Bancroftian filariasis in Kwale District of Kenya. III. Quantification of the IgE response in selected individuals from an endemic community.

One hundred and sixty-two individuals from a community in Kwale District, Kenya, endemic for bancroftian filariasis, were selected for a study on the IgE response to filarial antigen (prepared from adult Brugia pahangi). Following clinical and parasitological examination, the individuals were grouped into different categories, based on the presence/absence of microfilaraemia, the presence/absence of acute or chronic (hydrocele or elephantiasis) clinical manifestations, and age. The total and filarial-specific IgE responses were evaluated in all individuals, and the responses in the various cat…

Parasitologic and clinical features of bancroftian filariasis in a community in East Sepik Province, Papua New Guinea.

Bancroftian filariasis has been reported in several areas of Papua New Guinea. The epidemiologic features and natural history of Wuchereria … noted in individuals between the ages of 11 and 20 years (30%). Obstructive disease, manifested as elephantiasis and hydroceles, was present in 64 and 79% of 31-40 and greater than or equal to 41-year-olds, respectively. These data suggest that intense transmission of W. bancrofti infection occurs at an early age in this area of East Sepik Province; patent inf …

Women's participation in a disease management intervention for podoconiosis in northern Rwanda: understanding the context of women's lives.

BACKGROUND: Podoconiosis is a Neglected Tropical Disease which leads to foot and leg swelling (lymphoedema) and, ultimately, severe disability. People suffering from podoconiosis often cannot continue their economic or social roles for prolonged periods of time and experience stigmatisation. Research points towards women being more affected by podoconiosis and podoconiosis disease management interventions being less effective for women. Women’s participation in a podoconiosis disease management intervention in northern Rwanda was the focus of this study. The benefits women experienced when tak

Public-private partnership offers a strategic opportunity for lymphatic filariasis elimination in India by 2030.

India carries a significant burden of lymphatic filariasis. Although the national elimination programme has been implemented since the early 2000s, LF remains endemic in 345 districts. The economic and operational demands of large-scale interventions, such as mass drug distribution, morbidity management, and post-MDA surveillance, place a considerable strain on the public health infrastructure. Moreover, the disability, stigma and productivity loss associated with lymphedema impose substantial socio-economic burden. Given the large presence of private and non-profit sectors in India, public-pr

Strengthening lymphatic filariasis elimination programmes with molecular xenomonitoring: critical utility and unmet operational needs.

Molecular xenomonitoring (MX)-the detection of filarial DNA in mosquito vectors-has become an important component of post- mass drug administration surveillance within the Global Programme to Eliminate Lymphatic Filariasis (GPELF). MX offers non-invasive, high-sensitivity detection of residual or recrudescent Wuchereria bancrofti transmission, particularly where human microfilaraemia and antigenemia surveys have limited power. However, the current operational model relies heavily on gravid-trap-based sampling, which captures a physiologically narrow, behaviourally biased subset of mosquitoes,

A delayed diagnosis of lymphatic filariasis in a returned traveller from the Philippines.

A 71-year-old Australian-born man with previous extended travel to the Philippines presented with bilateral lymphoedema, fevers and rigors. Examination of a nocturnal blood film revealed microfilariae of Wuchereria bancrofti, confirming a diagnosis of Bancroftian filariasis. This case highlights the challenges of diagnosing and managing lymphatic filariasis in non-endemic regions.

Global insights can accelerate India's journey towards the elimination of lymphatic filariasis as a public health problem.

India has set an ambitious target to achieve the elimination of lymphatic filariasis (LF) as a public health problem by 2027, 3 years ahead of the global target. Achieving this goal demands accelerated progress and evidence-based strategies. This review synthesises insights from the policies and interventions of 21 countries that achieved LF elimination in the past decade (2015-2024). Key lessons for India include integrated disease management, tailored drug distribution, capacity building, targeted outreach to migrant and never-treated populations and enhanced morbidity management strategies.

Detection of potential transmission foci of lymphatic filariasis using molecular xenomonitoring in Huahine, French Polynesia.

