately leads to vitamin D deficiencies. A correlational study of 53 women in northern New England revealed that vitamin D insuffic

Autosomal dominant osteopetrosis: bone mineral measurements of the entire skeleton of adults in two different subtypes.

Bone mineral content (BMC) and density (BMD) were measured by dual-energy X-ray absorptiometry in two subtypes of autosomal dominant osteopetrosis (ADO). Both types have been radiologically characterized by diffuse symmetrical osteosclerosis, but with characteristic differences. Increased thickness of the cranial vault is a typical finding in type I ADO, whereas endobones in the pelvis and end-plate thickening in the spine are obligate findings in type II. Eleven patients with type I from three kindreds, and seven patients with type II, one family participated in the study, and were compared w

Dehydroepiandrosterone sulphate in the diagnosis of osteoporosis.

Several data suggest that adrenal androgens have an important role in the pathogenesis of postmenopausal osteoporosis. To study the possible relationship between bone density and serum levels of dehydroepiandrosterone sulphate (DHEAS) we investigated 105 women (45-69 years). Serum DHEAS level was significantly lower in the osteoporotic group than in the normal one. The serum level of DHEAS decreased significantly with age in both groups. We established a significant positive correlation between DHEAS and bone mineral density of the lumbar spine and femoral neck, and bone mineral content of rad

Osteoporosis in rheumatoid arthritis: effect of disease activity.

In addition to juxtaarticular osteoporosis, which appears to reflect predominantly local disease mechanisms, more generalized bone loss can occur in rheumatoid arthritis (RA). The aim of this study was to compare bone mineral density (BMD) of the lumbar spine and proximal femur in RA patients versus controls and evaluate the influence of disease related determinants. Twenty-seven patients with RA and twenty healthy subjects were included in this study. BMD was significantly reduced in RA patients compared with the control group. BMD was correlated with duration of disease, health assessment qu

A new anatomic index based on current knowledge for calculating the cumulative percentage of pagetic bone per subject and other parameters.

New data provided by dual-energy X-ray absorptiometry, together with discordances between Howarth's and Coutris' tables for estimating the percentage of bone tissue affected with Paget's disease in a given individual, prompted us to reevaluate the percentage of bone tissue contained in each segment of the skeleton. We weighed each bone in the skeleton of a 30-year-old male, calculated weight ratios from a collection of 48 bones, performed dual-energy X-ray absorptiometry measurements in 22 patients using a Hologic QDR 2000 apparatus, and obtained information from a company that supplies medica

Efficacy of ipriflavone in the prevention and treatment of postmenopausal osteoporosis.

The efficacy of ipriflavone was investigated in a 1-year double-blind, placebo-controlled, parallel group clinical trial. Ninety-one postmenopausal women completed the study, 41 received ipriflavone and 50 placebo treatment. After six months the bone mineral density of the L2-L4 vertebral region increased in the ipriflavone-treated group (0.015 g/cm2), whereas it decreased in the placebo-treated group. The differences between the treatment groups were statistically significant. Our results support the efficacy of ipriflavone in the treatment of postmenopausal osteoporosis. Since the positive e

Spurious dual-energy X-ray absorptiometry images in a patient exposed to the contrast agent Thorotrast.

A 69-year-old woman presented with a 20-year history of back pain and a 10 cm height loss. She had received an injection of the contrast agent, Thorotrast, at age 23. There was no history of fluoride exposure. Multiple vertebral compression fractures were seen on radiographs. Dual-energy X-ray absorptiometry (DXA) scans revealed high normal bone mineral content on the spine and, on whole body scan, visualization of the liver and spleen regions. Given the attenuation coefficient of thorium and the thorium concentrations reported for liver, spleen and vertebral bodies, it is likely that thorium

[The diagnosis of postmenopausal osteoporosis].

Post-menopausal osteoporosis is a metabolic bone disease which is histologically characterised by a reduction of bone mass with normal mineral content of the remaining bone tissue. In clinical term, the symptoms of disease include pain in the bones, pathological fractures (of the vertebra, femur, wrist) and consequent deformations of the skeletons. X-ray is still the most efficient diagnostic tool, although it is only capable of revealing osteopenia when bone has lost more than 40% of its mineral content. Bone densitometry is an important stage in the diagnosis and study of osteoporosis. It is

[Diagnosis of spinal fractures using optical processing of the roentgenologic image].

X-ray examinations by a traditional method in 694 patients with non-complicated compression fractures in the thoraco-lumbar part of the vertebral column do not always give the exact picture of the character of fractures of the vertebral column. The method of optic processing the roentgen picture was used in 30 patients for more detailed analysis of structural changes in the vertebrae bodies. The method allowed to specify structural changes in the vertebra bodies, including the detection of the fracture plane, abruption of a body portion, detection of an additional contour of the limiting plate

Photon-Counting CT Material Decomposition in Bone Imaging.

The accurate screening of osteoporosis is important for identifying persons at risk. The diagnosis of bone conditions using dual X-ray absorptiometry is limited to extracting areal bone mineral density (BMD) and fails to provide any structural information. Computed tomography (CT) is excellent for morphological imaging but not ideal for material quantification. Advanced photon-counting detector CT (PCD-CT) possesses high spectral sensitivity and material decomposition capabilities to simultaneously determine qualitative and quantitative information. In this study, we explored the diagnostic ut

Forearm bone mineral density in 1294 middle-aged women: a strong predictor of fragility fractures.

The aim of this study was to find out whether a single bone mineral density (BMD) measurement performed at middle age in early postmenopausal women could predict future fragility fractures. The Malmo Preventive Project, a population-based cardiovascular prevention study, included a subgroup of 1294 women, mean age 53, on which forearm BMD measurements were made using single-photon absorptiometry (SPA). Risk ratios (RRs) were calculated for an age-adjusted decrease in BMD of one standard deviation. During the 9-yr follow-up, 65 women sustained 86 fractures. The data were analyzed with Cox's pro

Caffeine does not affect the rate of gain in spine bone in young women.

The effect of nutrition and dietary caffeine consumption and physical activity on bone gain in women during the third decade of life was determined in a longitudinal, descriptive study of 145 healthy college-aged women. Estimates of caffeine and other dietary intakes were determined by repeated 7-day diet diaries. Measurements of bone mineral in the spine and total body mineral content were determined by dual-photon absorptiometry. Measures of physical activity were ascertained by physical activity monitor. The mean estimated caffeine, calcium and protein intakes for the young women students w

Osteoporosis after spinal cord injury.

Dual-photon absorptiometry characterized bone loss in males aged less than 40 years after complete traumatic paraplegic and quadriplegic spinal cord injury. Total bone mass of various regions and bone mineral density (BMD) of the knee were measured in 55 subjects. Three different populations were partitioned into four groups: 10 controls (healthy, age matched); 25 acutely injured (114 days after injury), with 12 reexamined 16 months after injury; and 20 chronic (greater than 5 years after injury). Significant differences (p less than 0.0001) in bone mass mineral between groups at the arms, pel

Reduced bone density and major hormones regulating calcium metabolism in anorexia nervosa.

Bone density of lumbar vertebrae (L2 to L4) and the whole body in 29 patients with anorexia nervosa were measured by dual photon absorptiometry, and the results were compared with those of 10 age-matched normal controls. The patients had significantly lower bone mineral density (BMD) in L3 and L2-4 than controls. However, there was no difference in whole-body BMD. L3 and L2-4 BMD was positively correlated with body weight and was negatively correlated with duration of illness and amenorrhea. Patients who had been more active 6 months before the time of the study had significantly higher L3 BMD
