Prulifloxacin: a review focusing on its use beyond respiratory and urinary tract infections.

Prulifloxacin is a fluoroquinolone antibiotic that has been approved in several European countries for the treatment of lower urinary tract infections and exacerbations of chronic bronchitis. In this review, PubMed and Scopus databases were searched for potential uses of prulifloxacin beyond respiratory and urinary tract infections. Nine individual articles (eight randomised controlled trials and one cohort study) were regarded as eligible for inclusion in the review. Three of the studies were double-blinded, whilst six were open-label trials. Three studies referred to the treatment of patient

Antimicrobial susceptibility profile of Pseudomonas aeruginosa isolates in Egypt.

PURPOSE: Pseudomonas aeruginosa is a leading cause of nosocomial respiratory tract, urinary tract and skin infections. Data are sparse on the antimicrobial resistance of P. aeruginosa in Egypt. We sought to detect and compare the antimicrobial susceptibility of P. aeruginosa isolates from respiratory tract, urinary tract and skin infections at 3 Egyptian hospitals. MATERIALS AND METHODS: Minimum inhibitory concentrations of antibiotics were determined by the agar dilution method. RESULTS: P. aeruginosa respiratory tract infections isolates were 100% resistant to ampicillin, ampicillin/sulbac

Efficacy and safety of antibiotic treatment in relation to treatment time.

Decisions on treatment times with antibiotics are often arbitrary and based on empirical decisions or clinical trials which are too small to exclude even considerable differences between two study groups. Single-dose treatment of uncomplicated cystitis in women has been advocated by many but a careful analysis of available information clearly shows that a single-dose has so far always been inferior to 3-day or greater than 5-day treatment. With trimethoprim-sulphonamide combinations, no further efficacy is gained by increasing the treatment time in uncomplicated cystitis above three days while

Fever in the neutropenic patient.

A total of 100 consecutive episodes of fever of 101 degrees F (38.3 degrees C) or above in 56 neutropenic patients have been investigated. All the patients had either acute leukaemia or aplastic anaemia. A cause for the fever was found in 68 of these episodes, in 87% of which it was due to infection. The commonest single finding was septicaemia (30 episodes). Only two episodes of fever could be ascribed solely to the underlying malignant disease.Infection should be assumed to be present and the cause of fever in neutropenic patients until proved otherwise.

Antimicrobial catheters coated with poly-L-lysine for the prevention of bacteriuria in adults requiring short-term catheterization: a multicenter randomized controlled trial.

BACKGROUND: To evaluate the safety and efficacy of a newly developed catheter coated with antimicrobial poly-L-lysine (PLL) in reducing the risk of catheter-associated bacteriuria. METHODS: A prospective, multicenter, randomized controlled noninferiority trial was conducted among patients who required indwelling catheterization between February 23, 2023, and January 30, 2024. The experimental group used antimicrobial PLL catheters, and the control group used noble metal alloys (NMA) coated catheters. The primary outcome was the incidence of catheter-associated bacteriuria. The secondary outco

Efficacy of phage vB_Ps_ZCPS13 in controlling Pan-drug-resistant Pseudomonas aeruginosa from urinary tract infections (UTIs) and eradicating biofilms from urinary catheters.

BACKGROUND: Pan-drug resistance (PDR) is a ticking time bomb, as it causes high human hospitalizations and mortality rates. For instance, Pseudomonas aeruginosa is associated with significant rates of urinary tract infections (UTIs) due to several reasons including antibiotic resistance, biofilm formation and the presence of various virulence factors. Consequently, there is an urgent need for safe and effective alternative antibacterials. Phage therapy is a promising alternative that uses naturally occurring bacteriophages (phages). Therefore, our present study investigated the isolation and c

Epidemiology and urological pathogenic potential of Aerococcus species in greater Glasgow and Clyde (Descriptive study of Aerococcus urinae in blood culture and urinary samples: clinical importance and potential marker of urinary tract pathology).

Introduction. Aerococcus species in particular A. urinae are increasingly reported as causative agents of bacteraemia, urinary tract infection, sepsis, and endocarditis. We sought to establish the epidemiology of A. urinae in Glasgow hospitals and whether the presence of the organism in clinical isolates could be an indicator of undiagnosed urinary tract pathology.Hypothesis/Gap statement. The knowledge gap among clinical staffs on Aerococcus species as emerging pathogens can be filled by understanding its epidemiology and clinical importance.Aim. Describe the epidemiology and clinical importa

Long-term antibiotics for preventing recurrent urinary tract infection in children.

BACKGROUND: Urinary tract infection (UTI) is common in children. Symptoms include fever, lethargy, anorexia, and vomiting. UTI is caused by Escherichia coli in over 80% of cases and treatment is a course of antibiotics. Due to acute illness caused by UTI and the risk of pyelonephritis-induced permanent kidney damage, many children are given long-term (several months to 2 years) antibiotics aimed at preventing recurrence. This is the third update of a review first published in 2001 and updated in 2006, and 2011. OBJECTIVES: To assess whether long-term antibiotic prophylaxis was more effective

Catheter-associated bacterial flora in patients with benign prostatic hyperplasia: shift in antimicrobial susceptibility pattern.

