Assisted Suicide (AS)/Assisted Dying (AD), where a physician prescribes lethal medication to a terminally ill patient who intends to take it to end his/her life, is a highly controversial matter. Sound knowledge about recent legal changes and related challenges should inform considerations for clinical ethics consultation, education, and policy making. This article describes a wide spectrum of current legal contexts for AS/AD in the United Kingdom, Italy, Austria, the United States (US), Switzerland, and Germany. This selection includes countries covering a wide range from the restrictive to t

Ethical human subjects research must have an acceptable risk-benefit ratio, which requires that the net risks participants face be proportionate to the research's social value. Yet existing scholarship does not explain what makes risks proportionate to social value. Here, I distinguish internal and external senses of proportionality and argue that it is internal proportionality that is the proper object of IRB/REC review. I propose understanding internal proportionality in terms of a literal ratio of expected health effects; I also argue against the alternative of attempting to assess proporti

Debates in public health ethics have been dominated by the assumptions of Western liberalism: a priority given to liberty and autonomy over other values, an individualistic view of social ontology, a focus on personal responsibility, a minimal set of obligations (only created through consent), and a marginalization of social, cultural, and religious context. Examining issues in public health ethics from the perspective of a different moral tradition reveals that such assumptions are not timeless truths, but rather choices that require justification. In this paper, we briefly outline a view of

Artificial intelligence (AI)-generated synthetic data has emerged as a promising solution to address the underrepresentation of underserved populations in medical AI systems. By artificially generating data that mimics real-world patient information, proponents argue that AI-generated synthetic data can fill data gaps, improve algorithmic fairness, and mitigate bias without requiring costly data collection or raising privacy concerns. However, in this article, I challenge this optimistic view. I argue that AI-generated synthetic data may amplify existing biases rather than mitigate them, and c

Government health research funders are frequently criticized on the grounds that their distribution of resources fails to reflect a population's disease burden. These critiques do not take into account what other research funders are doing. In this paper, I argue that this is a mistake. Both governmental and non-profit research funders ought to be aiming for a just distribution of overall research resources. Individual funders faced with the non-compliance of other funders therefore have two ethical obligations. First, an obligation to coordinate: each ought to share information about its fund

Scientific advancements offer new possibilities for developing novel treatments for children with monogenic neurodevelopmental disorders (mNDDs), targeting the underlying effects of their condition. Although, in qualitative studies, parents of children with mNDDs have predominantly positive attitudes towards the development of novel treatments, a few parents express concerns about whether treatments could change who their child is. Existing literature on personal identity, including literature on how personal identity is affected by medical (neuro-)interventions, can assist in clarifying these

Data-intensive health research complicates traditional requirements for informed consent, due to the amount of data contributors and data points, the open-ended nature of research questions, and repeated or secondary data use. To still enable data-intensive health research, there is a move towards other-generally lower-standards for consent such as opt-out systems, in current developments such as the European Health Data Space (EHDS). This is premised on the social value that data-intensive health research is assumed to provide through advancing research and innovation. Traditionally, social v

Artificial womb (AW) prototypes are currently being developed with the aim of improving the medical care of extremely premature infants. Despite the seemingly imminent reality of partial ectogenesis (i.e., gestation partially outside a human womb), there is persisting debate about the moral status of the fetus transferred in an AW-henceforth, the "developing entity." For some, AWs are simply another neonatal intensive cares' technology. Thus, developing entities in AWs should deserve the same protections as newborns in incubators. Others consider that AWs are fundamentally different technologi

Molecular HIV surveillance, involving the collection and analysis of HIV genome sequences, has become an integral part of public health programmes in high-income countries. By employing phylogenetic analysis, molecular HIV surveillance can identify individuals and their positions within networks of HIV transmission. While the primary aim of molecular surveillance is to yield public health benefits, such as linking people to care and reducing transmission, it also poses risks and potential infringements on individual privacy and liberty. This paper examines the ethical implications of using phy

