Antimicrobial Resistance Trends Among Obligate Anaerobic Bacteria Isolated From Chronic Suppurative Otitis Media Patients in a Tertiary Care Teaching Hospital in Central India.

INTRODUCTION: Chronic suppurative otitis media (CSOM) remains a major public health concern, especially in low- and middle-income countries, contributing significantly to preventable hearing loss. While aerobic bacteria are well-recognized pathogens in CSOM, anaerobic bacteria's role is increasingly acknowledged due to improved sampling and culture techniques. Anaerobes may synergize with aerobes, worsening disease severity and complicating management. This study aimed to identify the anaerobic bacterial profile in CSOM patients and assess antimicrobial susceptibility patterns of these isolate

Integrating UPLC-Q-Orbitrap MS with serum pharmacochemistry network and experimental verification to explore the pharmacological mechanisms of Cynanchi stauntonii rhizoma et radix against sepsis-induced acute lung injury.

Introduction: Patients with sepsis are at an incremental risk of acute lung injury (ALI). Baiqian, also known as Cynanchi stauntonii rhizoma et radix (Csrer), has anti-inflammatory properties and is traditionally used to treat cough and phlegm. This study aimed to demonstrate the multicomponent, multitarget, and multi-pathway regulatory molecular mechanisms of Csrer in treating lipopolysaccharide (LPS)-induced ALI. Methods: The bioactive components of Csrer were identified by ultrahigh-performance liquid chromatography Q-Orbitrap mass spectrometry (UPLC-Q-Orbitrap MS). Active targets predicted

Surgical Outcome of Cauda Equina Syndrome Secondary to Disc Herniation Presenting Late in Developing Countries.

Cauda equina syndrome (CES) is a serious neurologic state in which neurological dysfunction affects the lumbar and sacral nerve roots within the vertebral canal. The nerves of the cauda equine provide the muscles that manage the bladder, bowel and the legs and the dysfunction producing impairment of bladder, bowel or sexual function and perianal or saddle numbness. Identification of CES is not only important to neurosurgeons and spine surgeons, but also to prime care practitioners, emergency room physicians, physiotherapists and allied health care professionals concerned in management of back

Complementary deficits in perceptual classification in pure alexia and acquired prosopagnosia - New insights from two classic cases.

Pure alexia and prosopagnosia traditionally have been seen as prime examples of dissociated, category-specific agnosias affecting reading and face recognition, respectively. More recent accounts have moved towards domain-independent explanations that postulate potential cross-links between different types of visual agnosia. According to one proposal, abnormal crowding, i.e. the impairment of recognition when features of adjacent objects are positioned too closely to each other, might provide a unified account for the perceptual deficits experienced by an agnosic patient. An alternative approac

Vector saliva controlled inflammatory response of the host may represent the Achilles heel during pathogen transmission.

Infection with vector-borne pathogens starts with the inoculation of these pathogens during blood feeding. In endemic regions, the population is regularly bitten by naive vectors, implicating a permanent stimulation of the immune system by the vector saliva itself (pre-immune context). Comparatively, the number of bites received by exposed individuals from non-infected vectors is much higher than the bites from infected ones. Therefore, vector saliva and the immunological response in the skin may play an important role, so far underestimated, in the establishment of anti-pathogen immunity in e

Designing a therapeutic hepatitis B vaccine to circumvent immune tolerance.

An effective prophylactic hepatitis B virus (HBV) vaccine has long been available but is ineffective for chronic infection. The primary cause of chronic hepatitis B (CHB) and greatest impediment for a therapeutic vaccine is the direct and indirect effects of immune tolerance to HBV antigens. The resulting defective CD4+/CD8+ T cell response, poor cytokine production, insufficient neutralizing antibody (nAb) and poor response to HBsAg vaccination characterize CHB infection. The objective of this study was to develop virus-like-particles (VLPs) that elicit nAb to prevent viral spread and prime C

Comparative Study of Xenobiotic-Free Media for the Cultivation of Human Limbal Epithelial Stem/Progenitor Cells.

