The irritant properties of 3 methylated derivatives of dibenz[b,f]1,4-oxazepine (CR) have been investigated, using the guinea-pig blepharospasm test and isolated cat eye preparations, and compared with the irritancy of the unsubstituted CR molecule. In both tests, the 11-methyl derivative was non-irritant whereas 3 Me-CR and 9 Me-CR retained some irritancy although both were less potent than CR itself. The possible significance of these results are discussed in relation to earlier biochemical studies.

The purpose of this study was to compare the scores of two clinical rating scales and high-speed video system measurements obtained during spontaneous eyelid movements in hemifacial spasm (HFS) patients before and after treatment. Patients were evaluated before and 30 days after receiving treatment with onabotulinumtoxinA injections. The inner canthal orbicularis drives spontaneous blinking of the eyelids and is richly innervated by a branching network of twigs based largely on the buccal branch of the facial nerve. This segment of the orbicularis is relatively resistant to denervation from subciliary incisional approaches. Combined with proper eyelid tension, contraction of this segment of orbicularis results in the normal physiologic blink mechanism. The authors discuss how to achieve proper eyelid tension and position intraoperatively during the treatment of patients with a range of orbital morphologies.

Botulin is established as first-line treatment in cervical dystonia, blepharospasm, hemifacial spasm and stroke-related spasticity of the distal parts of the upper limb. In addition, many other motor disorders and muscular hyperactivity states and neurologic symptoms and manifestations, in which temporary muscle paralysis, analgesic properties and effects on the functioning of the autonomic nervous system brought about by botulin can be utilized, have become indications for use. With proper dosage, botulin has proven to be safe even in prolonged use, the effect usually lasting for years.

Blinking is a normal human phenomenon involving trigeminal and facial pathways. To gain understanding on the neurobiology of blinking, five normal subjects were investigated before and after application of transdermal capsaicin at the forehead for two weeks. No effects of topical capsaicin were detected in eye blink rates. The authors use injections of botulinum A toxin in treatment of facial spasms. 105 patients (70 patients with blepharospasm, 35 patients with hemispasm) were treated during 4 years. The toxin Wisconsin relieved spasms for an average of 10 weeks. Local complications such as ptosis and diplopia appeared in less than 1/5 of the cases. No general complications were observed. Repeated treatments are necessary. The authors noted a decrease of lacrimal secretion on the treated side in hemifacial spasm and concluded that botulinum A toxin injections could be used as a treatment of epiphora.

Ptosis occurs in a variety of disorders including myasthenia gravis, oculomotor palsy, Horner's syndrome and brain stem disorders. There are also supranuclear lesions causing blepharoptosis. Meige's disease is a distressing complaint, the treatment of which often poses a challenge to the neurologist. The patient described here had blepharospasm-oromandibular dystonia, which responded transiently to oral lisuride. On three occasions, drug holidays successfully restored efficacy but thereafter further trials proved fruitless. Continuous subcutaneous lisuride administration in 0.35 mg doses per day, by means of a portable infusion pump, led to sustained improvement for 7 months. No major side effects were observed. Our findings suggest that this treatment deserves further trials.

Selection of muscles for botulinum toxin injection for limb dystonia is particularly challenging. Limb dystonias vary more widely in the pattern of dystonic movement and involved muscles than cervical dystonia or blepharospasm.
