Parasymphyseal Fracture Following Facial Feminization Surgery.

Facial feminization surgery encompasses a range of procedures aimed at providing gender-affirming transformations of the facial skeleton and soft tissue for transgender women. The performance of these procedures has increased in recent years, and several states have expanded insurance coverage for gender-affirmation surgeries. Facial feminization is generally considered safe and well-tolerated; however, there is a paucity of large studies examining complication rates and post-surgical issues. Here, the authors describe the first reported case of a parasymphyseal mandibular fracture after slidi

Treatment of an Extruded Middle Phalanx Epiphysis in a 6-Year-Old Boy: A Case Report.

CASE: A Salter-Harris II fracture of the small finger middle phalanx with complete extrusion of the epiphyseal fragment in a 6-year-old boy is described. Closed reduction was unsuccessful. Open reduction was performed, the epiphysis was reduced, and a single Kirschner wire was placed obliquely across the fracture and joint. CONCLUSION: This is an extremely rare injury. Open reduction and pinning was successful in preserving functional range of motion. Despite eventual growth arrest, there was no clinically significant angular deformity, and the joint was preserved. Surgical technique and earl

[Treatment of tibial fractures seen in the light of long-term results].

The intra-articular fracture of the lower extremity of the tibia is an injury which often produces functional problems in the ankle joint, sometimes leaving the patient incapable of continuing his original employment. Conservative treatment is often unsatisfactory because of the difficulty of closed reduction of the fracture, and the difficulty in maintaining the reduction. Open reduction and stable internal fixation frequently gives good results, even in cases of complicated fractures. The diagnosis is essentially based on standard X-ray examination of the ankle (face and profiles) with obliq

Endoscopic repair of isolated anterior table frontal sinus fractures without fixation.

Frontal sinus fractures constitute 5% to 15% of maxillofacial fractures, and isolated anterior table injuries account for 33% of frontal sinus fractures. The treatment strategy of frontal sinus fractures should be individualized according to the extent of the injury. Endoscope-assisted repair without any fixation method for the treatment of mildly and moderately displaced (1-5 mm) and closed isolated anterior table frontal sinus fractures is a good alternative technique for treatment.Between April 2010 and December 2011, 5 patients with mildly and moderately displaced isolated anterior table f

Choosing the proper implant for extra-articular fractures of proximal phalanges: A study on 75 cases.

INTRODUCTION: Phalangeal fractures are the most common fractures of the hand and in particular the proximal phalanx of the long fingers is the most involved. These fractures can ben conservatively managed but, when the fracture pattern is considered unstable, surgical treatment is recommended. However, there is no consensus in literature about the proper surgical option for extra-articular proximal phalanx fractures. MATERIAL AND METHODS: We compared clinical and radiographical results after treatment of 75 cases of extra-articular proximal phalanx fractures using three different surgical tec

MR Imaging of Pediatric Elbow Trauma.

Magnetic resonance imaging (MRI) of the pediatric elbow is important because it allows delineation of fracture type in a joint with complex anatomy, made more complex by the variable appearance of ossification centers of the distal humerus.1-4 Acute and chronic injuries can be evaluated with MRI using several classification systems. Accurate diagnosis of pediatric elbow trauma with MRI has management implications. For example, a Salter-Harris II fracture can be differentiated from a Salter-Harris IV physeal fracture by delineating intra-articular fracture extension and displacement, which requ

Minimally invasive technique for medial subtalar dislocation associated with navicular and entire posterior talar process fracture: a case report.

This is a case report of 31-year-old woman who sustained medial subtalar dislocation with navicular and entire posterior talar process fracture. After closed reduction of the subtalar dislocation, compute tomography (CT) demonstrated that the talus fracture involved the entire posterior process and navicular. The fracture of the talus was fixed with a cannulated screw and the navicular was held with two K-wires to the main body using with a minimally invasive approach. Follow-up revealed that the functional and radiographic results were graded as good.

Simultaneous rupture of the tibialis posterior and flexor digitorum longus tendons in a closed tibial fracture.

An interposition of ruptured tendons of the tibialis posterior and flexor digitorum longus occurred between the lower third tibial fracture fragments in a closed tibial fracture in a 26-year-old man. The tendon ruptures were not diagnosed preoperatively but were recognized at the time of open reduction. The tendons were repaired and the fracture internally fixed. Six months postoperatively, the patient had a 10 degree dorsal extension deficit in the ankle joint, the motion was painless, and the strength of the posterior tibial compartment muscles was grade 5.

Development and Assessment of a Distal Radial Fracture Model as a Clinical Teaching Tool.

BACKGROUND: Simulation-based learning is increasingly prevalent in the curricula of many surgical training programs. Newly developed simulators must undergo rigorous validity testing before they are used to assess and evaluate surgical trainees. We describe the development of a model that simulates a distal radial fracture requiring closed reduction and cast application and demonstrate its validity. METHODS: We developed a model for simulated treatment of a distal radial fracture with use of a modified Sawbones forearm. Ten junior and ten senior orthopaedic residents were videotaped performin

Simultaneous fracture of the scaphoid and radius in a child.