The culture of human limbal epithelial stem/progenitor cells (LSCs) in the presence of animal components poses the risk of cross-species contamination in clinical applications. We quantitatively compared different xenobiotic-free culture media for the cultivation of human LSCs. LSCs were cultured from 2 × 2 mm limbal tissue explants on denuded human amniotic membrane with different xenobiotic-free culture media: CnT-Prime (CnT-PR) supplemented with 0%, 1%, 5%, and 10% human serum (HS), embryonic stem cell medium (ESCM) alone or in combination with the standard supplemented hormonal epithelium

Deep brain stimulation of the subthalamic nucleus regulates postabsorptive glucose metabolism in patients with Parkinson's disease.

OBJECTIVE: Subthalamic nucleus-deep brain stimulation (STN-DBS) is an alternative treatment for patients with uncontrolled symptoms of Parkinson's disease (PD), but it has other nonmotor impact. Because STN-DBS alters the energy expenditure in humans, we hypothesized that STN-DBS may affect postabsorptive glucose metabolism in patients with PD. RESEARCH DESIGN AND METHODS: Endogenous glucose production (EGP) and whole-body glucose disposal rates (GDRs) were assessed in the postabsorptive state during a primed continuous iv infusion of D-[6,6-(2)H2]glucose for 5 hours in 8 STN-DBS-treated pati

Syk-kinase inhibition prevents mast cell activation in nasal polyps.

BACKGROUND: Mast cells are crucial effector cells in the allergic cascade. The cross-linking of the high affinity IgE receptor (FcεRI) activates mast cells and basophils. Spleen tyrosine kinase (Syk) is positioned upstream of the IgE receptor signal transducing pathway and may represent an important target for the treatment of nasal inflammatory diseases. OBJECTIVE: We measured effects of a specific Syk inhibitor in the release of mast cell mediators in human cord blood-derived mast cells (CBDMCs) (in-vitro) and in human nasal tissue (ex-vivo). METHODS: Surgical samples were collected from p

Polycystic ovaries in nonobese adolescents and young women with ovarian androgen excess: relation to prenatal growth.

OBJECTIVE: Reduced growth before birth is known to associate with a smaller ovarian volume in adolescents and women without androgen excess. We studied whether prenatal growth relates also to ovarian size and polycystic ovary (PCO) morphology in nonobese adolescents and young women with ovarian androgen excess. DESIGN: A cross-sectional analysis of standardized case notes over a 2-yr period was performed. PATIENTS: Nonobese adolescents and young women (age approximately 17 yr; n = 86) seen for ovarian androgen excess, as confirmed by 17-hydroxy-progesterone hyperresponse to a GnRH agonist, w

Impact of acute and chronic low-dose glucocorticoids on protein metabolism.

CONTEXT: High-dose glucocorticoids cause acute protein loss by increasing protein breakdown and oxidation. Whether lower glucocorticoid doses, typical of therapeutic use, induce sustained catabolism has not been studied. OBJECTIVE: Our objective was to assess the effect of acute and chronic therapeutic glucocorticoid doses on protein metabolism. DESIGN AND SETTING: We conducted an open longitudinal and a cross-sectional study at a clinical research facility. PATIENTS AND INTERVENTION: Ten healthy subjects were studied before and after a short course of prednisolone (5 and 10 mg/d sequential

Dialysate as food: combined amino acid and glucose dialysate improves protein anabolism in renal failure patients on automated peritoneal dialysis.

Protein-energy malnutrition as a result of anorexia frequently occurs in dialysis patients. In patients who are on peritoneal dialysis (PD), dialysate that contains amino acids (AA) improves protein anabolism when combined with a sufficient oral intake of calories. It was investigated whether protein anabolism can be obtained with a mixture of AA plus glucose (G) as a source of proteins and calories during nocturnal automated PD (APD). A random-order cross-over study was performed in eight APD patients to compare in two periods of 7 d each AA plus G dialysate obtained by cycler-assisted mixing

A simultaneous outbreak of Serratia marcescens and Klebsiella pneumoniae in a neonatal intensive care unit.

