INTRODUCTION: "Tubeless" percutaneous renal surgery has previously been described at our institution. Avoiding external nephrostomy tube drainage significantly decreases the pain and morbidity of percutaneous renal surgery. However, internal drainage with a double-J ureteral stent still results in bothersome bladder symptoms, especially in younger male patients. In addition, removal of the stent requires uncomfortable office cystoscopy. We report a technical enhancement of our "tubeless" technique that uses a novel tail-stent for internal drainage.

OBJECTIVE: The purpose of this study was to evaluate the prognostic criteria of early fetal megacystis.
DESIGN: A prospective, transvaginal ultrasound, cross-sectional study at 11-15 weeks of gestation at a tertiary referral fetal medicine unit.
SUBJECTS AND METHODS: Sixteen pregnancies out of a total of 5240 were identified with early fetal megacystis. Fetal biometry, morphology, amniotic fluid, bladder size and volume were also evaluated. A 64-year-old patient came to urology OPD with a manually inserted electric wire piece in the urethra. A plain X-ray KUB confirmed the location of an electric wire in the urethra extending to the bladder. He was scheduled for a cystoscopy, on which it was observed that there was an electric wire piece present in the urethra, which was removed under direct vision. Urothelial cell carcinoma of the bladder is exceedingly rare in pediatric patients. A 43-year-old woman presented to our hospital with the chief complaints of gross hematuria and dysuria. This is a case report concerning a 60 years old man who lived for a short period in an endemic area of Khuzestan Province (neighboring province of Persian Gulf in Iran) for approximately 20 years ago. Recently he referred to the Urology Department of Kerman University of Medical Sciences with hematuria and dysuria. In the sonography a polypoid mass on the bladder floor was observed. In the cystoscopy and biopsy a bladder tumor (Simultaneous squamous cell carcinoma and Transitional Bladder Cell Carcinoma) and schistosomiasis (Schistosoma haematobium) was diagnosed.

A case of tubulovillous adenoma arising in an augmented bladder is described. Ureteroileal substitution and ileocystoplasty was performed when the patient was 18 years old. She noticed gross hematuria 44 years after the surgery. Cystoscopy revealed a non-papillary multiple tumor at the site of ileovesical anastomosis and transurethral resection biopsy was performed. Within 10 days after cystoscopy causing urosepsis this patient developed persistant neckpain as initial symptom of vertebral osteomyelitis. E. coli was isolated from urine, blood cultures and later from bone biopsy. Antibiotic treatment did not stop the progress of the disease. A transverse spinal cord syndrome occurred due to a pathological fracture of C5 and C6 and operative decompression was necessary. The rapid onset of osteomyelitis was impressive. For effective treatment of bacterial osteomyelitis a bone biopsy is sometimes unavoidable and indicated.

A 43-year-old man presented with left leg pain and an abdominal mass. Sonography and cystoscopy showed a bladder tumour obstructing the left ureteral orifice. The left kidney became nonfunctional. Computerized tomography suggested psoas abscess. Nephrectomy was done. Tumoral cells identified at cytological examination of psoas abscess but they were not found in the pyonephrotic kidney's pus. The patient declined further therapy and died 3 months postoperatively. This is the first case of bladder cancer presenting initially with psoas abscess.

We report a case of transitional cell carcinoma arising in the fossa navicularis. The patient was a 74-year-old man. He had no history of sexually transmitted disease or urethral stricture. Clinically, the tumor was suspected to be a condyloma acuminatum; however, the pathological diagnosis yielded an unexpected result: transitional cell carcinoma. Radiological examinations and cystoscopy showed no other tumor besides the primary cancer in the fossa navicularis. Partial resection of the urethra was performed and the patient has been without evidence of disease for 3 years.

Experience had with suprapubic cystoscopy and endovesical surgery, performed in a series of twenty-two patients presenting various diseases of the bladder--tumors, mainly of the anterior wall, calculi, foreign bodies, bladder neck sclerosis, defective Foley catheter--is shared. As a sole complication a case of penetrating the abdominal cavity without inflicting lesion to the intestine, is described. The suprapubic approach significantly broadens the scope of transurethral surgery, preserves its advantages, and is furthermore free of the shortcomings associated with open operations.

Secondary amyloidosis of the bladder is a rare cause of hematuria, with only 10 previous reports. Findings and treatment in 5 patients with histologically verified bladder amyloidosis secondary to long-standing rheumatoid arthritis are presented. All patients were hospitalized for massive hematuria. One patient died before cystoscopy. Clot evacuation and fulguration through a cystoscope stopped the bleeding in 1 patient. In the other 3 patients hemostasis was achieved by cystotomy, bilateral percutaneous nephrostomy and continuous alum irrigation of the bladder, respectively.

The association of daily oral cyclophosphamide with the development of transitional cell carcinoma or squamous cell carcinoma of the bladder had been documented. We report the first 2 cases of transitional cell carcinoma of the renal pelvis and an additional 4 cases of transitional cell carcinoma of the bladder following cyclophosphamide ingestion. Patients who have been taking daily oral cyclophosphamide for a prolonged period should have excretory urography, cystoscopy and bladder mucosal biopsies performed as part of a cancer surveillance protocol.

Watching the way a patient voids is the easiest way to determine the presence of prostatic obstruction. Uroflowmetry can objectively measure the amount of obstruction. Clinical evaluation of prostatism should include an intravenous urogram, serum creatinine determination, urinalysis, urine culture, and cystoscopy. A case of menouria (Youssef's syndrome) following a vesicovaginal fistula repair is presented. The patient had typical clinical features of cyclic hematuria, absence of vaginal bleeding and complete urinary continence.
