Real-time bowel perfusion monitoring with FUJIFILM ELUXEO® VISION endoscopic imaging system in colorectal surgery.

BACKGROUND: Anastomotic leak is a serious complication of bowel anastomosis, with multifactorial etiology. Thorough intraoperative evaluation of the anastomosis using white light endoscopy and leak tests is critical. Indocyanine green fluorescence angiography (ICG-FA) has emerged as a tool to assess bowel perfusion, but has limitations, including patient allergies and procedural complexity. The FUJIFILM ELUXEO® VISION Endoscopic Imaging System uses oxygen saturation imaging (OSI) to measure tissue oxygen saturation (StO2) in real time, without requiring dye injection. This study investigates a

Risk factors for unplanned 31-day readmission after surgery for colorectal cancer patients: a meta-analysis.

BACKGROUND: The high incidence of unplanned readmissions within 31 days after colorectal cancer surgery remains a significant challenge. However, the identified risk factors for these readmissions are inconsistent across the literature. This study aims to perform a comprehensive meta-analysis to estimate the incidence of unplanned readmissions and systematically identify the factors associated with this risk, providing robust evidence for targeted interventions to reduce readmission rates. METHODS: This study was conducted in accordance with the PRISMA guidelines. All study steps, including s

AUGUR-AI: a real-time artificial intelligence interpreter for indocyanine green fluorescence angiography in colorectal surgery.

BACKGROUND: Assessment of intestinal perfusion sufficiency is a key surgeon judgment regarding anastomotic construction. Recently indocyanine green fluorescence angiography (ICGFA) has been proven as a digital technology that at least matches such acumen when correctly interpreted. Here, we report a deployed artificial intelligence (AI) device that automatically provides and documents expert-ICGFA interpretation in real-time operations. METHODS: Intraoperative ICGFA videos from consenting patients undergoing colorectal resection and primary anastomosis without anastomotic leak were used to cr

Evaluating long-term outcomes after cytoreductive surgery for colorectal peritoneal metastasis: do long survivors really exist?

BACKGROUND: Cytoreductive surgery (CRS) combined with hyperthermic intraperitoneal chemotherapy (HIPEC) has demonstrated improved overall survival (OS) and disease-free survival (DFS) in carefully selected patients with peritoneal metastases from colorectal cancer (CRC-PM). The primary aim of this retrospective study was to identify patients treated with CRS + HIPEC who achieved long-term survival and to analyze the factors most strongly associated with prolonged survival. METHODS: Among those eligible for CRS + HIPEC, a group of patients with adequate follow-up was evaluated and divided into

Peritoneal infection after colorectal cancer surgery induces substantial alterations in postoperative protein levels: an exploratory study.

PURPOSE: Peritoneal infection, due to anastomotic leakage, after resection for colorectal cancer have been shown to associate with increased cancer recurrence and mortality, as well as cardiovascsular morbidity. Alterations in circulating protein levels could help shed light on the underlying mechanisms, prompting this exploratory study of 64 patients operated for colorectal cancer with anastomosis. METHODS: Thirty-two cases who suffered a postoperative peritoneal infection were matched with 32 controls who had a complication-free postoperative stay. Proteins in serum samples at their first p

Correlation between recurrence-free survival and overall survival after upfront surgery for resected colorectal liver metastases.

BACKGROUND: The role of recurrence-free survival (RFS) as a valid surrogate endpoint for overall survival (OS) in patients who underwent upfront surgery for colorectal liver metastases remains uncertain. The aim of the study was to compare the two survival measures in a nationwide cohort of upfront resected colorectal liver metastasis. METHODS: Data from patients with colorectal liver metastases without extrahepatic metastases who underwent curative surgery for liver metastases were retrieved from the Japanese nationwide database (data collection 2005-2007 and 2013-2014). RFS, OS, and surviva

[Laparoscopic downstaging surgery for colorectal cancer with synchronous liver metastases: what value in two-stage hepatectomies?].

Bilobar hepatic metastases from colorectal cancer pose a problem in terms of management, with curative surgery often requiring several stages. The purpose of our study was to evaluate laparoscopic approach with portal vein ligation in the first step of two-stage hepatectomy in the treatment of patients with synchronous liver metastases from colorectal cancers (SLMCRC). We conducted a single-center retrospective study from August 2016 to January 2020. It included patients with SLMCRC requiring two-stage curative surgery due to insufficient future liver remnant volume (FRL). The primary endpoint

Multispectral Imaging Using Fluorescent Properties of Indocyanine Green and Methylene Blue in Colorectal Surgery-Initial Experience.

Introduction: Image-guided surgery is becoming a new tool in colorectal surgery. Intraoperative visualisation of different structures using fluorophores helps during various steps of operations. In our report, we used two fluorophores-indocyanine green (ICG), and methylene blue (MB)-during different steps of colorectal surgery, using one camera system for two separate near-infrared wavelengths. Material and methods: Twelve patients who underwent complex open or laparoscopic colorectal surgeries were enrolled. Intravenous injections of MB and ICG at different time points were administered. Visu…

Novel surgical procedure for preventing anastomotic leakage following colorectal cancer surgery: A propensity score matching study.

Hypoperfusion is the main cause of anastomotic leakage (AL) following colorectal surgery. The conventional method for evaluating anastomotic perfusion is to observe color change and active bleeding of the resection margin of the intestine and the pulsation of mesenteric vessels. However, the accuracy of this method is low, which may be due to insufficient observation time. A novel surgical procedure that separates the mesentery in advance at the intended transection site can delay the observation of anastomotic perfusion, and can potentially detect more anastomotic sites with insufficient vasc…
