Left bundle branch pacing compared to left ventricular septal myocardial pacing increases interventricular dyssynchrony but accelerates left ventricular lateral wall depolarization.

BACKGROUND: Nonselective His-bundle pacing (nsHBp), nonselective left bundle branch pacing (nsLBBp), and left ventricular septal myocardial pacing (LVSP) are recognized as physiological pacing techniques. OBJECTIVE: The purpose of this study was to compare differences in ventricular depolarization between these techniques using ultra-high-frequency electrocardiography (UHF-ECG). METHODS: In patients with bradycardia, nsHBp, nsLBBp (confirmed concomitant left bundle branch [LBB] and myocardial capture), and LVSP (pacing in left ventricular [LV] septal position without proven LBB capture) were

A Novel Pacing Strategy With Low and Stable Output: Pacing the Left Bundle Branch Immediately Beyond the Conduction Block.

This report demonstrates the feasibility of pacing the left bundle branch (LBB) immediately beyond the conduction block to functionally restore the impaired His-Purkinje conduction system in a patient with heart failure and left bundle branch block (LBBB). The pacing required only a low pacing output (0.5 volts/0.5 ms) to correct the LBBB with accompanying right BBB on the electrocardiogram. Over 1-year of follow-up, the patient had a significant improvement in clinical outcome and echocardiographic measurements. The case shows a novel pacing strategy for patients with BBB that affects many pa

Significance of an isolated new right bundle branch block in a patient with chest pain.

Chest pain is a common presenting symptom in emergency departments, and a typical manifestation of acute myocardial infarction (AMI). Recognition of ECG changes in AMI is essential for timely diagnosis and treatment. Right bundle branch block (RBBB) may be an isolated sign of AMI, and was previously considered as a criterion for fibrinolytic therapy. Since the most recent European Society of Cardiology and American Heart Association guidelines in 2013, RBBB alone is no longer considered a diagnostic criterion of AMI, even if it occurs in the context of acute chest pain, as RBBB does not usuall

The RR index test for the differentiation of atrioventricular nodal block from His-Purkinje block during incremental atrial pacing in patients with bifascicular block.

AIMS: His-Purkinje block induced by incremental atrial pacing is highly predictive of an impending high degree atrioventricular block in patients with bifascicular block. The His potential is, however, sometimes not measurable or is lost in the ventricular depolarization. The aim of this study was to evaluate whether the comparison of RR intervals before and after atrioventricular block, induced by incremental atrial pacing, could differentiate between atrioventricular nodal and His-Purkinje block in patients with bifascicular block. METHODS AND RESULTS: In 98 patients, with bifascicular bloc

Excess mortality and morbidity associated with right bundle branch and left anterior fascicular block.

Excess mortality and morbidity associated with right bundle branch and left anterior fascicular block were evaluated in 108 patients with block (age 74 +/- 10 years, 69% male) and 108 age- and sex-matched control patients with normal conduction. Clinical characteristics were similar initially except for more congestive heart failure in patients with block. Life table analysis revealed a higher 12 year mortality with block, even after omitting patients with moderate or severe congestive heart failure (risk ratio 1.47, p less than 0.05). Compared with control subjects, the group of patients with

[Electrophysiological study of 2 cases of ventricular pre-excitation compatible with the presence of Mahaim's fibers].

Report of two cases of ventricular pre-excitation in which the presence of Mahaim's fibres was suggested by the electro-physiological studies. In both cases, the short-circuit of the His bundle was responsible for the abnormally short HV interval. In a patient, atrial acceleration induced an increase of rh at first, related to an A-V delay without HV variation. For a cycle of 360 ms, HV lengthened suddenly from 15 to 40 ms while QRS changed configuration, which is in favour of a functional exclusion of an accessory pathway. Combination with the pre-excitation of a left bundle-branch block comp

Case report: What course to follow when left bundle branch pacing encounters acute myocardial infarction?

Compared with traditional right ventricular apical pacing, His-bundle pacing (HBP) provides more physiologic pacing by activating the normal conduction system. However, HBP has some limitations including higher pacing thresholds. In addition, disease in the distal His-Purkinje system may prevent the correction of abnormal conduction. Left bundle branch pacing (LBBP) may overcome these disadvantages by providing lower pacing thresholds and relatively narrow QRS duration that improve cardiac function. Here, we describe a rare case of a transient loss of ventricular capture due to acute anterior

Masquerading bundle branch block: An electrocardiographic harbinger of advanced conduction system disease.

BACKGROUND: Masquerading bundle branch block (MBBB) is a rare and underrecognized electrocardiographic pattern characterized by features of both right and left bundle branch block on the same tracing. Unlike typical bifascicular block, MBBB is increasingly recognized as a marker of advanced and often bilateral His-Purkinje system disease, with potential progression to high-grade atrioventricular block. CASE PRESENTATION: We report the case of an 84-year-old woman with a history of atrial fibrillation, heart failure with preserved ejection fraction, chronic kidney disease, and aortic stenosis

The evolution and future of conduction system pacing.