BACKGROUND: Men with urinary retention secondary to benign prostatic hyperplasia (BPH) are prone to genitourinary infections. Physicians should be aware of the current antimicrobial susceptibility pattern in this population if empirical treatment is needed. The goal of this study was to evaluate variations in prevalence, composition and antimicrobial susceptibility of bacterial flora in men with indwelling catheters subjected to surgery for BPH in chosen time periods since 1994. Necessary changes in empirical therapy were also assessed. METHODS: All patients with indwelling catheters admitted

[Catheter-related urinary tract infections in the community].

The prevalence of urinary catheterization in the community ranges from 0.02% to 0.07%. Despite the generalized use of closed systems, the risk of bacteriuria in patients with urethral catheters is 3%-10% per day and its presence is universal when the device remains in place for 30 days or longer. Although most of these episodes of bacteriuria are asymptomatic, up to 30% of them lead to clinical symptoms and complications, including severe sepsis and death. The microorganisms infecting the urine of catheterized patients frequently belong to species less susceptible to antibiotics and form biofi

Single dose of ofloxacin treatment in women with dysuria and frequency.

The efficacy of single dose ofloxacin in lower urinary tract infections was investigated in this study. Fifty-one women with complaints of dysuria and frequency were treated with a single dose of 400 mg ofloxacin orally. The results of urinalysis and urine culture which were performed 4-7 days after the administration of the drug were evaluated. Thirty-eight of the 47 evaluable patients (80.85%) were asymptomatic with normal urinalysis and sterile urine, thus cured. The data of the study suggest that single dose ofloxacin may be the first treatment option in the particular group of women with

Chlorhexidine for meatal cleaning in reducing catheter-associated urinary tract infections: a multicentre stepped-wedge randomised controlled trial.

BACKGROUND: Evidence for the benefits of antiseptic meatal cleaning in reducing catheter-associated urinary tract infection (UTI) is inconclusive. We assessed the efficacy of 0·1% chlorhexidine solution compared with normal saline for meatal cleaning before urinary catheter insertion in reducing the incidence of catheter-associated asymptomatic bacteriuria and UTI. METHODS: A cross-sectional, stepped-wedge, open-label, randomised controlled trial was undertaken in Australian hospitals. Eligible hospitals were Australian public and private hospitals, with an intensive care unit and more than 3

Evaluation of a phage-inclusive multimodal strategy for recurrent urinary tract infections.

Urinary tract infections (UTIs) are among the most common bacterial infections worldwide and are associated with high recurrence rates and substantial morbidity. New therapeutic approaches are urgently needed, especially for recurrent UTIs (rUTIs), among which phage therapy represents a promising option. Here, we systematically evaluated the performance of six E. coli phages under urinary tract-relevant conditions and established a quantitative area-under-the-curve (AUC)-based framework for the rational selection of therapeutic phages. Phage lysis behavior was tested under oxic and anoxic cond

Uropathogenic Escherichia coli Biofilms: Antibiotic Pressure and Interaction with Human Neutrophils.

Uropathogenic Escherichia coli (UPEC) is a primary cause of urinary tract infections (UTIs), with recurrent cases often linked to its ability to form biofilms. This study investigated the effects of various antibiotics on UPEC biofilm formation and the subsequent interaction of these biofilms/their supernatants with human neutrophils. We determined the minimum inhibitory concentrations (MIC), minimum bactericidal concentrations (MBC), and biofilm eradication concentrations (MBEC) for ampicillin, gentamicin, chloramphenicol, ciprofloxacin, and levofloxacin. Our results showed an increase in MBE

A case of Staphylococcus saprophyticus bacteremia caused by obstructive pyelonephritis.

Herein, we present the case of a 64-year-old woman diagnosed with left calculous pyelonephritis. Gram-positive cocci in clusters were isolated from two sets of her blood culture. These cocci were later identified as Staphylococcus saprophyticus. Although S. saprophyticus is a common causative pathogen of urinary tract infection (UTI) among young women, coagulase-negative staphylococci including S. saprophyticus isolated from UTI patients' blood samples were often considered a result of contamination because of their rareness. Considering the high percentage of methicillin-resistant strains of

The Klebsiella pneumoniae ter Operon Enhances Stress Tolerance.

Healthcare-acquired infections are a leading cause of disease in patients that are hospitalized or in long-term-care facilities. Klebsiella pneumoniae (Kp) is a leading cause of bacteremia, pneumonia, and urinary tract infections in these settings. Previous studies have established that the ter operon, a genetic locus that confers tellurite oxide (K2TeO3) resistance, is associated with infection in colonized patients. Rather than enhancing fitness during infection, the ter operon increases Kp fitness during gut colonization; however, the biologically relevant function of this operon is unknown

Characterization of Transferrable Mechanisms of Quinolone Resistance (TMQR) among Quinolone-resistant Escherichia coli and Klebsiella pneumoniae causing Urinary Tract Infection in Nepalese Children.

BACKGROUND: Transferrable mechanisms of quinolone resistance (TMQR) can lead to fluoroquinolone non-susceptibility in addition to chromosomal mechanisms. Some evidence suggests that fluoroquinolone resistance is increasing among the pediatric population. We sought to determine the occurrence of TMQR genes among quinolone-resistant E. coli and K. pneumoniae causing urinary tract infections among Nepalese outpatient children (< 18 years) and identify molecular characteristics of TMQR-harboring isolates. METHODS: We performed antimicrobial susceptibility testing, phenotypic extended-spectrum β-l
