Surgical correction of metacarpophalangeal valgus deformity in an alpaca.

A 1-month-old alpaca cria presented with a 13 degree valgus deformity of the left metacarpophalangeal joint. The angular limb deformity was centered on the distal metacarpal physes. Transphyseal bridging of the physes was recommended. Two 2.7 mm cortical bone screws were placed either side of the distal metacarpal physes and a figure of eight wire was placed medially around the screw heads. The screws extended through the medial metacarpus into the axial cortex of the lateral metacarpus. Seven weeks after surgery the limb was straight and the screws and wire were removed. Transphyseal bridging…

Treatment of Metacarpal and Phalangeal Fractures: Intramedullary Screw Technique.

BACKGROUND: Metacarpal and phalangeal fractures are among the most common fractures that upper-extremity surgeons encounter, accounting for 30% of all hand fractures1,2. These particular fractures can be treated either operatively or nonoperatively, according to the amounts of displacement, malrotation, and shortening1. Operative treatment includes the use of Kirschner wire fixation, intramedullary screws, plate-and-screw constructs, or interfragmentary screws. Multiple studies have demonstrated superior biomechanical strength and early active range of motion with use of intramedullary screws

Biomechanical analysis of pedicle screw density and rod contouring in adolescent idiopathic scoliosis instrumentation.

BACKGROUND: Pedicle screw density and rod contouring are crucial in the posterior spinal fusion (PSF) in adolescent idiopathic scoliosis (AIS) patients. This study evaluates the three-dimensional (3D) biomechanical interaction between rod contouring and the number/pattern of implants anchoring these rods to determine a range of advisable combinations that improve clinical outcomes. METHODS: Patient-specific, deterministic computational models were developed based on ten Lenke 1 AIS surgical cases. First order assessment with various pedicle screw patterns and rod contouring angles was perform

The modified Bristow procedure for recurrent anterior dislocation of the shoulder. Results and complications.

Seventeen out of eighteen patients, who underwent the modified Bristow operation for anterior recurrent dislocation of the shoulder, were followed up after a mean observation time of 52 months. Fourteen patients were satisfied with the result. There were no redislocations. The principal limitation in range of motion was loss of some external rotation. Eight patients had non-union between the bone transplant and the glenoid rim, and four of these had complications related to the screw. Two had loosenings and two had fractures of the screw. Non-union without screw complications appeared not to i

Role of inferomedial supporting screws for secondary varus deformity in non-osteoporotic proximal humerus fracture: A biomechanical study.

INTRODUCTION: The purpose of this study was to evaluate the effect of a medial support screw through a proximal humerus fracture. For this purpose, we verified whether the biomechanics are different according to the position of the screw while using the same number of screws. In addition, we tried to verify whether the insertion of additional inferomedial screws would make a difference in stability. MATERIALS AND METHODS: Twenty-four proximal humerus bones were included in the study. A two-part fracture was created and fixed using a locking plate. Cyclic loading and load-to-failure test were

Finite element biomechanical evaluation of internal fixation methods for metacarpal shaft fractures under multiple factors.

Metacarpal shaft fractures often require surgical correction of displacement, angulation, or rotational deformities to restore function. Kirschner wires (K-wires), plates, and intramedullary screw fixation each have advantages and limitations. However, inconsistent biomechanical evidence in existing studies has precluded definitive conclusions regarding the optimal fixation technique. This studycompared K-wires, plates, and intramedullary screw for metacarpal shaft fractures using finite element analysis (FEA). In normal bone, intramedullary screw matched plate stability under low loads but wer

Effect of augmentation techniques on the failure of pedicle screws under cranio-caudal cyclic loading.

PURPOSE: Augmentation of pedicle screws is recommended in selected indications (for instance: osteoporosis). Generally, there are two techniques for pedicle screw augmentation: inserting the screw in the non cured cement and in situ-augmentation with cannulated fenestrated screws, which can be applied percutaneously. Most of the published studies used an axial pull out test for evaluation of the pedicle screw anchorage. However, the loading and the failure mode of pullout tests do not simulate the cranio-caudal in vivo loading and failure mechanism of pedicle screws. The purpose of the present

Preoperative CT planning of screw length in arthroscopic Latarjet.

PURPOSE: The Latarjet procedure has shown its efficiency for the treatment of anterior shoulder dislocation. The success of this technique depends on the correct positioning and fusion of the bone block. The length of the screws that fix the bone block can be a problem. They can increase the risk of non-union if too short or be the cause of nerve lesion or soft tissue discomfort if too long. Suprascapular nerve injuries have been reported during shoulder stabilisation surgery up to 6 % of the case. Bone block non-union depending on the series is found around 20 % of the cases. The purpose of t

Biomechanical analysis of expansion screws and cortical screws used for ventral plate fixation on the cervical spine.

Compared to bicortical screws, the surgical risk of injuring intraspinal structures can be minimized with the use of monocortical screws. However, this reduction should not be achieved at the expense of the stability of the fixation. With monocortical stabilization, the expansion screws have the potential of absorbing high loads. Therefore, they are expected to be a suitable alternative to bicortical screws for revision surgeries and in osteoporotic bone. The purpose of this in vitro study was to investigate the stiffness of the two screw-plate systems used for ventral stabilization of the cer
