An adult patient with previously undiagnosed sinus venosus atrial septal defect presenting with brain abscess: a case report.

BACKGROUND: Brain abscess is a common complication in children with cyanotic congenital heart disease. The presence of an underlying acyanotic congenital heart disease is usually not suspected in an adult patient presenting with brain abscess. CASE SUMMARY: A 51-year-old male patient with no known co-morbidities came with complaints of recent onset right lower limb weakness needing support while walking and on evaluation was found to have brain abscess. He underwent robotic endoscope assisted endoport excision of the brain abscess. Two-dimensional transthoracic echocardiography showed right a…

Hereditary haemorrhagic telangiectasia and pulmonary arteriovenous malformations in brain abscess patients: a nationwide, population-based matched cohort study.

OBJECTIVES: The extent of hereditary haemorrhagic telangiectasia (HHT) and pulmonary arteriovenous malformations (PAVMs) as a risk factor for brain abscess is unknown. METHODS: Nationwide and population-based registries were used to identify persons with first-time hospitalization for brain abscess (index date) and population controls matched by age, sex and residence (1:10). Accounting for competing risks, cumulative incidence curves of new diagnosis of HHT/PAVM after brain abscess were constructed. Next, Cox regression was used for computation of cause-specific hazard rate ratios (HRRs) adj…

Brain abscess complicating ischemic embolic stroke in a patient with cardiac papillary fibroelastoma - Case report and literature review.

A 72-year-old man without obvious risk factors initially presented with acute ischemic stroke and fever, without concomitant infection. Broad spectrum antibiotic therapy was initiated. Transthoracic and Transesophageal echocardiography, and cardiac MRI revealed a 20 mm round mass attached to the anterior mitral valve leaflet, suggesting the diagnosis of a benign cardiac tumor or a vegetation. At the site of infarction an abscess of 11 mm in diameter developed 30 days later. The patient underwent surgical valve repair for the prevention of further embolic complications. Histology revealed a car

Brain abscess caused by Nocardia cyriacigeorgica in two patients with multiple myeloma: novel agents, new spectrum of infections.

OBJECTIVE AND IMPORTANCE: Introduction of high-dose chemotherapy and the novel agents including bortezomib, Lenalidomide, and Thalidomide has provided a significant progress in the treatment of multiple myeloma (MM) with an increase in median overall survival up to 6-8 years. However, the advances in myeloma treatment comes at a price with new spectrum of treatment-related infectious complications which should be taken into consideration while treating these patients. CLINICAL PRESENTATION: We report here two patients with Ig G λ MM presenting with intracerebral mass lesions in the abscence o

[Nocardia farcinica brain abscess: clinical and specific radiological findings and management. Report of two cases in immunononcompromised patients].

Nocardia spp. cerebral abscesses are rare and usually occur in immunocompromised patients. We report two recent cases of cerebral abscesses due to Nocardia farcinica in immunocompetent patients and review the literature about diagnosis and therapeutic issues. Outcome was good for the two patients following an early identification of the bacteria. Stereotactic biopsy was performed in one case and craniotomy with excision of the abscess in the other case. In both cases, complete identification of the bacteria could be achieved, followed by prolonged antibiotic therapy. Exposure to the germ (main

[Direct Gram staining of blood culture sample enabled the early diagnosis of brain abscess due to Listeria monocytogenes].

A 58 year old woman had a long history of immunocompromised state. Since age 28 she had multiple endocrine neoplasm type 2A: her thyroid gla … uires prolonged therapy with antibiotics, since it is the intracellular organism. Monitoring of the brain abscess wall by the enhanced MRI is useful to determine the completion of therapy. Since listerial contamination is common among raw meat and unpasteurized milk, immunocompromised patients should be alarmed not to eat uncooked food products.

Ring-enhanced mass in the brain of a woman with systemic lupus erythematosus and elevated serum CA19-9 level: brain abscess or metastatic tumor?--case report.

A 70-year-old woman with systemic lupus erythematosus presented with a brain abscess manifesting as progressive monoparesis of the right lower extremity over 4 days. She had had no episodes of fever, and did not complain of headache or exhibit any signs of meningeal irritability. Computed tomography of the brain showed a round, low-density mass with strong ring enhancement in the left frontal lobe. Laboratory examination found a moderately elevated serum level of CA19-9, a marker of some digestive organ cancers. Together with the absence of febrile episodes, headache, and a rise in leukocyte c…

Tuberculous brain abscess mimicking a neoplastic lesion in a patient with well-controlled human immunodeficiency virus infection: illustrative case.

BACKGROUND: Tuberculous brain abscess is a rare manifestation of CNS tuberculosis, typically occurring in patients with significant immunosuppression. These lesions may present as solitary ring-enhancing masses and can radiographically resemble neoplastic processes, creating diagnostic and management challenges. OBSERVATIONS: A 43-year-old man with well-controlled human immunodeficiency virus (HIV) infection and a remote history of treated pulmonary tuberculosis presented with progressively worsening unilateral headaches. MRI demonstrated a small, lobulated, peripherally enhancing posterior t…

Cognitive impairments during the acute phase of bacterial brain abscess and long-term neuropsychological profiles: a case series study.

