Tn125-borne blaNDM-1 is decoupled from clonal background in a transcontinental Acinetobacter baumannii ST126/KL14 lineage.

BACKGROUND/OBJECTIVES: Carbapenem-resistant A. baumannii (CRAB) is a WHO Critical Priority pathogen. The blaNDM-1-carrying ST126/KL14 lineage has been independently reported from Vietnam (2015), Malaysia (2016), the USA (2023-2026), and Costa Rica (2024). Whether these geographically distinct reports represent a single transcontinental clone and through what mechanism blaNDM-1 disseminates has not been formally tested. METHODS: We performed comprehensive whole-genome reanalysis of the Vietnamese sentinel isolate DMS06669_L1 using three nested panels (n = 19, n = 138, and n = 609 Vietnamese A.

Bacterial terminal oxidase bo3 as a primary respiratory target of cathelicidin peptide hc-cath against Acinetobacter baumannii.

INTRODUCTION: Antimicrobial peptides (AMPs) hold promises to counter antimicrobial resistance. They cause several deleterious effects in bacterial cells by interfering with cell wall biosynthesis, DNA replication, transcription, and translation. OBJECTIVES: The essential role of cellular respiration in bacterial survival underscores its potential as a target for antimicrobial intervention. However, few studies have explored if and how AMPs target bacterial respiratory terminal oxidases, which contribute to proton motive force (PMF) for ATP synthesis and are essential for cell survival. Comprehensive whole-genome sequencing reveals the resistome, virulome, and mobile genetic architecture of multidrug-resistant Acinetobacter baumannii.

Acinetobacter baumannii is a major cause of hospital-acquired infections and has emerged as a critical multidrug-resistant (MDR) pathogen with limited therapeutic options. In this study, two MDR clinical isolates from a tertiary care hospital in Chennai, India, selected based on their strong biofilm-producing capacity, were subjected to whole-genome sequencing (WGS) to characterize their resistome, virulome, and genomic features. Both isolates exhibited resistance to β-lactams, carbapenems, aminoglycosides, fluoroquinolones, and trimethoprim-sulfamethoxazole, while remaining susceptible to tig…

Deciphering the genomic landscape of novel Acinetobacter non-baumannii lineages causing neonatal septicemia: carbapenem resistance and virulence.

BACKGROUND: Acinetobacter non-baumannii (Anb) species are reported worldwide to cause infections in both adults and neonates, although less frequently than Acinetobacter baumannii. However, limited information is available on their genomic diversity, resistance mechanisms, and virulence potential. This study investigates novel Anb isolates causing neonatal septicemia in India to characterize their resistance and pathogenic traits. METHODS: Anb isolates from neonatal blood cultures (2007-2025) were identified by VITEK2 Compact system, MALDI-TOF MS, and Whole-genome sequencing (WGS). Antimicrob…

Carbapenem-resistant Acinetobacter baumannii at a hospital in Botswana: detecting a protracted outbreak using whole genome sequencing.

UNLABELLED: Carbapenem-resistant Acinetobacter baumannii (CRAb) has emerged as a major and often fatal cause of bloodstream infections among hospitalized patients in low- and middle-income countries (LMICs). CRAb outbreaks are hypothesized to arise from reservoirs in the hospital environment, but outbreak investigations in LMICs are often limited in scope due to lack of access to whole genome sequencing (WGS). We performed WGS on 43 stored isolates (blood cultures [n = 23] and environmental swabs [n = 20]) presumptively identified as A. baumannii collected during 2021-2022 from a 530-bed refer…

Identification of drug targets in pan-drug resistant Acinetobacter baumannii via whole genome sequencing and subtractive genomics.

In this study, we report, to the best of our knowledge, the first complete genome sequence of a pan drug resistant (PDR) Acinetobacter baumannii strain (JRCGR-AK-AB-01) from Karachi, Pakistan. Strain JRCGR-AK-AB-01 exhibited a pan drug resistant phenotype, showing susceptibility only to polymyxin B and intermediate susceptibility to colistin. Hybrid genome sequencing using MinION long-reads and DNBSEQ short-reads revealed that the genome size of strain JRCGR-AK-AB-01 is 4.03 Mb. We identified that JRCGR-AK-AB-01 is closely related to other A. baumannii strains based on Average Amino Acid Ident…

A chronic Acinetobacter baumannii pneumonia model to study long-term virulence factors, antibiotic treatments, and polymicrobial infections.

Acinetobacter baumannii causes prolonged infections that disproportionately affect immunocompromised populations. Our understanding of A. baumannii respiratory pathogenesis relies on an acute murine infection model with limited clinical relevance that employs an unnaturally high number of bacteria and requires assessment of bacterial load at 24-36 h post-infection. Here, we demonstrate that low intranasal inoculums in tlr4 mutant mice allows for infections lasting at least 3 weeks. Using this "chronic infection model" we determine the adhesin InvL is a virulence factor required during later st…

A novel type of hemoglobin confers host-derived nitric oxide resistance to the opportunistic pathogen Acinetobacter baumannii.

Acinetobacter baumannii is an opportunistic Gram-negative pathogen responsible for various infections, such as those of the bloodstream and lungs, which often resist antibiotic therapy. In the course of an infection, the human innate immune system's phagocytic cells are activated producing nitric oxide (NO) that cause bacterial injury. While the antimicrobial effects of nitrosative stress and the bacterial resistance mechanisms are well-characterized for several pathogens, the adaptations of Acinetobacter spp. to NO have not been studied. In this work, we define the transcriptional response of…

Prognostic value of NPR and CLR-based nomogram modeling in elderly patients with Acinetobacter baumannii bloodstream infection.

BACKGROUND: Acinetobacter baumannii (A. baumannii) is one of the main pathogens that causesbloodstream infection (BSI) in elderly patients, with high morbidity and mortality rates once infected; new inflammatory indicators, such as the neutrophil-lymphocyte ratio (NLR) and platelet-lymphocyte ratio(PLR), have been proposed in recent years, and the prognostic effects of these new inflammatory indicators have not yet been adequately investigated in A.baumannii BSI in elderly patients. Therefore, we verified the effects of these inflammatory indicators on A.baumannii BSIprognosis in elderly patie…

Emergence of carbapenem-resistant Acinetobacter baumannii clonal complex 2 in multiple hospitals in São Paulo state, Brazil.

Carbapenem-resistant Acinetobacter baumannii (CRAB) is a common pathogen prevalent in Brazilian hospitals. Worldwide, dissemination of CRAB is associated with the Clonal Complex 2 (CC2); in South America, however, CC1, 15, 25, and 79 are the most prevalent clones. In July 2020, our reference laboratory received the first CC2 isolates from a COVID-19 hospital, and, in the following months, this clone was detected in 15 other Brazilian institutions. To understand the clonal structure of this emerging pathogen, we characterize 89 isolates by whole-genome sequencing and antimicrobial susceptibilit…

Outbreak with OXA-23-producing Acinetobacter baumannii in a COVID-19 ICU cohort: unraveling routes of transmission.

An outbreak of OXA-23-producing carbapenem-resistant Acinetobacter baumannii amongst ICU-patients with COVID-19 likely occurred by transmission through inanimate surfaces, potentially facilitated by a contaminated positioning pillow shared between patients. Subsequent rapid spread may have been caused by exposure to respiratory secretions contaminating healthcare worker's gloves and gowns during prone positioning.
