Juvenile rhegmatogenous retinal detachment.

Between Jan 1, 1966, and June 30, 1974, 179 children, birth through 16 years of age with rhegmatogenous retinal detachments, underwent scleralbuckling procedures at Wills Eye Hospital. The detachments were associated most frequently with trauma (44%), myopia (15%), aphakia (10%), and retinopathy of prematurity (8%). Successful reattachment was achieved in 80% of the cases. Vision of 20/100 or better was present in 22% preoperatively and 58% postoperatively.

Juvenile retinal detachment.

We feel that the management of juvenile retinal detachment can best be improved by earlier diagnosis. School vision-screening tests should be encouraged, and long-term followup of patients with high myopia, aphakia, and retrolental fibroplasia should be practiced. The peripheral retina should be examined in all traumatized eyes since delayed detachment is the rule. When retinal breaks are found in high-risk eyes, in our opinion, prophylactic treatment is indicated.

Intraocular Lens Calculation Concept Based on Aphakic Refraction-Considerations on a Cornea Model With Two Refracting Surfaces.

PURPOSE: To develop a concept for predicting the power of an intraocular lens (IOL) and the spectacle refraction after cataract surgery based on aphakic refraction (REFa) using vergence transforms with a two-surface cornea model. METHODS: This simulation study is based on a large dataset originally used in IOLCon ( www.IOLCon.org ) for optimising lens constants. Proxy values for REFa calculated from the available information in this dataset were used to develop a strategy for predicting the effective lens position (ELP). A variety of models, including linear, quadratic, sigmoidal and two step

Peripheral refraction in aphakic eyes.

Peripheral refraction was measured in aphakic eyes and control eyes of approximately the same age, using a Topcon refractometer. In addition, two aphakic eyes in young patients were also measured and compared with data obtained on young patients by Millodot using the same technique. It was found that aphakic eyes have much less oblique astigmatism than phakic eyes of the same age, suggesting a large contribution to the crystalline lens. The total oblique astigmatism of the eyes increases with age but that of the cornea remains approximately the same, thus implying that this increase with age i

An innovative alternative for spherophakia.

Objective: The aim of this case report is to report a new aphakic intraocular lens (IOL) that can be used for spherophakia. Methods: This is a single case report wherein the authors elaborate the technique of inserting the new IOL design in patients with spherophakia. Results: This new IOL design is very stable and is very promising in our follow-up of 6 months. Conclusion: The CM T-flex IOL can be a simple and alternate option for correcting aphakia.

Granular corneal dystrophy: treatment with soft contact lenses.

Perforating keratoplasty was performed in both eyes of two siblings (sister and brother) with typical granular corneal dystrophy. Three of the operated eyes showed a recurrence of the underlying disease. In one eye, a recurrence could not be detected 14 years after the operation. This eye had been fitted with a contact lens 16 months after keratoplasty because of aphakia. Thus a contact lens may have a therapeutic effect in this primary epithelial disease.

[Implantation of retropupillary intraocular lenses in post-traumatic conditions of the eye].

The investigation comprises the results of post-traumatic artephakias. In the course of 15 months the authors treated 15 patients with traumatic cataract after previous perforating or blunt eye injuries. In 12 patients after extracapsular extraction a retropupillary IOL was implanted. In three patients implantation was not performed. Retropupillary pseudophakia is the optimal correction of aphakia even in patients with previous serious eye injuries. A higher incidence of postoperative complications may be expected.

Ab interno laser sclerostomy in aphakic patients with glaucoma and chronic inflammation.

Five patients with aphakia, glaucoma, and chronic inflammation were treated with ab interno sclerostomy by using the continuous wave Nd:YAG laser focused through a sapphire probe. After a follow-up period of 24 to 28 months, three of five patients had good intraocular pressure control. The sclerostomy failed in one patient when it was occluded by vitreous. The second failure was attributed to closure of the sclerostomy because of chronic intraocular inflammation.

Medicare program; coverage of optometrists' services--Health Care Financing Administration. Notice of proposed rulemaking.

These regulations would expand Medicare coverage of services furnished by optometrists to include examination services related to the condition of aphakia (absence of the natural lens of the eye). Under our current regulations, Medicare coverage of optometric services is limited to dispensing services in connection with the actual fitting and provision of prosthetic lenses. The regulations are necessary to conform our existing Medicare regulations to a statutory change made by section 937 of the Omnibus Reconciliation Act of 1980.