Cardiac pacing has undergone a fundamental paradigm shift from conventional right ventricular apical pacing towards physiological activation of the native conduction system of the heart. Decades of evidence have established that chronic right ventricular pacing induces ventricular dyssynchrony, contributing to pacing-induced cardiomyopathy and adverse clinical outcomes. Conduction system pacing, which engages the intrinsic His-Purkinje network of the heart, has emerged as a transformative approach that preserves or restores the natural ventricular activation sequence. His bundle pacing, the ea

De subitaneis mortibus. XX. Cardiac electrical instability in the presence of a left superior vena cava.

Abnormalities of cardiac rhythm have been reported by others in patients with a persistent left superior vena cava. We present the histological findings from postmortem examination of the cardiac conduction system of two such patients. The first patient was a schoolboy who died suddenly and unexpectedly. His sinus node was abnormally small, his A-V node contained numerous venous lacunae and was stretched out beneath the enlarged coronary sinus, both A-V node and His bundle were dispersed in the central fibrous body in a fetal pattern, and isolated fragments of A-V nodal tissue were connected t

Characterization of Septal Scar After Left Bundle Branch Area Pacemaker Implantation.

BACKGROUND: Left bundle branch area pacing (LBBAP) is increasingly used in patients with a ventricular pacing indication but necessitates precise septal localization. The resulting effects of intraoperative lead repositioning on septal fibrosis remain unknown. OBJECTIVE: To assess the safety of cardiac magnetic resonance imaging (cMRI) in patients with LBBAP pacemakers and evaluate the septal myocardium via cMRI following LBBAP implantation compared with standard RV endocardial pacing (RVP). METHODS: Patients aged > 18 y.o. with bradycardia, LBBAP or RVP implant from 2021 to 2023, and at lea

Clinical Features and Outcomes of Long-Term Lead Performance by Left Bundle Branch Pacing.

Left bundle branch pacing (LBBP) is an emerging physiological pacing technique that expanded in recent reports. The long-term safety and feasibility of LBBP lack adequate evaluation. The study aimed to evaluate the long-term lead performance and clinical outcomes of LBBP.We retrospectively enrolled 123 consecutive patients scheduled for LBBP implantation from January to December 2018. The pacing parameters, electrocardiograms, echocardiographic measurements, and complications associated with LBBP were tracked at implant and follow-up.LBBP was successfully performed in 110 of 123 (89.4%) patient

Electroanatomical mapping-guided left bundle branch area pacing in patients with structural heart disease and advanced conduction abnormalities.

AIMS: Left bundle branch area pacing (LBBAP) can be technically challenging and fluoroscopy-intense. Three-dimensional electroanatomical mapping (EAM) facilitates non-fluoroscopic lead navigation and electrogram mapping. We sought to prospectively evaluate the feasibility, safety, and outcomes of routine EAM-guided LBBAP in patients with structural heart disease (SHD) and advanced conduction abnormalities. METHODS AND RESULTS: Consecutive patients with SHD and conduction abnormalities who underwent an attempt at EAM-guided LBBAP were included. The feasibility, safety, procedural, and mid-term

Electrical Synchrony Optimization for Left Bundle Branch Area Pacing in Patients With Bradycardia and Heart Failure.

Left bundle branch area pacing (LBBAP) has emerged as a promising physiological pacing modality. This study was designed to investigate the acute impact of the atrioventricular delay (AVD) on cardiac electrical characteristics and identify an optimal range of AVDs for LBBAP to achieve electrical atrioventricular and interventricular synchrony. Patients indicated for ventricular or biventricular pacing were studied during routine follow-ups at least 3 months after LBBAP implantation. Patients were excluded if they had a complete AV block or persistent atrial fibrillation. AVD was programed from

Identification and ablation of three types of ventricular tachycardia involving the his-purkinje system in patients with heart disease.

INTRODUCTION: Ventricular tachycardia (VT) with involvement of the His-Purkinje system (HPS) can be difficult to recognize in patients with heart disease, but it may be particularly susceptible to ablation targeting the HPS. This study defines the incidence and types of HPS involvement in VT. METHODS AND RESULTS: Involvement of the HPS was sought during electrophysiologic study with catheter mapping in 234 consecutive patients referred for catheter ablation of recurrent VT associated with heart disease. HPS VT was observed in 20 (8.5%) patients (mean ejection fraction 29%+/- 17%); in 9 (11%)

Prospective evaluation of feasibility and electrophysiologic and echocardiographic characteristics of left bundle branch area pacing.

BACKGROUND: His bundle pacing (HBP) is the most physiologic form of pacing but associated with higher thresholds and lower success in patients with His-Purkinje conduction disease. Recent reports have described transvenous left bundle branch area pacing (LBBAP). OBJECTIVE: We aimed to prospectively evaluate the feasibility and the electrophysiologic and echocardiographic characteristics of LBBAP. METHODS: Patients requiring pacing for bradycardia or heart failure indications (failed left ventricular [LV] lead) were prospectively enrolled. LBBAP was performed with a Medtronic 3830 lead. Prese
