Chromosomal microarray (CMA) analysis in infants with congenital anomalies: when is it really helpful?

BACKGROUND: Birth defects are very common, affecting two to three infants in every 100 births, and often represent a diagnostic and management challenge. The birth of a child with multiple malformations is the beginning of a complex diagnostic process, where the primary purpose is to determine a precise nosological definition. An accurate diagnosis is a key prerequisite in providing a care plan, a prognosis and genetic counselling. The poor specificity of birth defects, the aetiology and course of which can vary despite similar phenotypic patterns, often makes the diagnostic path problematic.

Antenatal management and outcomes of gastroschisis in the U.K.

BACKGROUND: The birth prevalence of gastroschisis is increasing worldwide, yet little evidence exists concerning the optimal monitoring strategies after diagnosis. The aim of this study was to describe the U.K. prevalence, antenatal management and outcomes of affected pregnancies. METHODS: Cases were identified throughout the U.K. between October 2006 and September 2007, using three different sources. RESULTS: The overall birth prevalence of gastroschisis was 4.2 cases per 10, 000 total births (95% CI 3.6-4.8). Infants were variably monitored with growth scans (90%), umbilical artery Doppler

Limited Dorsal Myeloschisis with and without Type I Split Cord Malformation: Report of 3 Cases and Surgical Nuances.

Limited dorsal myeloschisis (LDM) is a rare form of spina bifida which is characterized by a fibroneural stalk between the inner part of the skin and the spinal cord. It may be associated with split cord malformation (SCM). Diagnosis and management of this complex malformation is challenging. We presented 3 different cases of LDM. Two of them were associated with Type I SCM and the other had no associated malformation. All of them were evaluated radiologically just after the birth and underwent surgical treatment under intraoperative neuromonitoring. They discharged without any complication. N

Exploring Clinical and Biological Features of Premature Births among Pregnant Women with SARS-CoV-2 Infection during the Pregnancy Period.

Studies observed that women infected with SARS-CoV-2 during pregnancy had a higher risk of preterm birth. Although it is likely that COVID-19 during the late trimester of pregnancy can trigger premature birth, prematurity remains a concern, and it is vital to study additional clinical and biological patient factors that are highly associated with this negative pregnancy outcome and allow for better management based on the existing predictors. In order to achieve this goal, the current study retrospectively recruited 428 pregnant patients that were separated into three study groups using a 1:2:

Recent trend of the decrease in mean birthweights of infants in the western provinces of Gifu Prefecture, Japan.

The mean birthweight has decreased recently in Japan. The author assessed the etiology of this trend by comparing the birthweight during the … birthweight as a decrease of the mean birthweight of first infants and changes of the proportion of birth order. Influences of the increases in the proportion of multiple births and that of low birthweight infants to the decrease of mean birthweight were small. The mean birthweight of infants who were born in public hospitals was lower than that of infants bo …

[Management of podalic delivery].

A study has been made of obstetric assistance in 1 796 podalic version labors between 1966 and 1982, with 12.4% of cesarean sections, an extremely conservative criterion. The corrected intra-and postnatal mortalities were 141.1% and 26.6% for premature and mature deliveries respectively. 744 children were examined between 4 and 10 years after birth, using clinical studies, intelligence quotient measurements by the method of Wppsi or WISC (according to age), EEG observations if necessary and stimulatory, motor and behavioral neurological tests. The results are compared with those of a similar s

A Systematic Evaluation of Hospital Performance of Childbirth Delivery Modes and Associated Factors in the Friuli Venezia Giulia Region (North-Eastern Italy), 2005-2015.

Cesarean sections (CS) have become increasingly common in both developed and developing countries, raising legitimate concerns regarding their appropriateness. Since improvement of obstetric care at the hospital level needs quantitative evidence, using routinely collected health data we contrasted the performance of the 11 maternity centres (coded with an alphabetic letter A to L) of an Italian region, Friuli Venezia Giulia (FVG), during 2005-15, after removing the effect of several factors associated with different delivery modes (DM): spontaneous vaginal delivery (SVD), instrumental vaginal

Effects of different periodontal interventions on the risk of adverse pregnancy outcomes in pregnant women: a systematic review and network meta-analysis of randomized controlled trials.

BACKGROUND: Periodontal disease is widespread among pregnant women, and it is possible that taking action to improve oral health conditions can make improvements in adverse pregnancy outcomes. Herein, we summarize the recent evidence using a network meta-analysis to assess the effects of different periodontal treatment intervention strategies on the risk of adverse pregnancy outcomes in pregnant women. MATERIALS AND METHODS: Randomized controlled trials were retrieved from PubMed, Web of Science, Embase, and Cochrane Library databases. After literature screening, data extraction, and quality

Outcomes of siblings with classical galactosemia.

OBJECTIVES: To determine the long-term outcome of dietary intervention in siblings from 14 Irish families with classical galactosemia (McKusick 230400), an autosomal recessive disorder of carbohydrate metabolism and galactose-1-phosphate uridyltransferase (GALT) deficiency. STUDY DESIGN: Outcomes in siblings on dietary galactose restriction were studied to evaluate whether birth order (ie, time of commencement of diet) and compliance with lactose-restricted diet (galactose intake > or < 20 mg /day), assessed by dietary recall and biochemical monitoring of galactose-1-phosphate [Gal-1-P] and g…

Impact of fathers on daughters' age at menarche: a genetically and environmentally controlled sibling study.

Girls growing up in homes without their biological fathers tend to go through puberty earlier than their peers. Whereas evolutionary theories of socialization propose that this relation is causal, it could arise from environmental or genetic confounds. To distinguish between these competing explanations, the authors used a genetically and environmentally controlled sibling comparison design to examine the effects of differential exposure to family disruption/father absence in a community sample of sister pairs. As specified by evolutionary causal theories, younger sisters had earlier menarche
