Lengthening of below-the-knee amputation stumps using the Ilizarov technique.

Patients with short, traumatic, below-the-knee amputations (BKAs) frequently function as if they have knee disarticulations. The goal of this study was to evaluate the results in patients who had lengthening of their BKA stumps to improved prosthesis fit and increase ambulation. Three patients with traumatic BKAs, who were left with insufficient stumps for proper prosthesis wear, have had their stumps lengthened by Ilizarov's technique of distraction osteogenesis. The lengthening process produced a moderate degree of pain, and all patients had a temporary decrease in their range of motion. Two

[Lengthening technique of the ulna in Kienboeck's disease. Results after ten years].

The writers had re-examined several patients who had an operation for Kienböck disease before 1970. All had had an elongation of ulna through the same technique and no other operation for pseudarthrosis had been necessary. Eight patients were able to be examined 14.7 years after their operation (on an average). The analytical functional results were evaluated following Michon's criteria. The best results had been obtained as far as pain and strenght of the wrist were concerned, yet those obtained on mobility proved disappointing. X-Rays showed a lunatum keeping its shape 6 times out of 8 and a

Intrailiac Osteotomy With Superior Lateral Outcropping Bone: A Previously Undescribed Procedure for Hip Subluxation in Cerebral Palsy.

BACKGROUND: Surgical treatment of hip subluxation in cerebral palsy typically involves proximal femoral osteotomy with or without concurrent supra-acetabular pelvic osteotomy. The literature lacks data on isolated pelvic osteotomy for this condition. We present superior lateral outcropping bone as a novel procedure for augmenting pelvic osteotomies for additional femoral coverage. METHODS: In this retrospective case series, all patients were included for whom a single surgeon at a single institution performed pelvic osteotomy with adjunctive superior lateral outcropping bone for the treatment

An algorithm for surgical treatment of children with bone sarcomas of the extremities.

INTRODUCTION: Limb salvage surgery in children following bone sarcoma resection is a challenging problem because of the small size of the bones, the lack of appropriate size-matched implants, and the risk of limb-length discrepancy once skeletal growth is complete, secondary to the loss of the epiphyseal plate. Although several reconstruction options are available in children with bone sarcomas, such as vascularized fibula, massive bone allograft, extracorporeal devitalized autograft, endoprosthesis, and allograft-prosthesis composite, a consensus has not been reached on the best reconstructio

Septal Cartilage/Ethmoid Bone Composite Graft: A New and Improved Method for the Correction Underdeveloped Nasal Septum in Patients with Short Noses.

BACKGROUND: Septal extension grafts are an effective means of extending nasal length in patients with a short nose. However, such grafts can be challenging in patients who only have small quantities of weak septal cartilage, such as some East Asian patients. We developed a rhinoplasty technique using ethmoid bone to create a cartilage-bone complex to overcome this issue, allowing adequate nasal lengthening. METHODS: Sixty-four women with short noses and inadequate septal cartilage determined by preoperative computed tomography underwent septal extension grafting with a cartilage-bone complex

Management of chronic radial head dislocation associated with segment bone defect in ulna after osteomyelitis.

BACKGROUND: We report our treatment for chronic radial head dislocation associated with segment bone defect and shortening in ulna after osteomyelitis, using bone transport with a unilateral external fixator. METHODS: Between October 1994 and January 2009, 10 patients with chronic radial head dislocation associated with bone defect and shortening in ulna caused by osteomyelitis underwent bone transport using a unilateral external fixator in our unit. There were six males and four females, with a mean age of 14.2 years (range, 10-17 years). The mean ulna defect measured as the discrepancy betw

[Resection of navicular bone for severe midfoot deformity in children].

PURPOSE OF THE STUDY: The navicular bone lies at the apex of the deformity in severe talipes planovalgus with forefoot abductus, in the residual cavus of congenital talipes equinovarus, in certain cases of congenital convexity, and in certain types of neurological equinovarus. Resection of the navicular bone can be proposed to correct certain deformities. MATERIAL AND METHODS: This series included 15 feet operated on in 13 children from 1980 to 2003. The deformity to be corrected was a residual cavus after surgery for congenital talipes equinovarus (five feet), plantar and medial dislocation

Conservative Surgical Options for the Treatment of Forefoot Diabetic Ulcers and Osteomyelitis: An Evidence-Based Review and a Decision-Making Tool.

