Metabolic correlates of learning disability.

To summarize, the neuropsychologic findings in MSUD and MMA children in both groups demonstrated deficits in cognitive/language areas, but interesting individual differences existed. For example, a marked contrast in abilities existed between NT and GV, even taking into account the age difference between these children with MSUD. While NT's general intellectual functioning was within the low-average range, GV was severely impaired. In more specific areas of cognitive functioning, NT had an uneven performance profile with areas of strengths and weaknesses, while GV evidenced a flat profile with

Proactive interference in a semantic short-term memory deficit: role of semantic and phonological relatedness.

Previous research has indicated that patients with semantic short-term memory (STM) deficits demonstrate unusual intrusions of previously presented material during serial recall tasks (Martin and Lesch, 1996). These intrusions suggest excessive proactive interference (PI) from previous lists. Here, we explore one such patient's susceptibility to PI. Experiment 1 demonstrated patient M.L.'s extreme susceptibility to PI using a probe recognition task that manipulates the recency of negative probes (the recent negatives task). When stimuli consisted of letters, M.L. showed greatly exaggerated eff

Language disturbances in a group of participants suffering from Duchenne muscular dystrophy: a pilot study.

Results from several studies suggest that the process of language acquisition may be altered in patients suffering from Duchenne Muscular Dystrophy. In this study, a group of 8 male participants with Duchenne Muscular Dystrophy (M age = 16 yr., SD = 4.7) underwent an extensive neuropsychological and language assessment. They also performed a discourse production task. Results showed mild mental retardation associated with a specific deficit in Verbal rather than Performance IQ. At the linguistic assessment, 7 of 8 participants showed moderate to severe difficulties on oral language processing

Acoustic analysis of parkinsonian speech I: speech characteristics and L-Dopa therapy.

This paper reviews the literature pertaining to Parkinson's disease (PD) and the speech dysfunction typically associated with PD, including the effects on respiration, phonation, articulation, resonance, and prosody. The effect of treatment with the drug L-Dopa is also examined, along with the effect of L-Dopa treatment on Parkinsonian speech. This paper is the first of a two-part series. Part two examines the literature pertaining to the fluctuations that can occur during treatment with L-Dopa, the speech changes associated with these fluctuations, and methodological issues affecting the exam

Speech and swallowing function after oral and oropharyngeal resections: one-year follow-up.

BACKGROUND: The purpose of this study was to determine whether the speech and swallowing function of surgically treated oral cancer patients improves between 1 month and 1 year after surgery. METHODS: Speech and swallowing performances were assessed for 28 men and 10 women preoperatively and at 1, 3, 6, and 12 months postoperatively following a standardized protocol. Speech tasks included an audio recording of a brief conversation and of a standard articulation test; swallowing function was examined using videofluoroscopy. Data were also collected on the number and duration of speech/swallowi

A stenography of empathy: Toward a consensual model of the empathic process.

Empathy has gained popularity in the general population and the scientific world during the past decade. Recently, several researchers found a significant decrease in empathy scores of healthcare students (notably medical students) and recommend promoting empathy skills in several fields of education. The current paper presents a new model of the empathic process: a stenography of empathy compelling scientific data and contemporary conceptions. Indeed, we combined all pioneer researchers' conceptions of empathy (Davis, Decety, Batson, Preston & de Waal) into an integrative model. This model is

Nursing people diagnosed with Borderline Personality Disorder: 'We all need to be on the same hymn sheet'.

The diagnosis of borderline personality disorder (BPD) has been found to carry stigma and poor hope of recovery. More recently, it has been regarded as a treatable condition through psychotherapy. Despite this, patients often experience lengthy hospitalizations, limited access to treatment, and poor outcomes. This paper describes the experiences of psychiatric nurses working with people diagnosed with BPD in acute mental health in-patient settings in Ireland. Seven nurses were interviewed, and the transcripts were analysed using a reflective and inductive approach. Overall, the nurses did not

'Often there is a good deal to be done, but socially rather than medically': the psychiatric social worker as social therapist, 1945-70.

Seeking to align psychiatric practice with general medicine following the inauguration of the National Health Service, psychiatric hospitals in post-war Britain deployed new treatments designed to induce somatic change, such as ECT, leucotomy and sedatives. Advocates of these treatments, often grouped together under the term 'physical therapies', expressed relief that the social problems encountered by patients could now be interpreted as symptomatic of underlying biological malfunction rather than as a cause of disorder that required treatment. Drawing on the British Journal of Psychiatric So

Thoracic aortic pseudoaneurysm after spine trauma in ankylosing spondylitis. Case report.

Ankylosing spondylitis (AS) is a rheumatic disease characterized by consolidation of the articulating surfaces and inflammation of the vertebral column. Because of its associated spine stiffness and secondary osteoporosis, patients with this disorder are at increased risk of vertebral fractures. Ankylosing spondylitis presents a significant challenge to spine surgeons because of its complex effects on the spine, extraarticular organ manifestations, and potential neurological and functional sequelae. Traumatic thoracic and lumbar spine injuries in this patient population may be associated with

[Chiropractic in general and in low back pain].

