PURPOSE OF REVIEW: Although corticosteroids are frequently used to palliate cancer-related symptoms, limited published studies are available. This review summarizes recent literature on the impact of corticosteroids on fatigue, its related symptoms, and the role of 'corticosteroid rotation' in improving the corticosteroid-related side-effects.

We describe a 55 year-old woman with longstanding hypertension who developed hypokalaemia following diuretic treatment. Investigations revealed primary hyperaldosteronism due to an adrenal adenoma, and normal blood pressure was restored after surgical removal of the tumour. Primary hyperaldosteronism is a potentially curable cause of hypertension and should be considered in hypertensive patients who present with diuretic - induced hypokalaemia.

Infantile subglottic hemangioma is a rare, life threatening disease of the larynx. Various treatments have been described in the literature, including tracheostomy, systemic steroids, intralesional steroid injection, carbon dioxide laser therapy, and interferon alfa-2a. A 39-yr-old woman, who had been treated for Behcet's disease for 4 yr, was admitted for further investigation of recently identified hypokalemia and hypertension. Suppressed plasma renin activity with elevated plasma aldosterone concentration and an anomalous postural decrease in plasma aldosterone were observed. An abdominal CT scan revealed a right adrenal mass. The patient was diagnosed with Conn's syndrome. The association of Conn's syndrome with Behcet's disease was thought to be coincidental. To our knowledge, this is the first case of Conn's syndrome associated with Behcet's disease.

We report a case of an adenomatoid tumor (AT) of an adrenal gland with micronodular adrenal cortical hyperplasia (ACH). A 51-year-old man was found to have newly developed hypertension with clinical evidence of primary aldosteronism. A 33-year-old man presenting with muscle weakness and hypertension was found to have hypokalaemia and metabolic alkalosis. Computed tomography (CT) showed an adrenal mass. Conn's syndrome due to an adrenocortical adenoma was confirmed at surgery. Hypertension-related adrenal disorders such as Cushing's syndrome, carcinoma and phaeochromocytoma are discussed. CT is currently the single most useful imaging method for identifying adrenal masses.

Primary hyperaldosteronism (Conn's syndrome) rarely occurs during pregnancy. The concurrence of hypertension combined with hypokalemia and revealing subjective symptoms such as paresthesia, muscular weakness and lassitude can suggest this infrequent diagnosis. The diagnosis is confirmed on the one hand by chemical tests demonstrating an aldosterone level far above normal and clearly suppressed renin activity, and on the other hand through ultrasound for a morphological diagnosis. We consider causal surgical treatment with adrenalectomy as the therapy of choice during early pregnancy.
