Bacterial infection of the sternoclavicular joint is an unusual event, with cases being reported in those with diabetes mellitus, in intravenous drug abusers, and in patients afflicted with rheumatoid arthritis. A case of this unique infection occurred in a person not known to be at risk for septic arthritis. Our report shows the difficulty in diagnosing this disorder.

The origin of an infection of the knee joint can be of an endogenous/hematogenous nature or, in posttraumatic cases, of an exogenous nature. Hematogenous infections are found mostly in children, but also in patients with reduced immune functions, e.g. after organ transplantations. Posttraumatic joint infections are caused by open injuries, intraarticular injections, operations, or they can be transmitted from the surroundings. They are relatively rare after accident surgery and orthopedic surgery. Two statements are of special importance for pathophysiology: 1. An intraarticular infection will

Porcine articular cartilage from cases of experimentally induced Erysipelas arthritis was examined with different histological techniques and by electron microscopy. A small percentage of specimens of osteoarthritic cartilage revealed an interlacunar network in the extracellular matrix. These "matrix-streaks" showed characteristic features of densely packed fibrillar elements and collagenous fibrils, as narrow bands connecting adjacent chondrocytes. In a given cartilage site the network had a constant pattern. It is hypothesized that the network formation is correlated with altered joint metab

A description of the protracted course of kingella monoarthritis is given as an illustration of the importance of accounting for the slow growth, low viable numbers, and fastidious culture requirements of microbes that may be encountered in synovial samples. The practice of carrying out synovial fluid cultures in the same way as blood cultures is recommended.

A known pediatric hemophiliac was evaluated for what was thought to be a hemarthrosis of the knee. When the patient failed to respond to conventional treatment, the joint was aspirated and found to be septic. Septic arthritis is a rare complication of hemophilia and requires prompt recognition and treatment.

A 16-year-old Mexican-American boy presented with acute brucella sacroileitis. The diagnosis was based on a significantly elevated agglutination titer and confirmed by the growth of B. melitensis after four weeks incubation in blood culture. The patient recovered completely after six weeks of Tetracycline therapy. Because Brucella infection responds to treatment well, this infection is important in the differential diagnosis of arthritis, especially in an area endemic for Brucella. The readily available serologic techniques should be used to support the diagnosis, while recognizing that becaus

A review of the clinical features of seven patients with sporotrichosis arthritis showed that six had joint infection without previous skin or lung involvement and that one with myelofibrosis had joint and skin infection. The average time from onset of joint symptoms to diagnosis was 25 months, resulting in joint damage that required arthrodesis in four patients. Tissue from open synovial biopsy was superior to synovial fluid for obtaining a positive culture; concomitant synovial fluid and synovial tissue cultures were superior to either one alone. Granulomatous inflammation was seen in synovi
