[ENT medical findings in obstructive sleep apnea syndromes].

Correlations between the manifestation of obstructive sleep apnoea syndrome (OSAS) and anatomical or functional changes in the upper respiratory tract remain controversial. The correlation between obstruction of the upper respiratory tract and the degree of sleep apnoea syndrome was investigated in 60 patients with obstructive sleep apnoea (diagnosed by polysomnography) and in 55 healthy controls. After clinical examination, rhinomanometry and determination of the size of the lower jaw and oropharynx, the motility of the pharyngeal walls during Mueller's manoeuvre was evaluated by flexible end

Magnesium therapy in premature neonates with apnea neonatorum.

Apnea, bradycardia, and neuromuscular hyperirritability have been associated with magnesium (Mg) deficiency in young human infants and weanling animals. This is a retrospective review of a clinical experience of Mg therapy among 200 premature neonates who showed physical and clinical chemical changes compatible with Mg deficiency. The 200 infants all had idiopathic apnea neonatorum, and 93% also had the respiratory distress syndrome (RDS). This review was conducted to learn whether Mg therapy was associated with a significant reduction in apnea. The author suggested that the dose of Mg be 0.4 …

Obstructive sleep apnea following bilateral carotid body resection.

A patient who had undergone bilateral carotid body resection five years earlier for palliation of chronic airflow obstruction was found to have severe obstructive sleep apnea. He presented with hypercapnic respiratory failure, which improved after tracheostomy. A physiologic mechanism is proposed to explain this association. Previously reported studies of anesthetized animals suggest that loss of peripheral chemoreceptor activity could selectively decrease neural output to the genioglossus, the main protrusor muscle of the tongue, predisposing the upper airway to inspiratory occlusion.

Determination of obstructive site in obstructive sleep apnea.

Patients with obstructive sleep apnea syndrome (OSAS) may have airway obstruction at various levels, including the uvula-soft palate complex, base of tongue, and/or possibly other sites. For patients with tongue base and/or laryngeal obstruction, uvulopalatopharyngoplasty (UPPP, ppp) will not alleviate the obstruction. Prior authors have proposed that the hyoid bone position as determined by cephalometric x-rays can predict which patients have obstruction at a lower site than the soft palate. In this study, patients with obstructive sleep apnea syndrome were evaluated with polysomnographic tes…

Obstructive sleep apnea in adults with tonsillar hypertrophy.

Tonsillar enlargement is a more common cause of obstructive sleep apnea (OSA) in the pediatric population than it is in adults. The small number of reported cases suggests that tonsillectomy for adult patients with this association may be as a successful as it is known to be for children. We compared polysomnographic findings and/or symptomatology both before and after tonsillectomy in six patients aged 22 to 52 years, who had adenotonsillar hypertrophy and OSA. Tonsillectomy provided lasting relief of OSA in four patients. We review the literature for available polygraphic data from similar p…

Extreme bradycardia during sleep apnea caused by myxedema.

A 52-year-old man with myxedema was evaluated for anterior chest pain that was considered to be compatible with myocardial ischemia. The nig … adequate central neural drive, peripheral muscle force, and obesity all may have contributed to the apnea. Chest pain has not recurred, and results of electrocardiography have remained normal following full thyroid hormone replacement. The early recognition of myxedema causing sleep apnea will allow specific treatment to avoid the cardiovascular risks r …

Sleep parameters after surgery for obstructive sleep apnea.

Obstructive sleep apnea (OSA) is a disorder that is frequently treated surgically. Few reports in the literature give objective reports of the effect of such treatment on polysomnographic parameters before and after treatment, although symptomatic improvement is common. This article reviews total sleep time (TST) spent in apnea, number of apneic episodes per hour or sleep apnea index (SAI), and oxygen desaturations in such patients treated with tracheostomy, uvulopalatopharyngoplasty (UPP), and combinations of these. Although improvement is noted in these parameters after UPP, this is not as i…

Overnight nasal CPAP improves hypersomnolence in sleep apnea.

Eleven adult men with sleep apnea underwent nocturnal polysomnography on two successive nights. The first study, done without NCPAP, served as the control. The second (treatment) was done with the application of 7.5 to 15 cm H2O nasal continuous positive airway pressure (NCPAP). A subjective sleepiness index (SSI) was noted upon awakening from each night of polygraphic recording. During the control night, the mean frequency of apnea episodes/sleep hr was 35.95 +/- 4.5 SE, and the mean duration was 28.68 +/- 2.7 sec. Mean frequency of disorder of breathing (DOB) episodes/sleep hr was 19.25 +/- …

Undiagnosed sleep apnea in patients with essential hypertension.

The prevalence of sleep apnea was studied in 46 middle- and older-aged men with "essential hypertension." Thirty-four age- and weight-similar normotensive men were also studied. Fourteen hypertensive men and three controls had sleep apnea syndrome, as defined as greater than ten apneas per hour of sleep. Hypertensive men with apnea tended to be more overweight and slightly older than the hypertensive men without apnea, but differences were not statistically significant. Individual men with apnea could not be distinguished by their answers on a questionnaire that elicited symptoms related to ap…

Palatopharyngoplasty failure, cephalometric roentgenograms, and obstructive sleep apnea.

