Feature library for behavioural characterization of early and late seizures in an experimental model of post-traumatic epilepsy.

BACKGROUND: To generate a behavioral feature library for a more granular description of seizure semiology in rats with traumatic brain injury (TBI). To compare the new approach to the Racine score. NEW METHOD: A library of 59 seizure-related behavioral features was generated by annotating 329 seizures in 31 rats with TBI, which were monitored using high-resolution video-electroencephalogram. Of the 329 seizures, 149 were early, 85 post-electrode implantation (6th post-injury month), and 95 late seizures (7th post-injury month). Of the 59 behavioral features, 3 were pre-ictal, 43 ictal, and 13

Signal properties and stability of a chronically implanted endovascular brain computer interface.

BACKGROUND: Implanted brain-computer interfaces (iBCIs) establish direct communication with the brain and hold the potential to enable people with severe disability to achieve control of digital devices, enabling communication and digital activities of daily living. The ability to access brain signals reliably and continuously over many years post-implantation is crucial for iBCIs to be effective and feasible. This study investigates the signal characteristics and long-term stability of neural activity recorded with a stent-electrode array over 1 year post-implant. METHODS: We report on five

Vestibular Response to Electrical Stimulation of the Otolith Organs. Implications in the Development of A Vestibular Implant for the Improvement of the Sensation of Gravitoinertial Accelerations.

OBJECTIVE: Electrical stimulation of the utricular and saccular portions of the vestibular nerve improves stability in patients suffering from vestibular dysfunction. The main objective of this study was to evaluate a new technique, vestibular response telemetry (VRT), for measuring the electrically evoked vestibular compound action potential (saccular and utricular) after stimulating the otolith organ (saccular and utricular) in adults. This study used evidence that the otolith organ can be electrically stimulated in order to develop a new vestibular implant design to improve the sensation of

Cochlear implantation in X-linked deafness - How to manage the surgical challenges.

OBJECTIVE AND IMPORTANCE: In children with X-linked deafness, cochlear malformations challenge the implant surgeon to avoid electrode insertion into the internal auditory meatus and prevent a continuous cerebrospinal fluid (CSF) leak. We describe our experience of cochlear implantation (CI) in two children with profound hearing loss secondary to X-linked deafness, highlighting safer operative techniques to avoid potential complications. CLINICAL PRESENTATION: Descriptive cases of two children with X-linked deafness (patient 1 and patient 2) undergoing CI. Peri-operative imaging and work-up to

Multitarget, dual-electrode deep brain stimulation of the thalamus and subthalamic area for treatment of Holmes' tremor.

OBJECT: Holmes' tremor (HT) is generally considered to be a symptomatic tremor associated with lesions of the cerebellum, midbrain, or thalamus. Deep brain stimulation (DBS) therapy for essential tremor and parkinsonian tremor has proved quite successful. In contrast, surgical treatment outcomes for HT have often been disappointing. The use of 2 ipsilateral DBS electrodes implanted in parallel within the thalamus for severe essential tremor has been reported. Since dual-lead stimulation within a single target can cover a wider area than single-lead stimulation, it produces greater effects. On

Evaluating cochlear insertion trauma and hearing preservation after cochlear implantation (CIPRES): a study protocol for a randomized single-blind controlled trial.

BACKGROUND: In order to preserve residual hearing in patients with sensorineural hearing loss (SNHL) who receive a cochlear implant (CI), insertion trauma to the delicate structures of the cochlea needs to be minimized. The surgical approach comprises the conventional mastoidectomy-posterior tympanotomy (MPT) to arrive at the middle ear, followed by either a cochleostomy (CO) or the round window (RW) approach. Both techniques have their benefits and disadvantages. Another important aspect in structure preservation is the design of the electrode array. Two different designs are used: a "straigh

Evaluation of Spiral Ganglion Lesions by Electrophysiological Measures.

Background: Through the direct electrical stimulation of spiral ganglion neurons (SGNs) of the hearing nerve, cochlear implants overcome functionally impaired or missing hair cells in patients with profound to severe hearing loss. In routine clinical fitting, regions with severe local SGN degeneration (modiolar "dead regions") cannot be identified. As a result, the electrical fields of neighboring electrodes are broadened, which can lead to increased channel interaction and, consequently, poorer speech understanding and hearing. The objective of this study was to ascertain whether neural healt

Micro-Coil Design Influences the Spatial Extent of Responses to Intracortical Magnetic Stimulation.

OBJECTIVE: Electrical stimulation via cortically implanted electrodes has been proposed to treat a wide range of neurological disorders. Effectiveness has been limited, however, in part due to the inability of conventional electrodes to activate specific types of neurons while avoiding other types. Recent demonstrations that magnetic stimulation from a micro-coil can selectively activate pyramidal neurons (PNs) while avoiding passing axons suggest the possibility that such an approach can overcome some this limitation and here we use computer simulations to explore how the micro-coil design in

Selective and graded recruitment of cat hamstring muscles with intrafascicular stimulation.