A 15-year-old boy sustained an ipsilateral scaphoid and radius fracture, a combination of injuries that has only received brief mention in previous reports. Closed reduction and cast immobilization produced a satisfactory result. The distal radius injury was a comminuted fracture with a vertical component that extended to the physis without involvement of the epiphysis. The mechanism of injury may have been a severe impact delivered to the base of the hand with the wrist fixed in dorsiflexion, a force that would make a distal radial epiphyseal fracture unlikely.

Transverse divergent dislocation of the elbow with ipsilateral distal radius fracture in a child.

We report a rare case of an irreducible transverse divergent dislocation of the elbow with an ipsilateral distal radius torus fracture and a fracture of the coronoid process in a 9-year-old male. Closed reduction of the elbow was attempted, but the humeroulnar joint remained dislocated. At surgery, the avulsed anterior band of the medial collateral ligament complex of the elbow was found to be interposed between the medial condyle of the humerus and the olecranon. The dislocation was reduced after relieving the ligament entrapment, which was then repaired. The unstable proximal radioulnar join

Nasal fracture reduction: local versus general anaesthesia.

OBJECTIVE: To investigate the outcomes from nasal fracture reduction performed under local anaesthesia (LA) and general anaesthesia (GA). METHOD: A randomised multi-centred prospective trial and cohort analysis. Patients were randomised into two groups, 74 (53%) underwent closed reduction under LA, 65 (47%) patients underwent closed reduction under GA. MAIN OUTCOME MEASURES: Pain scores and patient toleration of local or general nasal manipulation was noted. RESULTS: The pain score ranged from 0 (no pain) to 10 (unbearable pain). The mean pain score in the LA group was 3, compared to 2 in t

Treatment of diaphyseal forearm fractures in children.

Both bone forearm fractures are common orthopedic injuries. Optimal treatment is dictated not only by fracture characteristics but also patient age. In the pediatric population, acceptable alignment can tolerate greater fracture displacement due to the bone's ability to remodel with remaining growth. Generally, these fractures can be successfully managed with closed reduction and casting, however operative fixation may also be required. The optimal method of fixation has not been clearly established. Currently, the most common operative interventions are open reduction with plate fixation vers

Trends in Medicare and Medicaid Physician Fee and Facility Pricing Reimbursements for Craniofacial Trauma Procedures From 2007 to 2024.

Reimbursement trends influence the sustainability of craniofacial trauma practices, yet limited literature addresses long-term patterns. This study evaluates national trends in Medicare physician fees (PF) and facility pricing (FP) for craniofacial trauma procedures from 2007 to 2024. Reimbursement data were obtained from the Medicare Physician Fee Schedule Look-Up (MPFS) tool by the Centers for Medicare & Medicaid Services (CMS). Craniofacial trauma surgeries were identified using Current Procedural Terminology (CPT) codes. PF values were calculated using national payment amounts, incorporati

Delayed Migration of Greater Tuberosity Fractures Associated With Anterior Shoulder Dislocation.

OBJECTIVES: Treatment of greater tuberosity (GT) fractures occurring during anterior shoulder dislocation generally consists of initial closed reduction of the shoulder. Undisplaced fractures are treated conservatively, whereas displaced fractures generally undergo surgical management. Our hypothesis is that many well-reduced GT fractures will migrate. The primary goal of this study is to evaluate the outcomes of GT fractures associated with shoulder dislocation to optimize their management. DESIGN: Retrospective review with prospective outcomes. SETTING: Tertiary Level 1 trauma center. PAT

[Secondary intramedullary nailing osteosynthesis in primarily conservative treatment of fractures of tibia and fibula shafts (author's transl)].

In a retrospective trial 235 patients with fractures of the tibia and/or fibula shafts were followed up. In 16 cases non-union of the fracture occurred after closed reduction and plaster cast treatment. In 16 of these patients closed nailing of the tibia was performed 8--14 weeks after the accident. The healing of the fracture was uncomplicated and occurred in 10--14 weeks p. op. The results in primary nailing cases 8--14 days after the accident were much better than in cases treated with plaster casts, with regard to joint mobility, venous insufficiency and duration of treatment; however two

A reduction apparatus for femur fractures.

The majority of femur fractures are closed fractures in the mid-third of the femoral shaft. A very well known fixation technique is the one using an intramedullary pin, which is inserted from the hip side. Open reduction of the fracture is mostly applied but the majority, between 85 and 93%, of the fractures are closed and therfore a more logical and safer technique would be closed reduction. However, this technique, also called blind-nailing, is more difficult. Besides an x-ray scanner with a monitor it also requires experience of the surgeon and some special tools to supply the necessary red

Displaced distal radius fracture presenting with neuropraxia of the dorsal cutaneous branch of the ulnar nerve (DCBUN).

Nerve injuries, mostly to the median nerve, are common following distal radius fractures. Ulnar nerve injuries are rarely encountered, with only few case reports of motor or motor and sensory loss described in the literature. In this paper, we report two consecutive cases of young patients with a distal radius fracture and a pure sensory ulnar neuropathy. Both patients had a radially displaced fracture and presented with sensory loss and paresthesia in the distribution of the dorsal cutaneous branch of the ulnar nerve (DCBUN), which resolved after fracture reduction. We believe this clinical s