It is plausible that time of birth affects one's prospects for wellbeing. Being born during a war or recession might have a negative impact on early life and lifetime wellbeing. In natural reproduction, delaying conception does not result in the same child being born later, but rather a different child altogether; therefore, prospective parents cannot harm/benefit their children by choosing their time of birth. However, we argue that for prospective parents undergoing the IVF process, things are different. Since it is possible to freeze and store embryos indefinitely before implantation, paren

Philosophers have debated on the "mere-difference" view of disability, according to which disability as such is neutral in terms of well-being, just like race and gender. It is contrasted with the "bad-difference" view, which holds that disability is bad for its possessor even in a non-ableist situation. We first illustrate how neither view can be sensitive to the diversity of disabled people and their disabilities. Subsequently, we propose an alternative outlook-the conditional bad-difference view of disability: a disability is bad for its possessor if and only if it hampers her aspired way o

The COVID-19 pandemic was a devastating worldwide event with great consequences on medical and surgical care that has not been fully evaluated. The pandemic caused an imbalance between rapidly increasing patient needs and limited hospital resources such as staff, personal protective equipment (PPE), and open beds. As a result, surgeons in the United States were encouraged or required to postpone "non-emergent" operations, including some cancer surgeries, in March of 2020. Resource allocation during the pandemic focused on the needs of the community to accommodate potential surges, and cancer p

In this paper, I defend two claims about best practices for genetic counselors advising patients in the reproductive context. The first claim is that defenders of non-directiveness about selection against disability traits should support directiveness in favor of testing for disability traits. The second claim is that genetic counselors can be non-directive about selection against disability traits yet directive about testing for those traits-there is no tension between these two positions. So, it is open to defenders of non-directiveness about selection to be in favor of directiveness about t

This study examines the ethical permissibility of biomedical moral enhancement (BME) for psychopaths, considering both coercive and voluntary approaches. To do so, I will first briefly explain what psychopaths are and some normative implications of these facts. I will then ethically examine three scenarios of BME for psychopaths: (1) coercive BME for non-criminal psychopaths, (2) coercive BME for psychopathic offenders, and (3) voluntary BME for psychopathic offenders. I will argue that coercive BME for non-criminal psychopaths is ethically problematic due to issues of cost, invasion of privac

Because many involuntarily childless people have equal interests in benefitting from assisted reproductive technologies like in vitro fertilization as a mode of treatment, we have normative reasons to ensure inclusive access to such interventions for as many of these people as is reasonable and possible. However, the prevailing eligibility criterion for access to assisted reproductive technologies-'infertility'-is inadequate to serve the goal of inclusive access. This is because the prevailing frameworks of infertility, which include medical and social infertility, fail to precisely capture an

Supporters of opt-in organ procurement policies typically claim that the absence of consent to postmortem transplantable organ retrieval is a normative barrier to such retrieval. On this ground, justification of opt-out policies is demanded. The paper shows that postmortem organ retrieval is normatively different from live organ removal, and so the doctrine of informed consent does not apply to it in the way it does in other types of cases. First, seen as the instrument of protection of autonomy or the right to self-determination, informed consent cannot be relied on in the case of dead person

In this paper, I claim that live uterus donors ought to be considered for the possibility of compensation. I support my claim on the basis of comparable arguments which have already been applied to gamete donation, surrogacy, and other kinds of organ donation. However, I acknowledge that there are specificities associated with uterus donation, which make the issue of incentive and reward a harder ethical case relative to gamete donation, surrogacy, and other kinds of organ donation. Ultimately, I contend that while reimbursement for the costs incurred by live uterus donors should be treated as

Uterus transplantation is a relatively new intervention. A woman with absolute uterine factor infertility receives, by a surgical procedure, a transplanted uterus, most often by living donation. The uterus recipient may thus become pregnant and conceive her own child. As with any other medical treatment, UTx requires legitimation. The anticipated benefits must outweigh the risks of the medical intervention. The risks and benefits of UTx are by no means unequivocal and cannot be easily determined. The benefits depend on the final evaluation of the suffering to be alleviated by the intervention.
