Evaluation of clinical problems associated with bone metastases from carcinoma from unknown primary sites.

INTRODUCTION: This study focused on the evaluation of data concerning the clinical features of patients who were initially diagnosed with bone metastases of carcinoma from unknown primary sites that could not be detected, even using state of-the-art diagnostic modalities. METHOD: The oncologic outcome of these patients is discussed. PATIENTS: The clinical records of seven patients who had presented with bone metastases of carcinoma from unknown primary sites were retrospectively reviewed. Clinical features, treatment and outcome were analyzed. Extraskeletal metastases were located in the lym

Developmental control of segment numbers in vertebrates.

Segmentation or metamery in vertebrates is best illustrated by the repetition of the vertebrae and ribs, their associated skeletal muscles and blood vessels, and the spinal nerves and ganglia. The segment number varies tremendously among the different vertebrate species, ranging from as few as six vertebrae in some frogs to as many as several hundred in some snakes and fish. In vertebrates, metameric segments or somites form sequentially during body axis formation. This results in the embryonic axis becoming entirely segmented into metameric units from the level of the otic vesicle almost to t

Vascular surgery of the upper limb: the first year of a new vascular service.

Upper limb vascular reconstruction represents a small part of the vascular surgical workload (5%). The aim of this study was to assess the i … fting. One patient had an embolectomy and endarterectomy. Procedures for TOS included excision of a cervical rib in two patients (one bilateral) and scalenectomy alone in one patient. Of these, one patient also had thrombolysis and thrombectomy of the axillary and brachial artery. All of the patients made a good functional recovery and all arteries remained pa …

Skeletal abnormalities in the Apert syndrome.

This paper reports on skeletal abnormalities in 38 patients with Apert syndrome. Analysis includes alterations in the shoulders, humeri, elbows, hips, knees, rib cage, and spine (except the cervical spine). Some patients had subacromial dimples and elbow dimples during infancy. Mobility at the glenohumeral joint was limited. Progressive limitation in abduction, forward flexion, and external rotation with growth was virtually a constant finding. The acromioclavicular joint was prominent and sometimes had an angular, pointed appearance clinically. This was often associated with atrophic musculat

Modified open-door cervical expansive laminoplasty for spondylotic myelopathy: operative technique, outcome, and predictors for gait improvement.

Twenty-five patients underwent an expansive cervical laminoplasty for nontraumatic cervical spondylosis with myelopathy during the period from June 1990 to November 1994, and all had a minimum of 18 months of follow-up review. The open-door laminoplasty procedure presently reported consisted of the same approach evaluated by Hirabayashi in 1977, except that the authors of this report used three rib allografts to anchor the "open door," rather than spinous process sutures or autologous bone grafts. Posterior foraminotomies and decompression were performed in patients with clinical radiculopathy

A Stop-gain Variant c.220C>T (p.(Gln74*)) in FLNB Segregates with Spondylocarpotarsal Synostosis Syndrome in a Consanguineous Family.

Spondylocarpotarsal synostosis (SCT) syndrome is a very rare and severe form of skeletal dysplasia. The hallmark features of SCT are disproportionate short stature, scoliosis, fusion of carpal and tarsal bones, and clubfoot. Other common manifestations are cleft palate, conductive and sensorineural hearing loss, joint stiffness, and dental enamel hypoplasia. Homozygous variants in FLNB are known to cause SCT. This study was aimed to investigate the phenotypic and genetic basis of unique presentation of SCT syndrome segregating in a consanguineous Pakistani family. Three of the four affected si

The importance of computed tomography (CT) scans in the early diagnosis of Gorham-Stout Disease - A case report.

Gorham Stout disease (GSD) or vanishing bone disease is an infrequent entity in clinical practice characterized by gross and progressive bone loss along with excessive growth of vascular and lymphatic tissue. Very little is known about the pathogenesis of GSD, which makes the diagnosis challenging. Due to the rarity of the disease, no treatment guidelines have been created yet. We report a case of GSD in a 53-year-old male patient. He presented with bone pain and initial imaging showed widespread osteolytic lesions in the cervical and mid thoracic spine, ribs, sternum, clavicles, scapula and h

Motor recovery beginning 23 years after ischemic stroke.

It is widely believed that most stroke recovery occurs within 6 mo, with little benefit of physiotherapy or other modalities beyond 1 yr. We report a remarkable case of stroke recovery beginning 23 yr after a severe stroke due to embolization from the innominate artery and subclavian artery, resulting from compression of the right subclavian artery by a cervical rib. The patient had a large right frontoparietal infarction with severe left hemiparesis and a totally nonfunctional spastic left hand. He experienced some recovery of hand function that began 23 yr after the stroke, 1 yr after he too…

Endoscopic treatment of an intrathoracic tracheal osteochondroma in a dog.

