Persistent vegetative state: evaluation of brain metabolism and brain perfusion with PET and SPECT.

Patients in persistent vegetative state (PVS) after severe head trauma were investigated with 99mTc-ECD SPECT and 18F-FDG PET to further characterize the degree of brain damage and to obtain insight into changes of brain perfusion and glucose metabolism. 18F-FDG PET and 99mTc-ECD SPECT were performed in 16 patients in PVS. Quantitative PET data were compared with that obtained from seven normal controls. After spatial normalization into Talairach space, global grey matter values and regional data using predefined ROI sets were derived. For comparison of PET and SPECT, regional data were normal…

Response of amoeboid microglia/brain macrophages in fetal rat brain exposed to a teratogen.

This study examined the time course response of amoeboid microglia/brain macrophages in the rat fetus induced by a single intraperitoneal injection of cyclophosphamide, a teratogen, into the mother rat at 13 days of gestation. Compared to the normal fetal brain, a marked increase in amoeboid microglia was observed in the telencephalon and diencephalon of experimental fetuses, especially in those killed at embryonic day 15. Conglomerations of microglia occurred in the dorsal and superior neuroepithelium of diencephalon, basal telencephalon, cortical neuroepithelium, and hippocampal formation as…

Experimental study on brain oxygenation in relation to tissue water redistribution and brain oedema.

The aim of the present experimental research was to study brain oxygenation parameters in relation to tissue water movement and brain cortex oedema caused by focal brain ischemia. It has been demonstrated that local osmotic dehydration of the parietal brain cortex, mercury compounds (aquaporin inhibitors) and brain cortex oedema resulting from focal brain ischemia all influence extra-capillary oxygen transport lowering tissue respiration rates and oxygen transfer coefficients. The changes of brain oxygenation parameters in case of cortex ischemic oedema are reflected in the gas composition (ox…

Effects of ethanol on brain lactate in experimental traumatic brain injury with hemorrhagic shock.

OBJECTIVE: Previous studies of traumatic brain injury (TBI) and hemorrhagic shock (HS) models, have shown cardiorespiratory depression in ethanol-treated animals. This study investigated the effects of ethanol (ET) on brain lactate concentrations and acidosis in a TBI/HS model. METHODS: Anesthetized swine were instrumented and subjected to injury (INJ) consisting of fluid percussion TBI of 3 atm with concurrent 30 ml/kg graded hemorrhage over 30 min. Three groups were studied: Sham, INJ and INJ/ET. ET was given preinjury as a 2-g/kg i.v. bolus over 30 min, and an infusion of 0.4 g kg(-1) h(-1…

[Continuous monitoring of brain electrical activity as a guide to treatment of acute brain lesion].

UNLABELLED: Cortical activity of the brain can be monitored continuously by simple, cheap and non-invasive methods. Nevertheless routine monitoring in Neuro ICUs seldom includes traditional or processed EEG. Our clinical experience with the Cerebral Function Monitor (CFM) indicates that a reliable guide to treatment of acute cerebral lesion can be obtained in different clinical situations and steps of management. ISCHEMIA: the lower border voltage of the CFM correlates with the minimum level of cortical activity and mirrors different degrees of cortical metabolic depression. Cerebral oligaemi…

Brain imaging as a tool in establishing a theory of brain pathology in obsessive compulsive disorder.

Positron emission tomography (PET) studies using 18F-2-deoxy-2-fluoro-D-glucose (FDG) to determine glucose metabolic rates have been used recently to correlate the symptoms of obsessive compulsive disorder (OCD) with neuroanatomically localized brain dysfunctions. These studies, as well as data from other techniques and other disease states, suggest that the orbital cortex and the striatum are dysfunctional in OCD. This information can be used to construct a theory of how the symptoms of OCD and the related Gilles de la Tourette's syndrome and chronic motor tics are mediated by the central ner…

Effects of internal carotid administration of MPTP on rat brain and blood-brain barrier.

Unlike primates, rats are resistant to systemic 1-methyl-4-phenyl-1,2,3,6- tetrahydropyridine (MPTP) neurotoxicity, but direct infusion of MPTP into rat substantia nigra causes specific destruction of dopaminergic neurons. We now demonstrate that rats are resistant to MPTP neurotoxicity even when MPTP is injected directly into the brain circulation. Injection of 1-3.5 mg of MPTP into the internal carotid artery of Wistar rats causes no behavioral or motor abnormalities and small, but significant, dopamine loss in the ipsilateral striatum. MPTP caused no changes in the levels of norepinephrine …

[Analysis of angiographical findings in patients with severe brain disorders progressing to brain death].

