Intraaortic balloon pump insertion through the subclavian artery. Subclavian artery insertion of IABP.

The intraaortic balloon pump (IABP) is frequently used in the management of cardiac failure in the setting of myocardial infarction or as a bridge for coronary revascularisation surgery. The IABP is usually inserted through the femoral artery. Occasionally severe aorto-iliac occlusive disease prevents the retrograde passage of the balloon, in which case an anterograde route, usually through the ascending aorta is used. We describe four patients in whom an IABP was placed through the subclavian artery by the joint efforts of cardiologists and vascular surgeons.

Treatment and outcome of serious structural congenital heart disease.

Serious structural congenital heart disease usually presents to the neonatal paediatrician, although increasingly these conditions are being diagnosed before birth. It is, therefore, important that those dealing with these fetuses and infants have some knowledge of their natural and modified history. The vast majority of lesions can either be corrected or given symptomatic palliation and this review discusses treatment options and provides up-to-date outcome information to enable fetal and neonatal staff to anticipate and to complement information given to families by paediatric cardiologists.

Accelerated growth in children.

Many tall-statured children will display variations of normal growth patterns, such as familial tall stature or familial rapid maturation. However, precocious puberty and a number of rarer diseases that can cause accelerated growth require the earliest possible detection and treatment to ensure that underlying problems are appropriately treated and optimum final adult height is attained. Most of these syndromes are rare. Nevertheless, the pediatric nurse practitioner should be acquainted with them to make timely and appropriate referrals to endocrinologists, geneticists, cardiologists, or orth…

Fatal pneumococcal septicaemia associated with asplenia and isomerism of the right atrial appendages.

A 22 month old male infant with isomerism of the right atrial appendages (associated with a complete atrioventricular septal defect, double outlet right ventricle, and pulmonary valve stenosis) died from pneumococcal septicaemia after a 36 hour illness. He had not been given penicillin prophylaxis. Asplenia was confirmed at necropsy. In a survey of 50 paediatric cardiologists in the United Kingdom, 13 (33%) of the 40 (80%) who replied did not advise any prophylactic measures against pneumococcal infection in patients with isomerism of the right atrial appendages.

Non-valvular atrial fibrillation in patients on peritoneal dialysis, prevalence, treatment and professionals involved.

Atrial fibrillation is the most frequent chronic arrhythmia in patients with chronic kidney disease. Oral anticoagulation with vitamin K antagonists and now direct oral anticoagulants have been and are the fundamental pillars for the prevention of thromboembolic events. However, there are no randomized clinical trials on the risk-benefit profile of oral anticoagulation in patients with chronic kidney disease stage 5 on peritoneal dialysis and there is little evidence in the literature in this population. The objective of our study was to know the prevalence, treatment and professionals involve

Atrial fibrillation.

Atrial fibrillation (AF) is the most common sustained cardiac arrhythmia and a leading preventable cause of ischaemic stroke, with rising prevalence and substantial healthcare burden worldwide. While cardiologists traditionally lead AF management, neurologists play a pivotal role given their expertise in diagnosing stroke, managing its secondary prevention and assessing the risk-benefit of anticoagulation. In this narrative review, we summarise recent advances in AF management and their implications for neurology practice, underscoring the importance of proactive and collaborative management t…

[Screening for iron deficiency and pneumococcal vaccination: What role do they play in community cardiology?].

This article presents a single-center, cross-sectional, descriptive epidemiological field study evaluating the screening for iron deficiency and pneumococcal prevention by community cardiologists in patients with heart failure. In total, 90% of the practitioners surveyed reported being familiar with the European Society of Cardiology's recommendations regarding iron deficiency screening, but only 60% perform it at least once a year. In contrast, 90% do not assess the patient's pneumococcal vaccination status. Nearly all respondents stated they are willing to collaborate with an advanced practi…

Dual left anterior descending artery - a clinical entity in search of an identity: A novel classification.

A dual left anterior descending artery (LAD) is not an uncommon anomaly of the coronary arterial system. The earlier classifications by various authors lacked standardization and uniformity and are too complex for clinical usage. Therefore, a new classification in the form of coding system is proposed to facilitate the identification and reporting of the anomaly by the cardiologists to maintain uniformity in reporting, which aids in adequate planning of the revascularization strategies in patients with symptomatic coronary artery disease involving the dual LAD system.

Pericardial Drain Placement in Interventional Radiology: An Overview.

Pericardial effusions can occur from multiple different etiologies. Although often incidentally noted or clinically silent, pericardial effusions may cause significant hemodynamic compromise. In some of these patients, pericardiocentesis may be temporizing only and either repeat procedures or placement of pericardial drains may be preferable. While cardiologists typically perform pericardial drain placement, it is a procedure that may also be performed by interventional radiologists. This article describes for the interventional radiologist the indications and placement technique, as well as p…

Unusual presentation of arteria lusoria along with ascending aortic aneurysm.

Arteria lusoria (AL) is a rare embryological variant that may have variable presentation ranging from asymptomatic to respiratory, abdominal, or both. Increasing incidence of radial access for coronary angiogram may lead to the incidental diagnosis of AL. Knowledge about this anatomical variation is essential for cardiologists, interventional radiologists, otorhinolaryngologists, cardiothoracic surgeons, and primary physicians. The symptomatic patient should undergo definite repair through either surgery or endovascular techniques. We present a case of incidentally diagnosed, asymptomatic AL a…

Inlet patch mimicking unstable angina pectoris.

The ectopic stomach mucosa island in the proximal esophagus, which is generally known as the inlet patch or cervical inlet patch, is called as the heterotopic gastric mucosa of the esophagus. Despite its asymptomatic progress, it may cause chest pain, shortness of breath and difficulty in swallowing due to the acid secretion from the ectopic mucosa. The study aimed to present a patient who underwent coronary angiography with an unstable angina pectoris diagnosis by cardiologists for gastric chest pain but found an inlet patch in gastroduodenoscopy.

Change of Paradigm in the Management of Patients with Accessory Pathways over the Last Forty Years: Wolff-Parkinson-White Syndrome as an Electrophysiological Marvel at Risk of Extinction.

Over the last decades, the approach to the Wolff-Parkinson-White syndrome, as well as its treatment, has substantially changed, leading to improvement in the prognosis and quality of life of these patients. From the first diagnostic electrophysiologic studies to the most recent evaluations, important data on pathophysiologic and clinical aspects have been gathered, and this learning journey is still not concluded. This body of knowledge is a fundamental part of any cardiologists' armamentarium despite the fact that this syndrome is rarely observed in adult patients.

Sudden cardiac arrest due to atrioventricular block: A diagnostic challenge.

Sudden cardiac arrest survivors continue to be a challenge for cardiologists. An appropriate initial diagnostic approach is crucial for planning a successful therapeutic strategy. We report the case of a 62-year-old woman who suffered an out-of-hospital cardiac arrest due to third-degree atrioventricular (AV) block. Despite suspected acute coronary syndrome, the coronary angiogram proved inconclusive, while cardiac magnetic resonance imaging ruled out other differential diagnoses such as acute myocarditis. The reversible nature of the AV block rendered permanent pacing unnecessary.
