Cardiac troponin I concentrations in neonates with hypoxic-ischaemic encephalopathy.

AIM: Myocardial dysfunction is a frequent sequel of perinatal asphyxia. Cardiac troponin I (cTnI) is a marker of myocardial injury and a surrogate marker of myocardial dysfunction in adults, but there are few data in neonates. Our aim was to compare serum cTnI concentrations with clinical severity of encephalopathy and with duration of inotropic support in asphyxiated neonates. METHODS: Retrospective study of 60 neonates admitted with hypoxic-ischaemic encephalopathy (HIE). cTnI concentrations measured within 36 h of birth were compared with clinical grade of HIE (Sarnat-Sarnat classification

Quantitative EEG in babies at risk for hypoxic ischemic encephalopathy after perinatal asphyxia.

OBJECTIVE: To evaluate an electroencephalography (EEG)-based index, the Cerebral Health Index in babies (CHI/b), for identification of neonates with high Sarnat scores and abnormal EEG as markers of hypoxic ischemic encephalopathy (HIE) after perinatal asphyxia. STUDY DESIGN: This is a retrospective study using 30 min of EEG data collected from 20 term neonates with HIE and 20 neurologically normal neonates. The HIE diagnosis was made on clinical grounds based on history and examination findings. The maximum-modified clinical Sarnat score was used to grade HIE severity within 72 h of life. Al

[Prognostic value of MR in term neonates with neonatal hypoxic-ischemic encephalopath: MRI score and spectroscopy. About 26 cases].

UNLABELLED: Neonatal hypoxic-ischemic encephalopathy remains a major cause of chronic disability in childhood. Early diagnosis and prognosis are necessary for the clinician to adapt the treatment. However, there is yet no reliable test to predict the patient's evolution. OBJECTIVE: The aim of our study was to evaluate the predictive value of a personal magnetic resonance imaging (MRI) scoring system and of magnetic resonance spectroscopy (MRS). MATERIAL AND METHODS: We included 26 term newborns in condition of neonatal brain suffering. MR examination was performed during the first week of li

Hypothermia.

Experimental studies show that, following hypoxic ischaemic injury, mild induced hypothermia-a reduction of body temperature by about 3 degrees C -- preserves cerebral energy metabolism, reduces cerebral tissue injury and improves neurological function. Randomized trials in full-term and near-full-term newborns suggest that treatment with mild hypothermia is safe and improves survival without disabilities up to 18 months of age. Although the optimal time of initiation, the depth and duration, and the method of cooling are uncertain, in the absence of specific treatments many clinicians will wi

Corpus callosum atrophy in adolescents with antecedents of moderate perinatal asphyxia.

BACKGROUND: The corpus callosum (CC) is a cerebral structure that reflects cognitive status in several neurological pathologies. Visual inspection of MRI has shown that hypoxic-ischemic encephalopathy(HIE) causes callosal damage. PRIMARY OBJECTIVE: To quantify the CC surface in a sample of patients with antecedents of HIE and a group of matched controls. RESEARCH DESIGN: Comparisons of CC measures among control subjects, mild HIE patients and moderateHIE patients as well as correlates of CC surface and neuropsychological performance. METHODS: Twenty-one adolescent patients with childhood an

Decision-making: initiation and withdrawing life support in the asphyxiated infants in developing countries.

The issues of life support in the asphyxiated infant are not only whether cardiopulmonary resuscitation or CPR will be successful, but also whether if successful, the infant will be severely damaged. This is particularly important in the developing countries because the damaged infants may burden the society. The country has to allocate huge financial and human resources to look after them. When it comes to decisions in initiation and withdrawal of life support, there are differences between the East and the West. Physicians are searching for reliable predictors of outcome of term asphyxiated

[Blood neuronal specific enolase in newborns with perinatal asphyxia].

INTRODUCTION: Neuron-specific enolase (NSE) is a sensitive marker of brain injury after hypoxia or ischemia. There are few studies about its usefulness in asphyctic newborns. OBJECTIVE: To study the correlation between blood NSE levels and neurological outcome in newborns with hypoxic ischemic encephalopathy. PATIENTS AND METHODS: We have determined the blood values of NSE by radioimmunoassay in 25 asphyctic term-newborns with clinical encephalopathy (of mild, moderate and severe degree) and in 22 healthy term newborns (control group). Blood samples were obtained between 24 and 72 hours afte

Measurement of the urinary lactate:creatinine ratio for the early identification of newborn infants at risk for hypoxic-ischemic encephalopathy.

BACKGROUND: Newborn infants with perinatal asphyxia are prone to the development of hypoxic-ischemic encephalopathy. There are no reliable methods for identifying infants at risk for this disorder. METHODS: We measured the ratio of lactate to creatinine in urine by proton nuclear magnetic resonance spectroscopy within 6 hours and again 48 to 72 hours after birth in 58 normal infants and 40 infants with asphyxia. The results were correlated with the subsequent presence or absence of hypoxic-ischemic encephalopathy. RESULTS: Hypoxic-ischemic encephalopathy did not develop in any of the normal

Preserving brain function during neonatal asphyxia.

Presently, there is no intervention that can be used that preserves brain function in hypoxic-ischemic encephalopathy. This review covers the interventional strategies that are available to the health care worker at present and suggests a different approach to this frustrating problem. The proposal is to use clinically available interventions and combine them into an effective intervention strategy. The merits of each intervention is discussed.

An international network for evaluating neuroprotective therapy after severe birth asphyxia.

Animal studies have shown great promise in their applicability to potentially neuroprotective therapies for severe birth asphyxia in human babies. It is now necessary to consider a strategy to evaluate some or all of these techniques within the context of human neonatal randomized control trials (RCT). We have set up a pilot study for an international RCT of mature babies with severe asphyxia (defined by an Apgar score of 5 or less at 10 minutes) and have shown that we can recruit from 120 centers in 17 countries an average of three babies a week, which is the required number to undertake a st