[The surgical results in rhegmatogenous retinal detachment].

The paper present the functional and anatomic results obtained in 376 retinal detachment operations done in the Ophthalmological Clinic from Cluj-Napoca. The following types of operations were done: cerclage Arruga, scleral indentation, surface diathermy, laser photocoagulation. The retrospective study revealed percentages of 73.41% the anatomic reapplication and 50% in functional recovery. The negative influencing factors of the prognosis were: aphakia, large and old retinal detachments, macular region affectation.

Epikeratophakia following previous refractive keratoplasty surgery: two case reports.

Epikeratophakia is becoming a significant and worthwhile surgical modality for selected cases of pediatric and adult aphakia, keratoconus, and myopia. Ultimately, it may also provide a safe and effective means of obtaining satisfactory surgical correction when the results from a previous refractive technique are insufficient. We present two successful cases of epikeratophakia following refractive keratoplasty: one following radial keratotomy for myopia and one following penetrating keratoplasty for keratoconus.

Cataract following radial keratotomy.

An intumescent cataract developed 16 weeks after radial keratotomy in a 31-year-old man. Corneal perforation was documented at the time of surgery, but no direct injury was noted to the lens capsule. Visual acuity decreased to hand motions during a four-month course. Successful extracapsular cataract extraction occurred seven months following the original radial keratotomy. Soft contact lens correction of aphakia recovered a visual acuity of 6/6.

Late dislocation of an anterior chamber intraocular lens into the vitreous.

We report the case of a patient with dislocation of an anterior chamber intraocular lens (AC IOL) into the vitreous, 5 years after surgery. The cause of the dislocation may have been the size of the pupil and habitual eye rubbing. This case report describes a rare complication of AC IOL implantation in eyes of previous iris lesions or pupil abnormalities. This complication should be kept in mind when an AC IOL implantation is planned for the correction of aphakia.

[Management of traumatic cataracts].

Traumatic cataract is the severe result of an accident often concerning young patients, sometimes children. Anatomical and clinical presentations vary depending on whether the lens is luxated, the wound involves perforation, or there is an intraocular foreign body. The associated wounds and the late complications, particularly retinovitreous ones, can impair the visual prognosis sometimes over the very long term. The correction of aphakia with an intraocular lens should be decided on a case by case basis.

Stereopsis after congenital monocular cataract extraction.

Congenital monocular cataract extraction was performed on a 1-day-old baby girl and a hydrophilic contact lens was fitted. A progressive regimen of part-time occlusive therapy was then instituted. Eight years later, the girl has a visual acuity of 20/25 in the operated-on eye and 50 seconds of arc of stereoacuity that has been confirmed by an independent pediatric ophthalmologist. This patient demonstrates the potential for high-grade stereopsis in congenital monocular aphakia.

Posterior Chamber Scleral Fixation of Intraocular Lenses in Post-Vitrectomised Aphakic Eyes.

INTRODUCTION: The best method of aphakia correction is in the bag implantation of Posterior Chamber Intraocular Lens (PCIOL). When this ideal procedure is not possible due to lack of integrity of posterior capsule or zonules, the other alternatives are broadly categorized into two: extraocular and intraocular. Whereas, the former includes contact lenses and aphakic glasses, the latter ones are further divided into anterior and posterior chamber methods. Anterior Chamber Intraocular Lenses (ACIOL) can be with or without iris claw. At the posterior chamber, fixation of the lenses can be with glu…

A retrospective analysis of surgical outcome in sutureless glueless scleral fixation of foldable intraocular lens.

INTRODUCTION: Cataract surgery constitutes one of the highest volume surgeries performed worldwide approximating 10 million annually, varying from 100 to 6000 per million population. Implantation of scleral fixated intra-ocular lens (SFIOL) is getting popular in managing difficult situations like aphakia, subluxated lens, and dislocated cataracts. OBJECTIVE: This retrospective series evaluated our experience with foldable 3-piece acrylic SFIOL implantation for various challenging surgical scenarios in a tertiary care ophthalmic institute in the southern part of India. RESULTS: The data from …
