[Evaluation of life quality in patients after surgical treatment due to neoplasmatic matastases in bones].

The aim of this study was to evaluate life quality in patients with disseminated neoplasmatic process after surgical treatment of pathological fracture due to metastasis to long bones. The examination group consisted of 33 patients (25 women, 8 men) treated surgically (av. age 67 years). Pathological fracture was revealed in femoral bone in 28 patients (bilateral in one patient) and in humeral bone in 5 patients. In case of femoral bone fracture unipolar arthroplastic of hip joint was used or stable osteosynthesis with the use of intramedullar nail or AO plate; while in case of humeral bone fr

[Surgical Treatment of Calcaneus Fractures with C-Nail].

PURPOSE OF THE STUDY The calcaneus bone is the largest tarsal bone of a complex shape, the restoration of which after fracturing, often caused by a high-energy injury, is critical. The top priority in treating these fractures is to correctly asses the condition of the surrounding soft tissues that may be further excessively traumatized by inappropriate timing, surgical approach or technique. Even when adhering to all the rules and guidelines, complications in surgical wound healing have been described in up to 16-33% cases when the extended lateral approach was used. Therefore, the development

Skin sepsis in meat handlers: observations on the causes of injury with special reference to bone.

Outbreaks of wound infection with Streptococcus pyogenes and Staphylococcus aureus occurred in a abattoir and a pork-processing factory in Autumn 1980. Investigations showed that staff handling the meat before de-boning were particularly affected and that bone was the cause of 48% of the wounds which became clinicially infected. Of the total isolates of Strep. pyogenes and Staph. aureus from wounds of known cause 75% were from lesions caused by bone. In one outbreak streptococcal infection was seen principally in newly-employed staff. Group L streptococci were found in several sites in the por

A modified cement spacer technique for infected total hip arthroplasties with significant bone loss.

Complications related to femoral spacers are common during addressing infected total hip arthroplasties by 2-stage revision. We evaluated 11 patients who had 2-stage revisions with massive bone loss after removal of the infected components by a trochanteric osteotomy. All femoral cement spacers were assembled on intramedullary nails. Femurs were protected by a plate, whereas acetabuli were augmented by a cage or roof ring depending on the remaining bone stock. This additional hardware was covered with antibiotic-impregnated cement. Infection was eradicated in 10 (90.9%) of 11 patients within 3

Treatment of distal tibia periarticular bone loss using the Masquelet technique with RIA bone graft.

We report on the management of a patient who following a road traffic accident sustained a grade IIIB open fracture of the distal tibia with acute bone loss of the talus and distal tibia. The patient was managed with acute bone resection, and the two-stage Masquelet technique. He underwent stabilization with a hind-foot nail, application of RIA auto-graft and arthrodesis of the affected joint with a satisfactory functional outcome.

Reconstruction femoral nailing for nonunion of subtrochanteric fracture: a revision technique following dynamic condylar screw failure.

We report two cases of nonunion of subtrochanteric fractures associated with failure of fixation with a dynamic condylar screw (DCS) due to plate fracture. In both cases the nonunion was successfully treated by insertion of a reconstruction femoral nail. This implant allowed early weightbearing despite the patients' advanced age and poor bone quality. We did not perform direct bone grafting and the fractures healed without complication.

Intramedullary nailing for pathological femoral fractures.

PURPOSE: To evaluate whether the operating time correlates with the survival and outcome in patients who underwent intramedullary nailing for metastatic femoral fractures. METHODS: Records of 10 men and 11 women aged 43 to 86 (mean, 66) years who underwent intramedullary nailing (4 bilaterally) for metastatic femoral fractures between 1999 and 2003 were reviewed. The patients were followed up for at least 2 years or until their death. The main outcome measure was the correlation between operating time and survival. Operating time was categorised into 5 groups from 60 to 210 minutes, with 30-m

[Adult femoral shaft fracture treated with an intramedullary nail].

Femoral shaft fractures often happen, and usually indicate only one part of the traumatic injuries sustained by the body. Preoperative evaluation and management may be life-saving. The favored treatment of fracture itself is now intramedullary nailing. There were 144 adult cases treated with intramedullary nails and followed up for at least two years at Chang Gung Memorial Hospital from July 1981 to June 1984. The results revealed a high union rate 96.5% (139/144): closed method 100% (34/34), open method 95% (105/110). Significant complications requiring repeated annoying surgical reinterventi

Incomplete Diaphyseal Atypical Femoral Fracture due to Increased Anterolateral Bowing: Treatment with Corrective Osteotomy and Intramedullary Nailing with Augmented Plate Fixation.