CASE DESCRIPTION: A 1.5-year-old spayed female Bernese Mountain Dog was examined for a 6-month history of intermittent vomiting, regurgitation, wheezing, and coughing. Initially, a diagnosis of gastroesophageal reflux disease with secondary aspiration pneumonitis was made but clinical signs did not resolve with treatment. CLINICAL FINDINGS: Thoracic and cervical radiography and CT revealed a sessile, irregularly marginated soft tissue opacity at the level of the fourth rib. Results of a CBC, serum biochemical analysis, and urinalysis were within reference limits. Results of abdominal ultrason

A Rare Case of Progressive Gorham's Disease of Right Shoulder Girdle and Cervical Spine in A Child: 10 Year Follow-up and A Review of Literature.

INTRODUCTION: 'Vanishing Bone Disease' or Gorham's disease is a very rare form of primary idiopathic osteolysis with only around 200 cases being reported till date. We present in this case report a ten year follow-up of a patient who had progressive osteolysis of the right shoulder girdle and eventually involved the spine inspite of supplementary calcium and bisphosphonate therapy. We also report the utility of using an intra-medullary device is to attain acceptable union in case of pathological long bone fracture in Gorham's disease. The roles and mechanism of ultradian oscillatory expression of the mouse Hes genes.

Somites, metameric structures, give rise to the vertebral column, ribs, skeletal muscles and subcutaneous tissues. In mouse embryos, a pair of somites is formed every 2h by segmentation of the anterior parts of the presomitic mesoderm. This periodic event is regulated by a biological clock called the segmentation clock, which involves cyclic expression of the basic helix-loop-helix gene Hes7. Hes7 oscillation is regulated by negative feedback with a delayed timing. This process has been mathematically simulated by differential-delay equations, which predict that negative feedback with shorter

Study for collecting background data on Wistar Hannover [Crl:WI(Han)] rats in embryo-fetal development studies--comparative data to Sprague Dawley rats.

The purpose of the present study was to collect the background data on Wistar Hannover [Crl:WI(Han)] (hereafter Wistar Han) rats in embryo-fetal development studies from the 6 safety research facilities of pharmaceutical companies and contract research organizations. In each facility, 20 or 22 female rats were dosed with vehicle solution during the organogenesis period. As a result, no abnormalities in clinical signs and necropsy findings in dams were found. Body weights and food consumption in dams were lower than those in Sprague Dawley (SD) rats. The number of corpora lutea (13.3 vs. 16.0 i

A morphological study of skeletal development in turkeys during the pre-hatching stage.

Skeletal chondrofication, ossification and growth of turkey embryos were investigated and analysed to enable assessment of the developmental status and evaluation of the experimental effects on skeletal development, skeletal mutations and development of cultured embryos. Ten embryos were prepared every 24 h from 8 to 28 days of incubation. The fixed embryos were cleared and stained in toto with Alcian blue & Alizarin red for cartilage and ossified components, respectively. Observation of the skeleton was performed under a stereoscopic microscopy, with special attention to the timing of chondro

Thoracic facet joint innervation: identifying and accessing the articular branch.

BACKGROUND: The thoracic zygapophyseal (Z) joints, also known as facet joints, are potential sources of thoracic back pain. Historically, medial branch innervation of these joints was thought to mirror that of cervical and lumbar Z-joints. However, recent anatomical studies suggest that distinct thoracic articular branch nerves innervate these joints. This finding provides potential interventional targets for thoracic facet blockade and radiofrequency neurotomy. OBJECTIVE: This study aimed to identify the thoracic articular branch nerves for mid-thoracic joints and develop techniques for acce

The Human Disharmony Loop: The Anatomic Source Behind Subacromial Impingement and Pain.

Background: Subacromial impingement or pain syndrome (SAPS) is the most common diagnosis for chronic shoulder pain. Current surgeries do not reduce long-term pain, suggesting they miss the root etiology. Previously, we described the Human Disharmony Loop (HDL), where the unique lower trunk innervation to the pectoralis minor (PM) causes scapular dyskinesis and deforms its connections, including tugging the acromion down and impinging the subacromial structures. We hypothesize that SAPS patients who meet HDL criteria would benefit significantly from PM tenotomy with infraclavicular brachial ple

Bilateral Lacrimal Gland Mantle Cell Lymphoma in 11-Year Follow-Up: Case Report and Review of 48 Cases With Ocular Adnexal Presentation in the Literature.

A 63-year-old woman, with 11-year history of breast cancer, showed bilateral lacrimal gland enlargement on magnetic resonance imaging. Gallium-67 scintigraphy, as the standard at that time in 2004, demonstrated abnormally high uptake only in bilateral lacrimal glands. The lacrimal glands were extirpated and the pathological diagnosis was mantle cell lymphoma (MCL). She underwent bilateral orbital radiation, based on no uptake of gallium-67 in other sites of the body. In a month, bone marrow biopsy revealed the infiltration with MCL, positive for cyclin D1. She showed hepatic lymphadenopathy an