BACKGROUND: In French Polynesia, substantial progress has been achieved in eliminating lymphatic filariasis (LF) caused by Wuchereria bancrofti var. pacifica, a parasite transmitted by the mosquito vector Aedes polynesiensis. However, despite multiple rounds of Mass Drug Administration (MDA), LF transmission persists on some islands, underscoring the need for robust surveillance to evaluate transmission risks and identify potential transmission foci. METHODOLOGY/PRINCIPAL FINDINGS: An extensive entomological survey combined with a Molecular Xenomonitoring (MX) study was conducted on Huahine I

A new guide for basic psychological support for persons affected by neglected tropical diseases: A peer support tool suitable for persons with a diagnosis of leprosy and lymphatic filariasis.

BACKGROUND: People with disabilities due to neglected tropical diseases (NTDs), such as leprosy and lymphatic filariasis (LF), often encounter situations of stigma and discrimination that significantly impact their mental wellbeing. Mental wellbeing services are often not available at the peripheral level in NTD-endemic countries, and there is a need for such services. Basic psychological support for persons with NTDs (BPS-N) from peers is an important potential solution for addressing mental wellbeing problems. As there was no written document advising delivery of such support, NLR India brou

Wuchereria bancrofti Lymphatic Filariasis, Barrancabermeja, Colombia, 2023.

We describe a recent case of lymphatic filariasis in Colombia caused by Wuchereria bancrofti nematodes. Our study combines clinical-epidemiologic findings with phylogenetic data. Resurgence of lymphatic filariasis may be linked to increasing urbanization trends and migration from previously endemic regions. Fieldwork can be a beneficial tool for screening and containing transmission.

Lymphatic filariasis endgame strategies: Using GEOFIL to model mass drug administration and targeted surveillance and treatment strategies in American Samoa.

American Samoa underwent seven rounds of mass drug administration (MDA) for lymphatic filariasis (LF) from 2000-2006, but subsequent surveys found evidence of ongoing transmission. American Samoa has since undergone further rounds of MDA in 2018, 2019, and 2021; however, recent surveys indicate that transmission is still ongoing. GEOFIL, a spatially-explicit agent-based LF model, was used to compare the effectiveness of territory-wide triple-drug MDA (3D-MDA) with targeted surveillance and treatment strategies. Both approaches relied on treatment with ivermectin, diethylcarbamazine, and albend

Implementation and Outcomes of a Pilot Collaborative Surgical Hydrocele Training in Côte d'Ivoire.

Lymphatic filariasis (LF) is a neglected tropical disease that can cause hydrocele and its associated stigma, loss of economic productivity, and depression. Hydrocele surgery is an essential part of LF morbidity management but can be difficult for national programs to implement. To improve access to hydrocele surgeries in Côte d'Ivoire, we provided a WHO-certified surgical training for six surgical teams from five health districts in Côte d'Ivoire. We then evaluated the surgical outcomes and assessed the impact of hydrocele surgery on quality of life of hydrocelectomy patients. Preoperative an

Semiannual Treatment of Albendazole Alone is Efficacious for Treatment of Lymphatic Filariasis: A Randomized Open-label Trial in Cote d'Ivoire.

BACKGROUND: Ivermectin (IVM) plus albendazole (ALB), or IA, is widely used in mass drug administration (MDA) programs that aim to eliminate lymphatic filariasis (LF) in Africa. However, IVM can cause severe adverse events in persons with heavy Loa loa infections that are common in Central Africa. ALB is safe in loiasis, but more information is needed on its efficacy for LF. This study compared the efficacy and safety of 3 years of semiannual treatment with ALB to annual IA in persons with bancroftian filariasis. METHODS: Adults with Wuchereria bancrofti microfilaremia (Mf) were randomized to

Lessons Learned from the Japanese Encephalitis Vaccine Introduction in India That Supported the Introduction of Ivermectin-Diethylcarbamazine-Albendazole for Lymphatic Filariasis Elimination.

We used the introduction of the Japanese encephalitis (JE) vaccine in India as an example to understand more fully the process of introducing any new clinical product in India. We discuss the key decision-making points as well as the many activities involved in introducing a new clinical product in India's public health program. We write from our experience in supporting the government of India to introduce new products successfully-namely, vaccines-to India's health system. In India, the process begins with identifying the public health problem (e.g., an outbreak of JE), deciding to take acti