Nine patients with obstructive sleep apnea who underwent unsuccessful palatopharyngoplasty (PPP) as documented by polygraphic monitoring had abnormal cephalometric roentgenogram measurements. Findings indicated a small posterior airway space and inferiorly placed hyoid bone. Cephalometry performed with appropriate techniques to investigate soft tissue location should be obtained systematically in obstructive sleep apneic patients before any surgery is performed. The roentgenogram finding is a helpful guide in deciding whether PPP alone or PPP in combination with other surgical procedures would…

[Nocturnal cardiac arrhythmias in the sleep apnea syndrome].

A day vs. night comparison of type and frequency of cardiac arrhythmias documented by long-term ECG was done in 30 patients with nocturnal respiratory disorders in the form of the sleep apnoea syndrome. The commonest nocturnal arrhythmia was pronounced sinus arrhythmia during the apnoeic phases with variations of cardiac frequency between 40 and 120/min. Supraventricular and ventricular extrasystoles occurred significantly more frequently during the night than during the day. In three patients bradycardiac arrhythmias presenting as sinoauricular and atrioventricular blockades of the 3rd degree

Auditory middle latency-evoked potentials during sleep apnea.

Auditory middle-latency evoked potentials (AMEP) were recorded in 10 subjects with predominantly central sleep apnea. AMEP were recorded during: waking; non-rapid eye movement (non-REM) sleep between apneic episodes; first half of apneas; and second half of apneas. Latencies of vertex positive peaks Po and Pa were determined, and the effect of the apnea phase on these latencies was evaluated. The latencies measured did not reveal chronic or acute functional abnormality of central auditory structures, which may result from or cause apneic episodes during non-REM sleep. The normalcy and stabilit…

Autonomic dysfunction and sleep apnea in olivopontocerebellar degeneration.

We studied ten men with olivopontocerebellar degeneration. Our findings of the autonomic deficits as manifested by orthostatic hypotension, impaired Valsalva's response, abnormal findings on cold pressor and mental arithmetic tests, and plasma renin and norepinephrine abnormalities in some patients with olivopontocerebellar degeneration suggested a defective central control of the sympathetic nervous system. Five patients had sleep apnea. Autonomic dysfunction and sleep apnea in olivopontocerebellar degeneration may result from degenerative lesions in the cerebellum and the brain stem.

Nasal CPAP effect on patterns of sleep apnea.

To evaluate the effect of continuous positive airway pressure via nasal mask (nasal CPAP) on occlusive, mixed, and central apneas (OA, MA, and CA, respectively), we performed nocturnal polysomnography without and with nasal CPAP on 21 patients with sleep apnea. Three patients were unable to tolerate nasal CPAP. The remaining 18 patients had significant reductions in the overall apnea frequency when using nasal CPAP (52.9 +/- 5 per hour slept vs 3.3 +/- 1 per hour slept, mean +/- SE, p less than 0.001). The use of nasal CPAP significantly reduced the frequency of OAs during both nonrapid eye mo…

Life-threatening apnea in infants recovering from anesthesia.

To determine whether prematurely born infants with a history of idiopathic apneic episodes are more prone than other infants to life-threatening apnea during recovery from anesthesia, the authors prospectively studied 214 infants (173 full term, 41 premature) who received anesthesia. Fifteen premature infants had a preanesthetic history of idiopathic apnea. Six of these required mechanical ventilation because of idiopathic apneic episodes during emergence from anesthesia. Two were ventilated for other reasons, and seven recovered normally. Infants ventilated for apnea were younger (postnatal a…

Endoscopic findings in sleep apnea associated with acromegaly.

Two acromegalic patients presented with the additional complaint of excessive daytime sleepiness. Subsequent sleep laboratory evaluation revealed that each patient had concurrent obstructive sleep apnea. In each patient, endoscopic examination of the upper airway revealed that on inspiration, the soft tissue of the posterior and lateral hypopharynx invaginated into the laryngeal vestibule before any posterior movement of the tongue. Neither enlargement of the tongue nor in movement in relation to respiration appeared to be the primary factors in the etiology of the upper airway obstruction. Af…

Apnea of infancy and sudden infant death syndrome.

The sudden infant death syndrome (SIDS) is the leading cause of death of infants between the end of the first month and the first year of life. The risk of SIDS is increased in subsequent siblings of infants with SIDS, infants with apnea of infancy (AI), and prematurely born infants. The cause of SIDS is unknown, but much clinical and investigative attention has been directed toward AI as a possible premortem expression of the abnormality that can potentially cause SIDS. Both AI and SIDS probably have multiple different mechanisms. Some infants with AI have been shown to have abnormal control …

Narcolepsy with concomitant features of obstructive sleep apnea.

A 17-year-old man presented with daytime sleepiness, episodic attacks of sleep and probable cataplexy. His EEG showed rapid eye movements and central sawtooth waves at sleep onset, and supported the clinical impression of narcolepsy. He improved with methylphenidate but died suddenly, and had cardiomegaly, right ventricular enlargement, and pulmonary hypertension at autopsy. These findings suggested concomitant features of sleep apnea which were not evident by history or examination.Central apneas have been frequently described in the sleep of narcoleptic patients. Few patients have had indica…
