[A re-operative case of bentall operation and aortic arch replacement using a stent graft for a Marfan syndrome, post sternum turnover and post mitral valve replacement].

A 36-year-old male with Marfan syndrome succesfully underwent Bentall operation and aortic arch replacement using a stent graft as an elephant trunk. He had received MVR with sternum turn over 14 years previously. Median sternotomy was performed. Under circulatory arrest with rertograde cerebral perfusion we performed Bentall operation and aortic arch replacement using a stent graft. The sternum was cured well. Retractive breathing was not detected. This surgical procedure was effective for cardiovascular disease with Marfan syndrome.

The Secret behind Extreme Hypoxia Tolerance: A "Slow-Growth" Thoracoabdominal Aneurysm.

A 61-year-old man presented to our institution complaining of back pain. Breathing was comfortable. An arterial blood gas showed extreme hypoxia causing chronic respiratory alkalosis. Further investigations revealed aneurysmal dilatation of the ascending aorta and the Crawford Type II thoracoabdominal aneurysm, with compression of both the left main bronchus and the right pulmonary artery. The patient was managed with a two-stage hybrid surgical approach comprising total arch replacement using the frozen elephant trunk technique followed by endovascular repair.

Redo aortic root and arch replacement with 3 different homografts.

A 74-year-old male entered the hospital with a medical history of an aortic arch and a descending thoracic aneurysm. To prevent arm ischaemia after the frozen elephant trunk procedure, a left subclavian artery to left common carotid artery anastomosis was performed. The postoperative period was complicated by sepsissternal infection and mediastinitis. We decided to perform a combined total aortic root and aortic arch replacement with 3 cryopreserved homografts. In addition, the "reverse arch technique" was applied to adapt the distal anastomosis. The operation is associated with a high risk of

Type A Acute Aortic Dissection Presenting With Arch and Abdominal Aortic Aneurysms.

Herein, we present the case of an 89-year-old female who presented with acute aortic dissection involving the arch and abdominal aneurysms. Emergent total arch replacement with a frozen elephant trunk was the first-line approach taken to save the patient's life. Although prolonged mechanical ventilation necessitated a tracheostomy, subsequent endovascular aortic repair mitigated the risk of rupture in the abdominal aortic aneurysm. While managing acute aortic syndrome with multiple aneurysms poses a challenge for surgeons, a diagnosis based on computed tomography angiography and timely interve

Hybrid repair for Kommerell's diverticulum and right aortic arch with aberrant right vertebral artery.

Kommerell's diverticulum (KD) is a rare aneurysm of the origin of an aberrant subclavian artery. Hybrid aortic arch repair for KD is being performed more often. We report hybrid arch repair for KD in a 63-year-old man with a right aortic arch and aberrant right vertebral artery, an extremely rare variant. We performed total arch replacement to completely reconstruct the five cervical arteries with elephant trunk to create an adequate landing zone, followed by second-stage endovascular stent-grafting from the ascending aorta to the proximal descending aorta.

An aorto-oesophageal fistula treated with total arch repair combined with oesophageal repair.

Aorto-oesophageal fistula is a rare but fatal disease. Open surgical repair or endotube repair of aorta and oesophagus save patients from circulatory deterioration, major haematemesis and severe infection. Here we present a rare case of an aorto-oesophageal fistula in the right hemi-arch caused by a fish bone treated with an alternative surgical strategy. Total aortic arch repair using the frozen elephant trunk procedure combined with oesophageal repair were performed. No aortic fistula, oesophageal fistula or artificial graft infection occurred in the follow-up.

Complete closure of coronary fistula communicating with the descending aorta.

We report a rare case of complex coronary fistula that contained two aneurysms and was connected to the descending aorta. The fistula was also associated with further aortic pathologies necessitating surgery. Preoperative computed tomography angiography, which revealed an abnormal vessel communication, was crucial for choosing the appropriate operative strategy. During a one-stage procedure comprising closure of both arterial (aortic and coronary) fistula origins and repair of the aortic valve and thoracic aorta, the coronary origin of the fistula was ligated and the frozen elephant trunk tech

Surgery for retrograde dissection and endoleak repair following endovascular repair of type B aortic dissection.

This video tutorial presents a case of repair of a delayed-onset retrograde type A dissection after TEVAR with a left carotid-subclavian bypass for acute type B dissection. One of our key challenges was problematic brain and spinal cord protection in a frail patient with distal dissection, and dissection involving an ectasic brachiocephalic trunk extending into the right carotid artery with a significantly increased risk of stroke and paraplegia.  We demonstrate the repair of the dissection using the frozen elephant trunk technique and we also present sequential brain and visceral perfusion di

Successful management of a giant dissecting aortic aneurysm in a patient with Behcet's disease.

Thoracic aortic aneurysm with subsequent dissection is a very rare but dangerous complication of Behcet's disease, which is a systemic syndrome that can involve blood vessels of all sizes. Surgical experience in dealing with this complication in patients with Behcet's disease has been limited. We report a 20-year-old patient with Behcet's disease who presented with a giant dissecting thoracic aortic aneurysm. The patient was successfully treated with an aortic root and arch replacement using a frozen elephant trunk technique. Timely surgical intervention and appropriate long-term medical manag

Management of a right aberrant subclavian artery during complex hybrid stent-graft procedures: a rare and complex issue.