Diabetic foot ulcers cause higher rates of amputation and mortality; amputations yield even higher mortality rates than diabetic foot ulcers. Conservative excisional surgical procedures could be a suitable alternative to amputations, with comparable results with respect to wound-healing and infection control. Also, toe-sparing conservative surgical procedures offer good to excellent outcomes when compared with the nonoperative standard of care. Fourteen types of conservative surgical procedures were identified including bone excision with ulcerectomy, interphalangeal or metatarsophalangeal joi

Surgical Technique of Corrective Osteotomy for Malunited Distal Radius Fracture Using the Computer-Simulated Patient Matched Instrument.

The conventional corrective osteotomy for malunited distal radius fracture that employs dorsal approach and insertion of a trapezoidal bone graft does not always lead to precise correction or result in a satisfactory surgical outcome. Corrective osteotomy using a volar locking plate has recently become an alternative technique. In addition, the use of patient-matched instrument (PMI) via computed tomography simulation has been developed and is expected to simplify surgical procedures and improve surgical precision. The use of PMI makes it possible to accurately position screw holes prior to th

Simultaneous distraction osteogenesis and microsurgical reconstruction for facial asymmetry.

Restoring facial balance in patients with severe facial asymmetry is a challenging problem for the craniofacial team. Attention to bony reconstruction as well as soft-tissue contouring is required for patients with moderate to severe deformities. Traditionally, facial skeletal reconstruction was performed with osteotomies and bone grafting. More recently, distraction osteogenesis has proven to be successful in achieving bone lengthening. For select cases, distraction osteogenesis has lessened the need for major skeletal procedures and has allowed earlier surgical intervention. The reconstructi…

Factors influencing haematological recovery in 53 patients with acute myeloid leukaemia in first remission after autologous bone marrow transplantation.

The kinetics of haematological recovery were retrospectively analysed in 53 patients with acute myeloid leukaemia in first remission after myeloablative chemoradiotherapy followed by autologous bone marrow transplantation. The median time to achieve a neutrophil count of 1 x 10(9)/l was 46 d (22-196 d) and median time to achieve unsupported platelet counts of 20 x 10(9)/l and 50 x 10(9)/l was 70 d (24-310 d) and 126 d (29-497 d) respectively. Multivariate analysis revealed two factors that were significantly associated with delayed neutrophil and platelet recovery: (1) use of high dose fractio

Implant site development: clinical realities of today and the prospects of tissue engineering.

The success of dental implants depends on their placement in bone of adequate density and volume in order to achieve primary stability. Optimal esthetics of implants requires their placement in a position approximating that of the natural teeth they replace. However, there is generally at least some degree of atrophy in the sites where implants are to be placed. This atrophy may occur either before or after tooth extraction. Following extraction of teeth, there is commonly alveolar ridge resorption in horizontal and vertical dimensions. Alternatively, some of the oral hard and soft tissues may

Fixed pelvic obliquity after poliomyelitis: classification and management.

We classified fixed pelvic obliquity in patients after poliomyelitis into two major types according to the level of the pelvis relative to the short leg. Each type was then divided into four subtypes according to the direction and severity of the scoliosis. In 46 patients with type-I deformity the pelvis was lower and in nine with type II it was higher on the short-leg side. Subtype-A deformity was a straight spine with a compensatory angulation at the lower lumbar level, mainly at L4-L5, subtype B was a mild scoliosis with the convexity to the short-leg side, subtype C was a mild scoliosis wi

French experience in the management of fibular hemimelia: Radiographic and functional outcomes.

INTRODUCTION: Fibular hemimelia is a rare congenital condition characterized by hypoplasia of a bone segment associated with malformations. Its clinical presentation is highly variable, not only in terms of the types of malformations but also their severity. If untreated, it can lead to significant functional impairments. Managing patients with this condition often spans several decades. There is no standardized treatment protocol, and to our knowledge, no recent French study has analyzed the surgical strategies implemented and the long-term functional outcomes in these patients. The last Fren

Femoro-pedal distraction in staged reconstructive treatment of tibial aplasia.

AIMS: The treatment of tibial aplasia is controversial. Amputation represents the gold standard with good functional results, but is frequently refused by the families. In these patients, treatment with reconstructive limb salvage can be considered. Due to the complexity of the deformity, this remains challenging and should be staged. The present study evaluated the role of femoro-pedal distraction using a circular external fixator in reconstructive treatment of tibial aplasia. The purpose of femoro-pedal distraction is to realign the limb and achieve soft tissue lengthening to allow subsequen
