Implementation of Universal Child Abuse Screening Tool (U-CAST): Real-world implementation of a standardized electronic health record-based child physical abuse screening in the pediatric emergency department.

BACKGROUND: Universal screening tools can help detect child physical abuse. Epic released an electronic health record-embedded universal child physical abuse screening tool (U-CAST) that can quickly be implemented by any institution that uses Epic. OBJECTIVE: We evaluated the first year of implementation of an adapted U-CAST in the pediatric emergency department at an academic hospital. PARTICIPANTS AND SETTING: All patients less than four years of age who arrived in the pediatric emergency department were screened using a four-question tool embedded into the electronic health record. METHO

Childhood interpersonal antecedents of adult romantic relationship adjustment: Prospective evidence from the National Institute of Child Health and Human Development Study of Early Child Care and Youth Development.

This report leveraged the subsample of romantically involved participants in the National Institute of Child Health and Human Development Study of Early Child Care and Youth Development at the most recent assessment of the cohort (n = 505; Mage = 28.6 years; 58.1% female; 81.7% White/non-Hispanic) to study the role of three theoretically salient childhood interpersonal experiences as potential antecedents of self-reported romantic relationship adjustment in early adulthood. Predictors were measured multiple times prospectively in childhood through adolescence and included (a) direct observatio…

A longitudinal birth cohort study of child maltreatment and mental disorders using linked statewide child protection and administrative health data for 83,050 Queensland residents from 1983 to 2014.

AIMS: Most information about the association between childhood maltreatment (CM) and subsequent psychiatric morbidity is based on retrospective self-reports. Findings from longitudinal studies using prospective reports to statutory agencies may be subject to attrition. We therefore compared the prevalence to age 30 of inpatient psychiatric diagnoses in those who experienced agency-reported CM with those of the rest of the cohort using administrative data to minimise loss to follow-up. METHODS: We used linked administrative data for two birth cohorts of all individuals born in Queensland, Aust

Can volunteer community health workers manage multiple roles? An interrupted time-series analysis of combined HIV and maternal and child health promotion in Iringa, Tanzania.

Community health workers (CHWs) play a critical role in health promotion, but their workload is often oriented around a single disease. Renewed interest in expansion of multipurpose CHWs to cover an integrated package of services must contend with the debate over how effectively CHWs can perform an increased range of tasks. In this study, we examine whether an existing cadre of HIV-focused paid volunteer CHWs in Iringa, Tanzania, can take on new maternal, newborn and child health (MNCH) promotion tasks without adversely affecting their HIV role. HIV household visits conducted per month were ex…

The impact of digital interventions on health insurance coverage for reproductive, maternal, newborn and child health services utilization in Kakamega, Kenya: a cluster randomized controlled trial.

The National Hospital Insurance Fund (NHIF) of Kenya was upgraded to improve access to healthcare for impoverished households, expand universal health coverage, and boost the uptake of essential reproductive, maternal, newborn and child health (RMNCH) services. However, premiums may be unaffordable for the poorest households. The Innovative Partnership for Universal Sustainable Healthcare (i-PUSH) programme targets low-income women and their households to improve their access to and utilization of quality healthcare, including RMNCH services, by providing subsidized, mobile phone-based NHIF co…

The science of child and adolescent mental health in Mozambique: A systematic review of prevalence estimates, measurement tools, and interventions.

Mozambique is home to 18 million children and adolescents largely underrepresented in mental health research. We conducted a systematic review of evidence-based resources (prevalence estimates, assessment instruments, and interventions) on mental health-related outcomes addressing individuals aged 0-19 years in Mozambique. We searched PubMed, Web of Science, PsycINFO, CINAHL, Google Scholar, African Index Medicus, and local catalogues. Extraction followed COSMIN and Cochrane manuals (PROSPERO registration: CRD420251016208). The search yielded 81 unique publications, including 61 reports presen

[Mental health of mothers of children with cancer treated in Pediatric Oncology at the mother and child hospital in Ivory Coast].

Scientific literature has shown that the management of pediatric cancer affects the mental health of parents. We therefore undertook this research to describe the psychosocial experiences of mothers of children with cancer in Ivory Coast. This mixed, cross-sectional, prospective, descriptive and analytical study involved 30 mothers of children with cancer. In terms of socio-demographic characteristics, the mothers surveyed had an average age of 39.37 years, and 83.4% of them were living as a couple. A proportion of 66.7% were employed. All had more than one child, except for one mother. Psycho

Investigation of the levels of As, Cd, Hg, and Pb in infant and young child formulas and health risk assessment in Türkiye.

