Viral infections have long been suspected to be causative agents in a number of inner ear dysfunctions. With few exceptions, the virus has not been demonstrated as the direct agent leading to hearing loss and/or vertigo. Selective inner ear changes have been observed recently in sensory and nonsensory epithelial cells in the ferret model for Reye's syndrome after intranasal inoculation with influenza B combined with aspirin administration and the creation of an arginine deficiency. Such findings suggest that these agents act synergistically on the inner ear, particularly on cells that are meta…

The question of damage to the ear from exposure to ionizing radiation was addressed by exposing groups of chinchillas to fractioned doses of radiation (2 Gy per day) for total doses ranging from 40 to 90 Gy. In order to allow any delayed effects of radiation to become manifest, the animals were sacrificed two years after completion of treatment and their temporal bones were prepared for microscopic examination. The most pronounced effect of treatment was degeneration of sensory and supporting cells and loss of eighth nerve fibers in the organ of Corti. Damage increased with increasing dose of …

Analysis of routine hospital statistics confirms anecdotal reports of an epidemic of surgery for glue ear. The surgical rate rose by 74% in Oxford Region between 1975 and 1981. The peak ages for surgery are 5-7 years, and the rates are higher in boys than in girls (1.4:1.00). The relative risk is high in social class I (1.16) and low in social class V (0.61). Although the prevalence of glue ear is hard to ascertain, the current surgical epidemic probably reflects an increase in apparent prevalence rather than a real increase in morbidity.

The impedance bridge test is a sensitive and objective measure of the integrity and function of the middle ear. Tympanograms obtained from children with a specific reading disability were compared with those from a matched group of controls. A significantly higher prevalence of defects of the middle ear was shown in the test group.

For the past 20 years, the author has been treating recurrent chronic seborrheic dermatitis with a suspension of human cerumen in glycerine. The cerumen is obtained from healthy children and young adults in which there is an over-production which requires removal. If the cerumen contains debris it is not used. The collected cerumen is suspended in 50% glycerine and left for several weeks to sterilize. It is then strained through a fine filter having been heated so that its is workable. The material is dispensed to the patient in a small dropper bottle and the patient is instructed to place two…

Microdissection and scanning electron microscopy were used to study temporal bone specimens from a newborn infant with Potter's syndrome. The 34-year-old mother of the infant had juvenile-onset rheumatoid arthritis and she took prostaglandin inhibitors (aspirin, indomethacin, and Clinoril) during pregnancy. Autopsy findings included bilateral renal dysgenesis, pulmonary hypoplasia, abnormal facies with low-set ears, and bilateral cataracts. The infant's left external ear canal was stenotic and the left tympanic anulus was narrowed. Otherwise, both middle ears, as well as the sensory structures…

Custom-built in-the-ear hearing aids from five manufacturers were submitted to an extensive battery of audiologic tests to compare the usefulness of each unit for a moderate-severe sensorineural hearing loss as well as to examine differences between monaural versus binaural fittings. Using a ranking procedure, the hearing aids from one manufacturer were determined to be the best for this 30-year-old patient. Substantial differences were observed in each test condition. The variability of speech discrimination measures, however, was not definitive in determining the hearing aid which was best, …

Speech perception was tested in high level noise under controlled laboratory conditions in noise-exposed workmen and normal subjects, with and without a hearing protector. The group was further divided by age and English fluency, the latter group being included because of the high proportion of non-fluent English speakers in the Canadian workforce. In normal-hearing subjects the highest discrimination scores were found without background noise, they were lower with white noise as a masker, and even lower with crowd noise as a masker; wearing of a protector had no effect on intelligibility. The…

The benign, monomorphic adenoma, derived from the respiratory epithelium of the middle ear, is a well circumscribed, greyish, avascular tumor. Its histology--documented by light and electron micrographs contained in this report--supports a distinct entity and pathogenesis apart from the malignant adenocarcinoma. Utilizing two case reports, this compendium emphasizes the clinical presentations, as well as radiographic and audiometrical characteristics, of the tumor. The first case represents the typical tumor with conductive hearing loss. The second case is the first reported facial paralysis a…

A controversy exists concerning whether or not roentgenographic narrowing or nonvisualization of the vestibular aqueduct is a specific sign for Meniere's disease. Of 190 ears that were evaluated, abnormal aqueducts were seen in 42.9% of ears with Meniere's disease, 45.4% of contralateral, noninvolved ears from patients with Meniere's disease, 41.3% of ears with diseases other than Meniere's disease, 30.4% of ears with no disease, and 51.6% of normal ears. Narrowing or nonvisualization of the vestibular aqueduct is a nonspecific roentgenographic sign that is seen in diseased, as well as in norm…

The prominent ear must always be seen as an aesthetic unity made up of a large number of anatomic details so that a natural and not a surgically corrected result is obtained. To achieve this goal no standard techniques should be used. Each anatomic detail must be treated in each ear.

Stiffening and tenderness of the helix of the ear, with radiologically demonstrable calcific deposits, has been reported in various conditions such as frostbite, and also in Addison's disease. To a total of 14 clinical observations with only 4 well documented cases, a first autopsy report of the disorder in a case of tuberculosus Addison's disease is added. It is shown that the elastic cartilage of the pinna, of the epiglottis and of the trachea undergoes ossification without preceding dystrophic or metastatic calcification. The ossification has been observed only in males with longstanding Ad…

After the definition of the naturally-looking auricle, a combination of the well-known excision-suture, incision-suture, and scraping-technique is described. The crus inferior and the corpus of the anthelix are built by a bridge of cartilage, whereas the crus superior is formed by scratching the cartilage on the front side. The concha is made smaller by excision. This technique can be applied almost always: it is leading to a good cosmetic result and it is time-sparing.