In the present study, the authors analysed the serial angiographical findings progressing to brain death and their relation to the intracranial pressure (ICP) and the cerebral perfusion pressure (CPP). Seventy two patients, from four to eighty four years old (fourty six males and twenty six females) admitted in the Department of Emergency Medicine, University of Tokyo Hospital during the period from January, 1981 to April, 1986, were studied. Their underlying diseases were supratentorial primary brain lesions except two cases with asphyxias which progressed to brain death. ICP was continuously…

Utility of the Brain Gauge in Traumatic Brain Injury: Results of the Al Asad Missile Exposure Traumatic Brain Injury and Health Implications Study.

INTRODUCTION: Blast exposure is responsible for the great majority of Traumatic Brain Injury (TBI) in deployed military service members (SMs). More rapid and reliable methods of diagnosis of blast TBI are needed. The Brain Gauge has been touted as an option, but published reports have had mixed results. MATERIALS AND METHODS: A review of a case series of 15 U.S. SMs (2 females) with TBI from incoming missiles fired at Al Asad Air Force Base, Iraq, on January 8, 2020. All had medical evacuation to Landstuhl Regional Medical Center (LRMC), Germany where the Brain Gauge was administered; 10 of 1…

Extracellular vesicles derived from different brain tissue cells: A potential therapeutic measure for hypoxic-ischemic brain injury in immature brains.

Neonatal hypoxic-ischemic encephalopathy/neonatal hypoxic-ischemic brain damage and neonatal acute ischemic stroke are common causes of hypoxic-ischemic brain injury (HIBI) in the neonatal period, which may lead to permanent neurological sequelae. It is difficult to distinguish the two in the early stage. As a timely brain protection measure, hypothermia is still the standard treatment, but its efficacy in the treatment of immature brain injury is still controversial. The underlying pathophysiological mechanisms and effective treatment strategies of neonatal HIBI have been an active area of re…

Studies of isolated global brain ischaemia: I. Overview of irreversible brain injury and evolution of a new concept - redefining the time of brain death.

Despite advanced cardiac life support (ACLS), the mortality from sudden death after cardiac arrest is 85-95%, and becomes nearly 100% if isc … eurological dysfunction remains a devastating and unresolved problem. Our studies suggest that both brain and whole body damage reflect an ischaemic/reperfusion injury that follows the present reperfusion methods that use normal blood, which we term 'uncontrolled reperfusion'. In contrast, we have previously introduced the term 'controlled reperfusion', …

A Clinical Test for a Newly Developed Direct Brain Cooling System for the Injured Brain and Pattern of Cortical Brainwaves in Cooling, Noncooling, and Dead Brain.

To ensure the direct delivery of therapeutic hypothermia at a selected constant temperature to the injured brain, a newly innovated direct brain cooling system was constructed. The practicality, effectiveness, and safety of this system were clinically tested in our initial series of 14 patients with severe head injuries. The patients were randomized into two groups: direct brain cooling at 32°C and the control group. All of them received intracranial pressure (ICP), focal brain oxygenation, brain temperature, and direct cortical brainwave monitoring. The direct brain cooling group did better i…

Blood-brain barrier and blood-brain tumor barrier penetrating peptide-drug conjugate as targeted therapy for the treatment of lung cancer brain metastasis.

Lung cancer is the leading cause of cancer deaths worldwide. Brain metastasis of lung cancer, which counts for nearly 50% of late-stage lung cancer patients, is a sign of a really poor prognosis. However, the presence of blood-brain barrier (BBB) and blood-brain tumor barrier (BBTB) limits the penetration of drugs from the blood into the brain and thus restricts their accumulation in brain tumors. Systematic delivery of drugs into brain and brain tumor lesion using BBB and BBTB penetrating vehicles represents a promising strategy to overcome the BBB and BBTB limitations. Hence, we validated on…

Progressive brain compression. Changes in vital physiological variables, correlated with brain tissue water content and brain tissue displacement. Experimental MR imaging in dogs.

Continuous recording of vital physiological variables and sequential MR imaging were performed simultaneously during continuous expansion of an epidural rubber balloon over the left hemisphere in anaesthetised dogs. Balloon expansion led to a progressive and slightly nonlinear rise in intracranial CSF pressures and a fall in local perfusion pressures. Changes in systemic arterial pressure, pulse rate, and respiration rate usually appeared at a balloon volume of 4% to 5% of the intracranial volume (reaction volume), together with a marked transtentorial pressure gradient and MR imaging changes

Itaconate protects the blood-brain barrier by preserving microvascular bioenergetics after traumatic brain injury.

Traumatic brain injury (TBI) is a leading cause of mortality and long-term disability, yet effective pharmacological therapies remain unavailable. Early blood-brain barrier (BBB) disruption accelerates neuroinflammation and secondary brain injury. It has been well recognized that brain microvascular endothelial cells (BMECs) depend on mitochondrial oxidative metabolism to maintain BBB integrity. Here, we investigated the role of the mitochondria-derived immunometabolite itaconate in regulating cerebrovascular bioenergetics and BBB integrity after TBI. Using a controlled cortical impact model, …
