A randomized double-blinded study assessing the effect of different doses of transnasal dexmedetomidine on the median effective concentration of ropivacaine for a caudal block.

BACKGROUND: Perineural administration of dexmedetomidine (PN-DEX) can enhance the efficacy of local anesthetics used in regional nerve blocks while decreasing the median effective concentration (EC50) of these anesthetics. Intranasal administration of dexmedetomidine (IN-DEX) is more accessible for sedation during regional anesthesia because of its non-invasive systemic administration and demonstrates synergism with local anesthetic. However, it remains unclear whether IN-DEX affects the EC50 of local anesthetics used in caudal blocks. METHODS: This study was a prospective, single-center, dou

Pruritus after continuous administration of epidural morphine for post-cesarean delivery analgesia: a case control study.

BACKGROUND: Pruritus is one of the most common side effects of epidural morphine administered for post-surgery analgesia, and pregnant women tend to be highly susceptible. The relative contributions of morphine concentration, local anesthetics, and level of pain to pruritus after epidural morphine for post-cesarean delivery analgesia remain unclear. Accordingly, the present study aimed to identify risk factors for pruritus after continuous administration of epidural morphine for post-cesarean delivery analgesia. METHODS: This case control study was based on routinely collected clinical data.

The Eyes Have It: Eyelid Swelling and Rash in a 79-year-old Woman With Macular Degeneration.

INTRODUCTION: A 79-year-old woman with macular degeneration was referred to the Allergy/Immunology clinic for the evaluation of a potential allergy to anti-vascular endothelial growth factor (anti-VEGF) treatments. The patient developed urticaria and eyelid swelling immediately following a retinal injection of aflibercept, which she had previously tolerated. She previously had allergic reactions following ranibizumab and bevacizumab injections. Injections of anti-VEGF treatments were discontinued given concern for allergy with progression of the patient's disease. OBJECTIVE: To assess the cul

Efficacy evaluation of neuromuscular blocking agents as adjuncts to intravenous regional anesthesia: a meta-analysis of randomized controlled trials.

PURPOSE: To evaluate the efficacy of neuromuscular blocking agents (NMBAs) as adjuncts to intravenous regional anesthesia (IVRA). METHODS: Two researchers independently searched PUBMED, EMBASE, the Cochrane Library databases, and CBM for randomized controlled trials assessing the efficacy of NMBAs as adjuvants in IVRA. RESULTS: This meta-analysis included 420 patients from 7 randomized controlled trials. Compared to IVRA using local anesthetics alone, the adjunctive use of NMBAs significantly shortened the onset time of sensory block [mean difference (MD) = -1.38 min, 95% CI: -2.02 to -0.75;

US-Guided Interventional Procedures for Total Hip Arthroplasty.

In patients with total hip arthroplasty (THA) with recurrent pain, symptoms may be caused by several conditions involving not just the joint, but also the surrounding soft tissues including tendons, muscles, bursae, and peripheral nerves. US and US-guided interventional procedures are important tools in the diagnostic work-up of patients with painful THA given that it is possible to reach a prompt diagnosis both directly identifying the pathological changes of periprosthetic structures and indirectly evaluating the response and pain relief to local injection of anesthetics under US monitoring.

Combined spinal epidural anesthesia in achondroplastic dwarf for femur surgery.

Achondroplasia is the commonest form of short-limbed dwarfism and occurs in 1:26,000-40,000 live births. This is an autosomal dominant disorder with abnormal endochondral ossification whereas periosteal and intramembranous ossification are normal. The basic abnormality is a disturbance of cartilage formation mainly at the epiphyseal growth plates and at the base of the skull. The anesthetic management of achondroplastic dwarfs is a challenge to the anesthesiologist. Both regional as well as general anesthesia have their individual risks and consequences. We report a case of an achondroplastic

Influence of topical anesthesia on tonometric values of intraocular pressure.

The air pulse noncontact tonometer provides a safe and reliable method for measuring intraocular pressure (IOP), and makes it possible to avoid topical anesthesia. Based on previous reports that suggested possible anesthetic-induced IOP variations, this study was undertaken to investigate with this procedure the influence of local anesthetics on IOP and of some topically used drugs that could modify IOP values. In 212 normal or glaucomatous patients who underwent IOP measurement with a noncontact tonometer, IOP was determined before and in the first minutes following instillation of one of fou

The Neural Circuits Underlying General Anesthesia and Sleep.

