The SAPS score proposed by Le Gall, was used in 128 patients undergoing biliary surgery between January 1987 and April 1988. The score is based on the laboratory data for the first day. The cumulative post-operative complication rate was established during hospital stay and at one month after discharge. There is a highly significant correlation between the SAPS score, the morbidity and the mean hospital stay. From this study it seems valuable to use this score in surgery reports.

Twelve of 123 patients who were treated for benign stricture of the bile duct after cholecystectomy presented with an external fistula of the biliary tract. Nine of these patients had undergone multiple operations prior to referral, six having undergone earlier attempts at repair of the bile duct. The management of gallstones in patients with acute pancreatitis (AP) is controversial. This paper reports an analysis of 171 attacks of AP associated with gallstones. Whilst awaiting elective cholecystectomy 56% of patients had further symptoms and 1 died. Non-urgent operations during the same admission as the pancreatitis (n = 34) resulted in 2 deaths and 7 septic complications, but prediction of complications may be possible. The age-adjusted mortality rates and trends of primary liver cancer in Slovakia showed during the investigated decade 1968-1977 small reduction in males, accompanied with statistically significant decline in females. As a diagnostic aid of malignant hepato-biliary tract disease, carcinoembryonic antigen (CEA) levels in the bile and serum were evaluated in 12 patients with benign and 19 patients with malignant hepato-biliary diseases. Ultrasound was found to be useful in the evaluation of jaundice. Extrahepatic obstruction can be diagnosed by the presence of dilated intra- or extrahepatic bile ducts. Using a high-resolution real-time mechanical sector scanner, it was possible to show the common hepatic duct as it passes anterior to the right portal vein in 98 out of 100 consecutive adults without biliary tract disease. At this level, the normal duct measures 4 mm or less in internal diameter. All patients with a duct larger than 5 mm thus far have had either biliary tract obstruction or previous cholecystectomy.

PURPOSE OF REVIEW: Biliary tract cancer (BTC) is a heterogeneous group of malignancies with limited systemic treatment options and poor prognosis. While immunochemotherapy has become the first-line standard, second-line therapy remains inadequate. Antibody-drug conjugates (ADCs) have emerged as a promising therapeutic modality leveraging antigen-specific targeting to deliver potent cytotoxic payloads. Breast metastasis from biliary tract cancer (BTC) is an extremely rare clinical occurrence. We report the case of a 59-year-old woman who was incidentally found to have a hepatic mass, leading to a comprehensive workup prompted by progressive chest discomfort.