BACKGROUND: Increased anterolateral bowing of the femur is associated with atypical femoral fractures among older patients (including both those receiving and those not receiving treatment with bisphosphonates) as well as a greater likelihood of occurring in the diaphyseal versus the subtrochanteric region. Prophylactic intramedullary (IM) nailing is recommended for the treatment of incomplete atypical femoral fractures, which are radiographically evident. However, this bowing deformity is considered to be a major obstacle to IM nailing as it may be associated with postoperative complications.

Snowblower injuries to the hand.

A retrospective review of 22 patients who sustained snowblower injuries to the hand was performed. There were 17 men and 5 women, ranging in age from 20 to 68 years (average age, 39.7 years). Fifty percent were manual laborers, 25% were unemployed, 15% were office workers, and 10% were not categorized. The dominant hand was involved in 86% of patients. In all patients, injuries occurred during an attempt to unclog manually the snowblower of wet snow. Patients were evaluated initially in the emergency room, where their wounds were irrigated and debrided, subungual hematomas drained, and nail be

Pin penetration depths in the neurocranium using a three-pin head fixation device.

In estimated 10-15% of neurosurgical interventions employing a conventional three-pin head fixation device (HFD) the patient's head loses position due to slippage. At present no scientifically based stability criterion exists to potentially prevent the intraoperative loss of head position or skull fractures. Here, data on the skull penetration depth both on the single and two-pin side of a three-pin HFD are presented, providing scientific evidence for a stability criterion for the invasive three-pin head fixation. Eight fresh, chemically untreated human cadaveric heads were sequentially pinned

Fibular Nail Fixation: Topical Review.

BACKGROUND: Intramedullary implants have been used historically in long bone fractures with success. In recent years, a variety of intramedullary implants for the treatment of fibular fractures have been investigated. These various implants have not been assessed together in a cohesive manner. In this review, we assess implants used for intramedullary fixation of fibular fractures with respect to implant design and clinical results. METHODS: A comprehensive systematic literature review for intramedullary implants in fibular fractures was performed. All publications that assessed intramedullar

Knee arthrofixation with modular nail as salvage for periprosthetic infection: Evaluation of quality of life and implant survival.

INTRODUCTION: Periprosthetic joint infection (PJI) is a complication that can lead to severe bone defects, a lack of soft tissue coverage, and poor infection control. Knee arthrofixation, using modular intramedullary nails and a cement block as a spacer, is a salvage procedure for these situations, serving as an alternative to limb amputation. The primary objective of this study was to evaluate the postoperative quality of life of patients undergoing knee arthrofixation with modular intramedullary nails. Secondary objectives included assessing implant survival, complication rates, and the rate

Fixators dynamization for delayed union and non-union of femur and tibial fractures: a review of techniques, timing and influence factors.

The optimal balance between mechanical environment and biological factors is crucial for successful bone healing, as they synergistically affect bone development. Any imbalance between these factors can lead to impaired bone healing, resulting in delayed union or non-union. To address this bone healing disorder, clinicians have adopted a technique known as "dynamization" which involves modifying the stiffness properties of the fixator. This technique facilitates the establishment of a favorable mechanical and biological environment by changing a rigid fixator to a more flexible one that promot

Pediatric femoral shaft fracture requiring revision surgery for nonunion associated with vitamin D and K deficiencies: a case report.

BACKGROUND: Nonunion of femoral shaft fractures in children is rare, and there is no clear treatment protocol. In this case report, a pediatric femoral shaft fracture that developed in nonunion due to vitamin deficiency after osteosynthesis, which was successfully treated with vitamin augmentation and replacement with a rigid antegrade intramedullary nail, is described. CASE PRESENTATION: The patient is an 11-year-old Japanese girl. She injured her right femoral shaft fracture when she hit a wall after kickboarding down a hill and underwent osteosynthesis with a titanium elastic nail. Six mon

Reamed Exchange Nailing in Nonunion of Tibial Shaft Fractures: A Review of the Current Evidence.

One of the most dreaded complications of fracture management is a nonunion. Nonunions are usually difficult to manage and can be a source of significant mental, physical, and financial distress to the patient. The incidence of nonunion is dependent on multiple factors including degree of comminution, open versus closed, concomitant infection, and vascular status, and therefore the management of such nonunions continues to be an often debated topic. Currently, there is no clear consensus on the role of reamed exchange nailing for tibial shaft nonunions. While reamed exchange nailing for aseptic