INTRODUCTION: A bacterial brain abscess (BA) is a localized cerebral infection that requires prompt neurosurgical intervention, although the implementation of treatment may take several days. Approximately half of BA patients exhibit neurological signs, while less is known about cognitive abnormalities following BA from the acute to chronic phases. This case series study aimed to investigate cognitive impairments in patients with BA during the acute phase and evaluate post-acute selective cognitive deficits that align with BA location in neuropsychological profiles during the first year follow…

Brain Abscess Complicating Metastatic Scalp Basal Cell Carcinoma With a Malignant Fungating Wound: A Case Report and Literature Review.

Basal cell carcinoma (BCC), the most prevalent skin cancer, rarely metastasizes to the brain. Here, we report a case of a 63-year-old man with metastatic BCC complicated by a malignant fungating wound (MFW), a cancerous lesion seen in advanced diseases, and Staphylococcus aureus infection, resulting in brain abscesses and encephalopathy. The BCC initially involved the scalp and was treated with excision but later recurred and metastasized. It was associated with a chronic, open MFW lesion with necrosis and ulceration. MRIs revealed cranial structure involvement and brain abscesses. Despite tre…

Localized asymptomatic cerebellar abscess after penetrating brain injury by wooden foreign object with adequate antibiotics administration: A case report.

INTRODUCTION: Brain abscess is a compilation of pus enclosed in capsule as a result of focal infection in brain parenchyma. It is one of several complications found in patients who suffered penetrating brain injury. CASE PRESENTATION: Thirty-four-year-old man suffered a penetrating brain injury after a 50 cm piece of wood penetrated through his facial skull and ended its tip in his cerebellum, the wood priorly ejected from a moulding machine. As a consequence, he had to undergo a craniotomy procedure to remove the foreign body object and its debris. Following the surgery, adequate antibiotics…

Posttraumatic gas-containing brain abscess caused by Clostridium perfringens with unique simultaneous fungal suppuration by Myceliophthora thermophila: case report.

OBJECTIVE AND IMPORTANCE: Gas-containing brain abscesses are rare, and the vast majority are caused by Clostridium perfringens. Significant simultaneous fungal infection in a bacterial abscess is even rarer. We present such a case and review the literature. CLINICAL PRESENTATION: A 21-month-old male patient sustained a penetrating head injury in a barnyard, developed a gas-containing left parietal brain abscess, and presented with high fever, galeal swelling, and seizure. INTERVENTION: The patient initially underwent debridement of his wound and then repeated aspirations. The initial culture…

Epilepsy following brain abscess. The evaluation of possible risk factors with emphasis on new concept of epileptic focus formation.

The study is based on follow-up data of 108 patients treated for brain abscess (mean follow-up period 11 years). The overall incidence of epilepsy was 34% (37% in supratentorial lesions). The author compared the incidence of several parameters between two groups of patients: those presenting, and those not presenting with epilepsy after brain abscess treatment. Parameters characterized by significantly different incidence in each group were taken into account as possible risk factors influencing the development of epilepsy. Sex, age of the patient, localisation and size of the abscess are disc…

Precision Under Pressure: Scalp Block and Dexmedetomidine for Brain Abscess Drainage in a Patient With Complex Cyanotic Heart Disease.

Cyanotic heart disease is a known risk factor for brain abscess. Anesthesia for burr-hole drainage in these patients can be challenging due to complications like an increase in right-to-left shunt, arrhythmias, heart failure, and paradoxical air embolism. The risk of these complications can be decreased by choosing a scalp block with conscious sedation.

Metagenomic Next-Generation Sequencing for Brain Abscess: Improved Pathogen Detection, Targeted Antimicrobial Therapy, and Association with Fewer Surgical Interventions.

BACKGROUND: Brain abscesses demand prompt, accurate pathogen identification; however, identification using conventional culture is limited, especially for anaerobic and polymicrobial infections. We compared the diagnostic and clinical utility of metagenomic next-generation sequencing (mNGS) with that of conventional culture in patients with brain abscess. METHODS: We retrospectively included 115 patients with confirmed brain abscess pathogens. Seventy-two patients underwent both mNGS and conventional culture, and 43 underwent culture alone. We evaluated diagnostic performance, pathogen profil…

A glioblastoma mimic: successful multimodal treatment of a rare dual fungal brain abscess in an immunocompetent patient. Illustrative case.

BACKGROUND: Cerebral phaeohyphomycosis is a rare but often fatal CNS infection caused by dematiaceous fungi. Although most invasive fungal infections affect immunocompromised patients, this entity disproportionately affects immunocompetent individuals. No standardized treatment exists, and mortality frequently exceeds 70%, even with aggressive therapy. OBSERVATIONS: A previously healthy 64-year-old woman presented with progressive left-sided weakness and a parietal lesion with a presumed radiological diagnosis of high-grade glioma. She underwent three craniotomies due to rapid recurrence, and

Mapping Half a Century of Global Brain Abscess Research: A Bibliometric Analysis of Output, Themes, and Collaboration (1975-2025).

Brain abscess is an uncommon but life-threatening intracranial infection managed across neurosurgery, infectious diseases, microbiology and neuroradiology, for which no single unified international guideline exists; despite a large literature, the research field has been mapped only once, in a single-database study of 994 title-matched records. We performed a comprehensive bibliometric analysis of global brain-abscess research, retrieving journal articles (1975-2025) from the OpenAlex database using an exact-phrase title/abstract search and computing performance metrics, bibliometric laws and …