The practice of chiropractic was for many years regulated by "The Quack Act" in Norway, and the numbers of chiropractors decreased year by year. They are now authorized health care practitioners with academic training; most Norwegian students attending courses in chiropractic or clinical biomechanics go to European universities. An international council ensures reciprocity and a quality assured academic programme in all recognized colleges of chiropractic. Recent research have broadened our understanding of the biomechanical interrelationship between the nervous system, the musculature and the

Disfluency in spasmodic dysphonia: a multivariate analysis.

This study examined visual analog scaling (VAS) judgments of disfluency by normal listeners in response to oral reading by speakers with spasmodic dysphonia (SD) and by nondysphonic controls, as well as the variables of frequency of occurrence of disfluencies, speaking rate, number of reading errors, and temporal acoustic measures of interword interval duration and articulation time. MANOVA yielded statistically significant differences between SD and control speakers for all variables except reading errors. Although no significant fluency-related differences were observed in terms of type of v

Characterizing the morphosyntactic processing deficit and its relationship to phonology in developmental dyslexia.

This study explores the morphosyntactic processing deficit in developmental dyslexia, addressing the on-going debate on the linguistic nature of the disorder, and directly testing the hypothesis that the deficit is based on underlying processing difficulties, such as acoustic and/or phonological impairments. Short German sentences consisting of a pronoun and a verb, either correct or containing a morphosyntactic violation, were auditorily presented to 17 German-speaking adults with dyslexia, and 17 matched control participants, while an EEG was recorded. In order to investigate the interaction

Measuring severity of involvement in speech delay: segmental and whole-word measures.

PURPOSE: This study examined whether any of a series of segmental and whole-word measures of articulatory competence captured more of the variance in impressionistic ratings of severity of involvement in speech delay. It also examined whether knowing the age of the child affected severity ratings. METHOD: Ten very experienced speech-language pathologists rated severity of involvement from conversational speech samples obtained from 17 children with delayed speech. The ratings were then correlated with the candidate measures. The ratings by those who knew the ages of the children were also com

Investigations of a pressure-sensitive theory of marginal velopharyngeal inadequacy.

PURPOSE: This two-part project was designed to test a pressure-sensitive theory of marginal velopharyngeal inadequacy (MPVI). Specifically, are select subgroups of children with MPVI perceived as hypernasal because they fail to achieve consistent closure during vowels and semivowels while demonstrating adequate closure during pressure consonants? METHODS: In part one, 36 children with cleft palate and other craniofacial anomalies were examined using a clinical assessment protocol that included nasometry and perceived ratings of hypernasal resonance. Children with nasalance percentages above t

Failed pharyngoplasty and subsequent management.

An investigation of 18 patients who had experienced "surgical failure' following pharyngoplasty for the correction of velopharyngeal dysfunction was undertaken in an attempt to identify the causes. These included poor case selection, poor surgical design and execution. Subsequent management of this group included three modalities of treatment; of the three, a carefully designed and executed surgical procedure produced the most satisfactory results in this particular patient cohort.

Toffee clearance and lingual sensory and motor activities in normal children and in children with articulation problems of speech.

The rate at which a standardized mouthful of toffee containing technetium (99mTc) tin colloid was cleared from the mouth during and after chewing was measured by counting the residual radioactivity in the mouth at short time intervals by external gamma counting. The clearance curve was resolved into three phases for each subject--chewing time (1), which coincided with the time to reach 10% of the initial counting rate, scavenging time (2), and a final slow diffusion phase (3). Although the mean chewing time was only 1.9 min, the technique allowed the pattern of clearance during this phase to b

Vocal fold paresis in Shy-Drager syndrome.

Twelve patients with Shy-Drager syndrome (SDS) presenting symptoms of multiple nervous system atrophy and orthostatic hypotension were examined for laryngeal movement disorders and vocal impairment in speech. Vocal fold abductor paresis was found in 11 patients and was bilateral in 10. Speech task performance was recorded in SDS patients, Parkinson patients and age- and sex-matched controls. Trained listeners with inter-rated reliability greater than or equal to .85 judged each recording on 20 attributes while blind to speaker identity. SDS patients had a breathy and strained voice quality, re

Spinal or cortical direct current stimulation: Which is the best? Evidence from apraxia of speech in post-stroke aphasia.

To date, new advances in technology have already shown the effectiveness of non-invasive brain stimulation and, in particular, of transcranial direct current stimulation (tDCS), in enhancing language recovery in post-stroke aphasia. More recently, it has been suggested that the stimulation over the spinal cord improves the production of words associated to sensorimotor schemata, such as action verbs. Here, for the first time, we present evidence that transpinal direct current stimulation (tsDCS) combined with a language training is efficacious for the recovery from speech apraxia, a motor spee

Protocol for the Connected Speech Transcription of Children with Speech Disorders: An Example from Childhood Apraxia of Speech.

OBJECTIVE: While it is known that connected speech has different features to single-word speech, there are currently few recommendations regarding connected speech transcription. This research therefore aimed to develop a clinically feasible protocol for connected speech transcription. The protocol was then used to assist with description of the connected speech of children with childhood apraxia of speech (CAS), as little is known about their connected speech characteristics. PARTICIPANTS AND METHODS: Following a literature review, the Connected Speech Transcription Protocol (CoST-P) was ite
