Interocular differences in subfoveal choroidal thickness in monocular intermittent exotropia.

PURPOSE: To determine whether subfoveal choroidal thickness and central foveal thickness differ according to the presence of fixation preference in patients with intermittent exotropia without anisometropia or amblyopia. METHODS: Children ≥4 years of age with either monocular (fixation preference) or alternating intermittent exotropia on three consecutive visits who had no anisometropia, no amblyopia, and spherical equivalent of ± 1.25 D or less in both eyes were recruited for this study. Subfoveal choroidal thickness and central foveal thickness, measured using spectral domain optical cohere…

Unilateral Application of Cathodal tDCS Reduces Transcallosal Inhibition and Improves Visual Acuity in Amblyopic Patients.

Objective: Amblyopia is a neurodevelopmental disorder characterized by visual acuity and contrast sensitivity loss, refractory to pharmacological and optical treatments in adulthood. In animals, the corpus callosum (CC) contributes to suppression of visual responses of the amblyopic eye. To investigate the role of interhemispheric pathways in amblyopic patients, we studied the response of the visual cortex to transcranial Direct Current Stimulation (tDCS) applied over the primary visual area (V1) contralateral to the "lazy eye." Methods: Visual acuity (logMAR) was assessed before (T0), immedia…

Early monocular defocus disrupts the normal development of receptive-field structure in V2 neurons of macaque monkeys.

Experiencing different quality images in the two eyes soon after birth can cause amblyopia, a developmental vision disorder. Amblyopic humans show the reduced capacity for judging the relative position of a visual target in reference to nearby stimulus elements (position uncertainty) and often experience visual image distortion. Although abnormal pooling of local stimulus information by neurons beyond striate cortex (V1) is often suggested as a neural basis of these deficits, extrastriate neurons in the amblyopic brain have rarely been studied using microelectrode recording methods. The recept…

Modification of Plusoptix referral criteria to enhance sensitivity and specificity during pediatric vision screening.

PURPOSE: To determine the impact of using several different proposed sets of referral criteria on the specificity and sensitivity of the plusoptiX S08 photoscreener for detecting amblyopia risk factors. METHODS: During a 2-month period, 144 children ages 9 months to 14 years were screened at the Tennessee Lions Eye Center before receiving a comprehensive eye examination and cycloplegic refraction. Three previously published sets of referral criteria were used for screening, including the manufacturer's criteria and the criteria proposed by Arthur and colleagues, which are nearly identical to …

Monocular activation of V1 and V2 in amblyopic adults measured with functional magnetic resonance imaging.

PURPOSE: Although previous neuroimaging efforts clearly indicate visual cortical dysfunction in adults with amblyopia, the extent of abnormalities remains unclear. METHODS: This functional magnetic resonance imaging (fMRI) study directly compared activity in visual cortex produced by monocular stimulation in 18 adults (six esotropic strabismics, six anisometropes, and six controls). Measures were made in three cortical regions-of-interest, individually defined using standard retinotopic mapping techniques in the nonamblyopic eye, corresponding to extrafoveal V1, extrafoveal V2, and the foveal…

Binocular eyelid closure promotes anatomical but not behavioral recovery from monocular deprivation.

Deprivation of patterned vision of frontal eyed mammals early in postnatal life alters structural and functional attributes of neurones in the central visual pathways, and can produce severe impairments of the vision of the deprived eye that resemble the visual loss observed in human amblyopia. A traditional approach to treatment of amblyopia has been the occlusion of the stronger fellow eye in order to force use of the weaker eye and thereby strengthen its connections in the visual cortex. Although this monocular treatment strategy can be effective at promoting recovery of visual acuity of th…

Management of Descemet membrane detachment secondary to forceps assisted delivery in a newborn.

To describe the clinical signs and management of Descemet membrane (DM) detachment after forceps-related trauma during birth. A 2-day-old te … ve apposition. Central visual axis remained partially spared in the months to follow, and intensive amblyopia treatment was indicated. Prolonged topical treatment may be helpful to reduce edema and risk of severe amblyopia in DM tears secondary to forceps traumatism at birth, but insufficient in cases of large DM detachment.

Pediatric Corneal Transplant Surgery: Challenges for Successful Outcome.

Visual rehabilitation of pediatric corneal blinds is a major challenge to corneal transplant surgeons. Penetrating keratoplasty (PKP) is the only way to restore vision and prevent irreversible blindness due to amblyopia in children. Performing penetrating corneal grafts in children poses difficulty in evaluation, technical difficulties during surgery and problems during follow-up. Younger children do not cooperate for proper slit-lamp examination and need to be examined under general anaesthesia. In addition, the complications encountered post PKP, including allograft rejection, post PKP astig…

[Bilateral persistent pupillary membrane and high myopia].

After birth, remnants of pupillary membrane which constitute the vascular supply to the crystalline lens can persist. These are contiguous to the iris collarette causing possible deprivation amblyopia by obstructing the pupillary area. We here report the case of a 4-year-old child, born to consanguineous parents, presenting with bilateral persistent pupillary membrane associated with high myopia. Clinical examination showed bilateral remnants of pupillary membrane, intact iris sphincter and diffuse chorioretinal atrophy in the fundus of eye . Visual acuity was difficult to assess. On the other…

Optical changes and apparent emmetropization in a patient with a peripheral unilateral lens coloboma.

Lens coloboma is a developmental defect resulting from abnormalities of the zonules and ciliary body. It may present as an isolated pathology or be accompanied by anomalies in different ocular structures. We report the case of a 20-year-old man referred for evaluation of anisometropic amblyopia in the right eye. Manifest refraction was -2.25 +3.00 ×35 in the right eye; corrected distance visual acuity, 20/50. Corneal topography revealed regular astigmatism of +2.46 D at 124°, and wavefront aberrometry revealed an irregular internal astigmatism of +6.27 D at 35°. Only after full pupillary dilat…