We describe two concurrent outbreaks of Serratia marcescens and Klebsiella pneumoniae in a neonatal intensive care unit (NICU). Over a 16-month period, a total of 27 infants were either colonized (N=14) or infected (N=13). There were 15 cases of S. marcescens and 11 cases of K. pneumoniae. Both micro-organisms were involved in one fatal case. Seven preterm babies developed septicaemia, two had bacteraemia, three had respiratory infections and one had purulent conjunctivitis. The S. marcescens and K. pneumoniae isolates were investigated by three molecular methods: enterobacterial repetitive in

Structure, activity, and immune (T and B cell) recognition of botulinum neurotoxins.

Botulism, which was first reported over a century ago, is caused by botulinum neurotoxins produced by Clostridium botulinum in seven immunological serotypes (A through G). The primary structures of a number of these BoNTs have been determined and are reviewed here, together with their gene structure and synthesis. The biological actions of BoNTs, which result in their ability to block neurotransmitter release have been the subject of intensive study, and in this review we discuss the binding of BoNTs to the cell surface as well as the mechanism of their intercellular action. The ability of BoN

Neurotoxicity due to central nervous system therapy for childhood leukemia.

Therapy for occult or overt meningeal leukemia produces subclinical or clinical neurotoxicity in a variable proportion of children with acute lymphoblastic leukemia (ALL). The type, frequency, and permanence of these central nervous system (CNS) changes depend primarily on the therapy itself, although the contribution of additional factors, such as young age, may be substantial. Neurotoxicity in patients who have received 2,400 cGy cranial irradiation plus 5 concurrent doses of intrathecal methotrexate as CNS prophylaxis has been characterized more fully than the CNS changes accompanying other

A recombinant pseudorabies virus co-expressing G2b and G2c S1 broadens neutralizing antibody responses against PEDV variants and enhances pre-existing vaccine-induced antibody responses.

Porcine epidemic diarrhea virus (PEDV) G2c variants harbor substantial antigenic alterations that reduce the cross-neutralizing activity of antibodies induced by current vaccines and may contribute to vaccine failure in the field. To explore a vaccine candidate capable of broadening antibody responses against genetically and antigenically diverse PEDV strains, we generated three gene-deleted pseudorabies virus (PRV) recombinant vaccines expressing S1 proteins of G2b, G2c, or both G2b and G2c PEDV strains. The biological safety, genetic stability and immune efficacy of the three candidates were

Common Ragweed Allergy Under Global Change Linking Invasion-Driven Aeroallergen Exposure with Molecular Sensitization and Allergic Airway Disease.

The case of common ragweed (Ambrosia artemisiifolia) stands as a prime example of an invasive plant that links global change, biological invasion, aeroallergen exposure, and allergic airway disease. However, evidence remains fragmented across invasion biology, aerobiology, molecular allergology, and respiratory medicine. By separating plant occurrence, pollen abundance, molecular allergen dose, sensitisation, and airway disease, this review develops an integrated invasion-exposure-disease logic for interpreting ragweed-related health risk. We examine how climate change, land-use disturbance, r

A Live Single-Cycle RSV Vaccine with Immune Responses Focused on Prefusion F and the Central Conserved Region of Attachment Protein G.

Respiratory syncytial virus (RSV) is a major cause of severe lower respiratory tract disease in infants and children, and a pediatric vaccine is not available. Among various approaches, live vaccines are attractive as they induce broad multi-antigenic responses and have never been associated with vaccine-enhanced disease in RSV-naïve children. We previously reported a live prefusion F (preF) expressing single-cycle RSV vaccine, which was deficient in cell-cell transmission and yet induced protective responses in mice. To improve efficacy, we focused the anti-attachment protein (G) response on