The muscles of the hamstring group can produce different combinations of hip and knee torque. Thus, the ability to activate the different hamstring muscles selectively is of particular importance in eliciting functional movements such as stance and gait in a person with spinal cord injury. We investigated the ability of intrafascicular stimulation of the muscular branch of the sciatic nerve to recruit the feline hamstring muscles in a selective and graded fashion. A Utah Slanted Electrode Array, consisting of 100 penetrating microelectrodes, was implanted into the muscular branch of the sciati

SEEG-guided RF thermocoagulation of epileptic foci: feasibility, safety, and preliminary results.

PURPOSE: Depth electrodes recordings may be required in some cases of epilepsy surgery to delineate the best region for cortical resection. We usually implant depth electrodes according to Talairach's stereoelectroencephalography (SEEG) method. By using these permanently implanted depth electrodes, we are able to perform radiofrequency (RF)-thermolesions of the epileptic foci. We report the technical data required to perform such multiple cortical thermolesions, as well as preliminary results in terms of seizure outcome in a group of 20 patients. METHODS: Lesions were performed by using 100-

Device profile of the MED-EL cochlear implant system for hearing loss: overview of its safety and efficacy.

INTRODUCTION: Patients suffering from severe to profound hearing loss or even deafness can achieve a hearing improvement with a cochlear implant (CI) treatment that is significantly higher than the results achieved with conventional hearing aids. The CI system consists of an implantable stimulator, which is inserted retro-auricularly into the mastoid, and an externally worn processor unit, which provides the pickup of sound and processing of acoustic information as well as the power supply for the stimulator and internal current sources. The stimulator has an electrode array that is inserted i

Blind axillar venous access.

The axillary vein has been suggested as an alternate site for venous access to avoid the "subclavian crush phenomenon. " Many techniques have been used to access this structure. They include complicated anatomical landmarks, contrast venography, Doppler, and ultrasound. A simple technique using the basic anatomical landmark of the deltopectoral groove and a blind venous stick has been used successfully in 165 of 168 consecutive pacemaker and ICD procedures; there were only three failures. These required an alternate approach. With a thorough knowledge of the regional anatomy, the axillary vein

[Development of an epiretinal prosthesis for stimulation of the human retina].

Degenerations of the outer retina in retinal diseases such as retinitis pigmentosa lead to blindness due to photoreceptor loss. A therapeutic option for visual rehabilitation is presently not available. Over the last few years, a retinal prosthesis has been developed and its use has been tested in animal experiments as well as in humans. With the epiretinal implant images of the environment are taken by a camera, these data are transmitted to an intraocular encoder, and the retina is electrically stimulated by a retinal stimulator placed epiretinally. The stimulation electrodes are placed as f

Auditory brainstem implant (ABI): new frontiers in adults and children.

UNLABELLED: Previous studies have considered only patients with neurofibromatosis type 2 (NF2) older than 12 years as candidates for an auditory brainstem implant (ABI). Our study expands the potential criteria to include both children and adult subjects with other cochlear or cochlear nerve malfunctions who either would not benefit at all from a cochlear implant (eg, cochlear nerve aplasia or avulsion) or whose benefit was or would be severely compromised (eg, cochlear ossification, cochlear fracture). STUDY DESIGN: In our department, over the period from April 1997 to September 2002, 29 pat

Benefit of cochlear implantation in a patient with Myhre syndrome.

Myhre syndrome is a rare disorder characterised by short stature, skeletal anomalies, facial dysmorphism and hearing loss (HL), resulting from heterozygous mutations of the SMAD4 gene. We describe the benefits of cochlear implant (CI) in a patient with sensorineural HL carrying a mutation (NM_005359.6: c.1498A>G; p.lle500Val) within the SMAD4 gene, detected by whole-exome sequencing. The CI was inserted through the round window despite otospongiotic abnormalities. Pure-tone audiometry improved up to 20 dBHL. Speech perception in noise (Simplified Noise Reduction - SNR +10) increased from 0% pr

[Pacemaker lead implant via the persistent left superior vena cava].

BACKGROUND: Persistent left superior vena cava (PLSVC) is a structural, asymptomatic and infrequent anomaly, present in 0.5% of the general population. Typically the diagnosis reveals itself unexpectedly at the time of pacemaker implantation, when it acquires anatomic relevancy. Several techniques are used for the transvenous approach to enter the central venous circulation; the left subclavian vein has become a common access site for electrode implantation and, occasionally, one can find an anomalous venous structure such as a PLSVC. Placement of electrodes through this anomalous venous struc

Cochlear implantation in 3 patients with osteogenesis imperfecta: imaging, surgery and programming issues.

Osteogenesis imperfecta (OI) is a heterogeneous disease of the connective tissue caused by a defective gene that is responsible for the production of collagen type I, leading to defective bone matrix and connective tissue. Hearing loss affects 35-60% of the patients and will progress to deafness in 2-11% of OI patients for whom cochlear implantation may become the only remaining treatment option. Three patients with OI were retrieved from the Nijmegen Cochlear Implant Centre's database. Most of the specific observations in ear surgery on patients with OI, such as brittle scutum, sclerotic thic