General anesthesia is characterized by loss of consciousness, amnesia, analgesia, and immobility. Important molecular targets of general anesthetics have been identified, but the neural circuits underlying the discrete end points of general anesthesia remain incompletely understood. General anesthesia and natural sleep share the common feature of reversible unconsciousness, and recent developments in neuroscience have enabled elegant studies that investigate the brain nuclei and neural circuits underlying this important end point. A common approach to measure cortical activity across the brain

Baricity and the distribution of lidocaine in a spinal canal model.

The role of the baricity of local anaesthetic solutions in determining the distribution of local anaesthetics injected into the subarachnoid space (and hence the level of anaesthesia) has been challenged. A recent study found no difference in the extent of cephalad spread of hyperbaric and isobaric solutions and concluded that density had no effect on the spread of local anesthetics. The present study, to determine the validity of this conclusion, utilized a spinal model filled with a "cerebrospinal fluid equivalent." Following the injection of hyperbaric lidocaine, the local anaesthetic was m

[Value of injection therapy for degenerative diseases of lumbar spine].

Local injection therapy is of great value in the border area between non-operative or operative treatment of discogenic and spinal stenotic radicular syndromes. With a series of deep periradicular infiltrations of local anesthetics and antiphlogistics it is possible to reduce the pain peaks in the spontaneous course of degenerative spinal diseases, so that planned surgery is no longer necessary. For the rare serious palsies immediate surgery is still necessary. On-going improvement is reached by physiotherapy and psychotherapy and with the back school.

[The effect of local anaesthetics on uterine blood flow and O2-consumption in pregnant sheep (author's transl)].

The local anesthetics bupivacaine, etidocaine, lignocaine and chloroprocaine were infused intravenously for 30 min to pregnant sheep, prepared for chronic studies. The amide compounds reached highere plasma concentrations than chloroprocaine. All local anaesthetics decreased uterine blood flow and O2 consumption without marked alterations of arterial or uterine venous blood gases. These changes were within the wide physiological range of variation, which was established in controls during 24 h. The findings may be of significance during paracervical block or pathological states of UBF or fetal

[The use of subarachnoid bupivacaine analgesia in malignant hyperthermia-susceptible subjects. A clinical case report].

The authors report a case of malignant hyperthermia in susceptible woman undergoing safenectomy. Spinal anesthesia with bupivacaine was performed after dantrolene prophylaxis per os. No complications occurred during and after surgery. This case confirms the safety of local anesthetics and it suggests that anaesthesia should not be refused to caffeine halothane contracture test positive patients.

A multimodal analgesia protocol for total knee arthroplasty. A randomized, controlled study.

BACKGROUND: Although numerous methods of postoperative analgesia have been investigated in an attempt to improve pain control after total knee arthroplasty, parenteral narcotics still play a major role in postoperative pain management. Local anesthetics have the advantage of blocking pain conduction at its origin and minimizing the systemic side effects associated with postoperative narcotic use. This study was performed to evaluate the benefits and safety of a multimodal analgesia protocol that included periarticular injection of large doses of local anesthetics in patients undergoing total k

[Epidural fentanyl provide sufficient analgesia for extracorporeal shock wave lithotripsy (ESWL)].

Epidural fentanyl (0.2 mg in 10 ml normal saline) was administrated in 10 unpremedicated patients undergoing extracorporeal shock wave lithotripsy (ESWL). No test dose of local anesthetics was administrated and accessory drugs such as narcotics or analgesics were not used. Painful procedures except for shock wave such as insertion of urethral catheter were not applied. We observed analgesic effect using original score (grade 0-3) and monitored heart rate, blood pressure, respiratory rate and Spo2 during ESWL. At 1, 2, 3, 4 and 24 hours after ESWL, patients were questioned regarding the presenc

Toxicity and allergy to local anesthesia.

Considering the amount of local anesthetic administered on a daily basis, dental professionals must be familiar with the factors that influence the dose and type of local anesthetic that induces a toxic or allergic reaction. In addition to the route and rate of administration, the patient's physical condition and health may also influence the dose of local anesthetic that could be safely administered. This article reviews the different causes of local anesthesia toxicity and allergy. With prevention and early recognition of the warning signs, poor prognosis can be avoided.

The impact of cardiovascular drugs on the efficacy of local anesthesia in dentistry.

BACKGROUND AND AIM: Drugs used chronically by patients with diseases of the cardiovascular system (group C of the ATC classification) may act on adrenergic receptors and/or certain ion channels, which gives them the potential to interact with the action of local dental anesthetics. The aim of the study was to investigate the effect of systemically administered chronic cardiovascular medication (oral route) on the efficacy of intraoral local anesthesia in patients with diseases of the cardiovascular system. PATIENTS AND METHODS: This was a prospective cohort study which analyzed the efficacy o