Non-essential heavy metals pose health risks even at low concentrations in contaminated food. Complementary foods are widely used for infants and young children, often due to insufficient breastfeeding or unmet nutritional requirements. This study aimed to investigate the concentrations of As, Cd, Hg, and Pb in formulas for infants, babies, and young children (n = 51) sold in Türkiye and to assess the non-carcinogenic risks associated with the consumption of these products. The mean concentrations (ppb) were as follows: As (3.48), Cd (<LOD), Hg (4.68), Pb (10.42) in infant formula (n = 10); As

Prevalence and long term mental health outcomes of child maltreatment in Ethiopia: A systematic review and meta-analysis.

BACKGROUND: Child maltreatment (CM) is a pervasive issue with serious mental health (MH) symptoms. Data on prevalence and associations with MH from countries like Ethiopia remain limited. This gap highlights the need for evidence synthesis to inform public health planning and targeted interventions. OBJECTIVES: This review aimed to synthesize available evidence on the prevalence of CM, MH symptoms and to examine the CM-MH links. SETTING AND PARTICIPANTS: This review included total of 85,570 participants from 58 CM prevalence studies and 26,514 from 28 studies examining MH outcomes following

Whose business is autism? Exploring the intersecting roles of developmental paediatrics and child and adolescent mental health services.

This opinion article looks at how autism services for children and young people are organised within child development and child and adolescent mental health services. Although both play vital roles, their responsibilities often overlap, which can lead to delays and uncertainty for families. Rising demand, limited resources and differing pathways have added further pressure, especially since the COVID-19 pandemic. The article highlights key challenges, including age cut-offs, gaps in continuity, the need for a collaborative integrated model, the impact of stigma and the difficulties faced by c

Maternal, newborn, and child health continuum of care: completion and associated factors in Xiengkhuang and Huaphanh, Lao PDR.

BACKGROUND: Completing the full continuum of maternal, newborn, and child health (MNCH) care remains challenging in remote Lao PDR, which has among the highest maternal and neonatal mortality rates in Southeast Asia. Evidence on continuum of care (CoC) completion and its determinants is limited for Xiengkhuang and Huaphanh, two underserved provinces. METHODS: A community-based cross-sectional household survey was conducted in February 2023 using stratified three-stage cluster sampling across eight districts. Women aged 15-49 with a live birth in the preceding 24 months were enrolled (n = 380)…

Prioritising communicable disease research in Afghanistan: an application of the Child Health and Nutrition Research Initiative (CHNRI) methodology.

INTRODUCTION: Communicable disease control in Afghanistan has deteriorated amid growing fragility, health system disruption and declining international aid since the 2021 regime change. Outbreaks of measles, pertussis, pneumonia, cholera, malaria, dengue, Crimean-Congo haemorrhagic fever, tuberculosis and polio continue to plague the population in Afghanistan. This study addresses a critical evidence gap by systematically ranking research priorities for communicable diseases in Afghanistan. METHODS: This study applied the Child Health and Nutrition Research Initiative (CHNRI) methodology, whi…

Prenatal nitrate and nitrite intake in mammals: A scoping review of effects and associations with pregnancy and maternal and child health.

Nitrate and nitrite are present in drinking water, vegetables and processed meats. While they may offer health benefits in some contexts, high levels, especially during pregnancy, can be harmful. This scoping review investigates prenatal nitrate/nitrite exposure and maternal, child, pregnancy and birth outcomes, in animal models and humans. Six databases were searched systematically for peer-reviewed publications relating to nitrate and/or nitrite exposure in pregnancy. 3834 unique papers were identified, with 133 meeting inclusion criteria. In human studies (n = 66), high processed meat nitri

How State Factors Moderated the Impacts of the 2021 Child Tax Credit on Mental Health and Financial Hardship.

INTRODUCTION: Mental health problems during the COVID-19 pandemic in the U.S. were partly alleviated by economic policies. For example, the 2021 expansion of the child tax credit provided cash support to many families and improved mental health. Although the child tax credit was a federal policy, there may be geographic disparities in policy impacts depending on state contexts. This study examined how state factors modified the child tax credit's effects. METHODS: This study used April 2021-January 2022 individual-level data from the Census Household Pulse Survey (N=944,189). Outcomes include

Child maltreatment and mental health in later life: The moderating role of longitudinal lifestyle patterns in a UK cohort.

BACKGROUND: A healthy lifestyle may buffer the adverse mental health effects of childhood adversity. However, most studies assess isolated lifestyle behaviors at a single time point, failing to capture their synergistic and dynamic nature over the life course. The interplay between child maltreatment (CM) and long-term lifestyle trajectories in shaping mental health outcomes remains underexplored. OBJECTIVE: To examine how lifestyle trajectories moderate the association between CM and later-life mental health, and whether these moderation effects differ by sex and age. PARTICIPANTS AND SETTI