A 76-year-old man with an ascending arch and proximal descending aortic aneurysm underwent a complex aortic replacement through a sternotomy with ligation of a right aberrant subclavian artery (RASA) distal to the right vertebral artery. The second-stage procedure was performed with a stent-graft deployed within the elephant trunk. At 6- and 12-month follow-up, the RASA was opacified by the patent right vertebral artery. Under ultrasound guidance, the patient's RASA stump was occluded by coils. Management of an RASA during complex hybrid stent-graft procedures is discussed.

Neuropathology, novel tissue tropism, and evidence of vertical transmission in South American pinnipeds naturally infected with high pathogenicity avian influenza H5N1 virus in Argentina.

In 2023, high pathogenicity avian influenza (HPAI) H5N1 outbreaks caused mass mortality events affecting pinnipeds throughout South America. Here, we present the clinical, pathological, immunohistochemical, and molecular findings associated with HPAI H5N1 infection in 3 South American sea lions (Otaria flavescens) and 1 southern elephant seal (Mirounga leonina) stranded during the outbreak in Chubut, Argentina. Neurological signs were observed in 2 sea lions, with 1 also exhibiting respiratory distress. Necropsies were conducted on all 4 animals. Neuropathology revealed mild-to-severe lymphohi

Gender-related presentation, surgical treatment, outcome and failure to rescue after surgery for type A aortic dissection: results from a multicentre registry.

OBJECTIVES: Reports on gender-related differences in perioperative characteristics and the outcome after surgery for type A acute aortic dissection are contradictory. METHODS: Perioperative characteristics, outcome and overall or itemized failure-to-rescue rates were collected retrospectively and dichotomized by gender in patients operated on at 5 referral institutions. A propensity score matched analysis was performed to compared males and females with similar preoperative risk profiles. Multivariable analysis assessed gender-related predictors of 30-day mortality. RESULTS: A total of 1271

Hybrid Repair of Kommerell Diverticulum and Aberrant Subclavian Artery with Compressive Symptoms and a New Strategy: Case Report.

Surgical indication and treatment for patients with Kommerell diverticulum and aberrant subclavian artery are still not well established. A patient with esophageal and tracheal compression resulting from these anatomical abnormalities was successfully treated with a hybrid approach of total arch replacement, frozen elephant trunk technique, aberrant left subclavian artery transection, and left subclavian artery reconstruction through median sternotomy. Compressive symptoms were relieved without resecting the enlarged diverticulum. In this case, the importance of preoperative investigation for

Successful Treatment of Staphylococcus schleiferi Infection after Aortic Arch Repair: In Situ Aortic Arch Replacement and Domino Reconstruction of the Debranching Graft using Autologous Iliac Artery.

A 62-year-old Japanese male presented with graft infection by Staphylococcus schleiferi 50 days after debranching of the left subclavian artery and frozen elephant trunk repair for the entry closure of a Stanford type B aortic dissection. The graft was removed, and the patient was successfully treated using in situ reconstruction of the arch with omental flap coverage, removal of the debranching graft, autologous iliac artery grafting, and longterm antibiotics. Domino reconstruction of the infected debranching graft using autologous external iliac artery and a Dacron graft can thus be a good o

Management of a chronic Stanford type B dissection in a patient with a right-sided aortic arch.

We report the case of a 44-year-old male with a right-sided aortic arch who presented with dissection extending from his distal arch to his left common iliac artery. He was initially managed conservatively, but later became symptomatic with back pain; enlargement of the descending thoracic aorta was demonstrated on computed tomography scanning. He underwent staged repair of the aortic arch and descending thoracic aorta with the elephant trunk procedure - two operations one year apart, each time making a good postoperative recovery.

Evaluation of remodeling of the descending aorta using 3DCT after total arch replacement for acute aortic dissection.

OBJECTIVES: The frozen elephant trunk (FET) technique during total arch replacement (TAR) in patients with acute type A aortic dissection (ATAAD) has been shown to promote favorable aortic remodeling. However, few reports have compared the morphological assessments between the conventional elephant trunk (CET) and FET procedures. Here, we investigated whether the FET technique during TAR for the patients with ATAAD could affect aortic remodeling through morphological analysis. METHODS: In total, 464 patients diagnosed with ATAAD underwent emergency ascending aorta or TAR between 2010 and 2023

COVID-19 (Novel Coronavirus 2019) - recent trends.

Understanding how air-breathing diving animals moderate their dive behaviour when foraging successfully is foundational in the study of their foraging ecology. Yet, this fundamental relationship remains unresolved with previous research pointing to inconsistent relationships, differing nominally according to sex, habitat type and scale. Empirically testing the relationships between dive effort responses and foraging success is further hampered because of challenges obtaining concurrent measures of behavioural responses and foraging success at sea. We compiled a multi-decadal dive dataset from
