The neurosurgeon is often consulted in cases of shoulder-arm pain that have as their cause compression of the neurovascular bundle in the thoracic outlet. He or she should, therefore, be familiar with the clinical presentation and differential diagnosis of this syndrome, as well as the electrographic and angiographic findings that may be present in this condition. If the diagnosis is established and the patient proves refractory to conservative treatment with physical therapy, medication, exercises, and modification of activities, the neurosurgeon may elect to perform scalenotomy, paying parti

1. Studies on a large number of tissues obtained from fatal cases of human poliomyelitis have revealed that the virus is distributed predominantly in two systems: (a) certain regions of the nervous system, and (b) the alimentary tract. 2. Poliomyelitis virus was demonstrated in the walls of the pharynx, ileum, and only once in those of the descending colon, while the contents of the descending colon regularly contained the virus. 3. The presence of virus in the walls of the alimentary tract appears to be the result neither of generalized dissemination of the virus nor of secondary centrifugal

In forensic anthropology, identification begins by determining the sex, age, ancestry and stature of the individuals. Asymptomatic variations present on the skeleton, known as discrete traits, can be useful to identify individuals, or at least contribute to complete their biological profile. We decided to focus our work on the upper part of the skeleton, from the first vertebra to the pelvic girdle, and we chose to present 8 discrete traits (spina bifida occulta, butterfly vertebra, supraclavicular nerve foramen, coracoclavicular joint, os acromiale, suprascapular foramen, manubrium foramen an

The extrinsic innervation of the abdominal organs in neonatal and adult female rats is described. Mainly an in toto acetylcholinesterase method was used; moreover, formaldehyde-induced fluorescence and acetylcholinesterase were demonstrated in sections. The major splanchnic nerve has its origin in the ninth and tenth thoracic sympathetic trunc ganglia. In the major splanchnic nerve a suprarenal ganglion is present. Sometimes the minor splanchnic nerve, arising from the tenth thoracic ganglion, joins the distal part of the suprarenal ganglion. The left and right major splanchnic nerves join the

The surgical treatment of 30 cases of vascular thoracic outlet syndrome (TOS) in 25 patients is presented. Patients included 17 women and 8 men with average age of 26.1 years. The causes of compression were cervical rib ( n = 16), soft tissue anomalies ( n = 12), and scar tissue after clavicle fracture ( n = 2). Ten subclavian artery aneurysms containing intraluminal thrombus as well as one subclavian artery occlusion were found. All such cases had multiple distal arterial embolization. Presenting features of cases with arterial TOS included: hand ischemia ( n = 11), transient ischemic attack

INTRODUCTION: The respective effects of direct and indirect decompression in the clinical outcome after anterior cervical disc fusion (ACDF) is still debated. The main purpose of this study was to analyze the effects of indirect decompression on foraminal volumes during ACDF performed in patients suffering from cervico-brachial neuralgias due to degenerative foraminal stenosis, i.e. to determine whether implant height was associated with increased postoperative foraminal height and volume.
METHODS: A prospective follow-up of patients who underwent ACDF for cervicobrachial neuralgias due to de

BACKGROUND: In complete brachial plexus palsy, we have hypothesized that grafting to the musculocutaneous nerve should restore some hand sensation because the musculocutaneous nerve can drive hand sensation directly or via communication with the radial and median nerves.
OBJECTIVE: To investigate sensory recovery in the hand and forearm after C5 root grafting to the musculocutaneous nerve in patients with a total brachial plexus injury.
METHODS: Eleven patients who had recovered elbow flexion after musculocutaneous nerve grafting from a preserved C5 root and who had been followed for a minim

To clarify the relationships between electromyography (EMG) and magnetic resonance imaging (MRI), we compared findings in 37 selected patients who presented with cervical root avulsion injuries. Nerve root repair with C4-T1 hemilaminectomy was subsequently performed on 19 patients. The agreement between the two evaluative modalities with complete or incomplete lesions of ventral root and pre- or postganglionic lesions of dorsal root was measured for each root level. Both with ventral and dorsal root evaluation, C6, C7, and C8 yielded high agreement values, ranging from 86% to 94%. C5 manifeste

The sensation of pain derived from intracranial and extracranial blood vessels is conveyed to the central nervous system chiefly by the trigeminal nerve, with first order neurones terminating in the nucleus caudalis of the spinal trigeminal tract and an area lateral to the dorsal horn of the spinal cord at the second cervical segment. The evoked discharge of second order neurons can be suppressed by activation of the endogenous pain control system or, in about one third of cases, by the local application of ergot derivatives or the serotonin (5HT1) agonist sumatriptan (GR43175). Stimulation of

As discussed by Urschel [13], 50 to 70 percent of patients with symptoms of thoracic outlet syndrome may be treated conservatively by nonsurgical methods. Patients who still have symptoms after years of such therapy are referred to us for surgical treatment. Failure to obtain symptomatic relief after scalenotomy alone [8] caused us to abandon this treatment. The advantages of the subclavicular approach to first rib resection are as follows: (1) The operation is easy and fast. (2) There is good visibility in the operative field. (3) The neurovascular bundle to the arm lies cephalad and out of h

BACKGROUND: Contralateral C7 nerve transfer is widely applied for the treatment of brachial plexus injuries or central paralysis of the upper extremities. The surgical approach has evolved from the precervical subcutaneous route to the prespinal route, which is currently the most commonly used one. We report a patient with central paralysis of the right upper extremity treated with contralateral C7 nerve transfer via the posterior spinal route.
CASE DESCRIPTION: A 59-year-old female patient was admitted on 3 July, 2018 with right hemiplegia. The muscle strength of the right lower and upper ex

Ansa-to-recurrent laryngeal nerve (ANSA-RLN) reinnervation procedures are now often first-line treatments for some children with unilateral vocal fold immobility. Although many describe that children with prolonged denervation and true vocal fold atrophy should not undergo this procedure, there has been no gold-standard means of identifying true denervation. Here, we describe a novel technique using evoked vagal electromyography to predict degree of chronic nerve injury prior to recurrent laryngeal nerve reinnervation in children. This is a simple, readily available technique that may play an

Ventral root avulsion induces dramatic loss of the affected spinal cord motoneurons. The neuroprotective effect of riluzole has been previously proven on the injured motoneurons: the vast majority of them can be rescued even when they have no possibility to regenerate their axons. In this study the number of injured motoneurons rescued by riluzole treatment and their capacity to reinnervate the denervated forelimb muscles was investigated. Surgical reconnection with a peripheral nerve graft between the affected spinal cord segment and the C7 spinal nerve was established immediately or with 1-

PURPOSE: We reviewed patients with partial supraclavicular brachial plexus injuries in order to refine the myotome values of the upper limb.
METHODS: Forty-two patients with defined partial injuries to the supraclavicular brachial plexus were reviewed from a prospective database. The injuries patterns covered C5, C5-6, C5-7, C5-8, C7-T1 and C8-T1 roots. Upper plexus injuries were classified on the basis of surgical exploration and intraoperative stimulation and lower plexus injuries from MRI.
RESULTS: Flexor Carpi Radialis (FCR) was paralyzed in C5-7 injuries, in addition to paralysis of del
