Perilymphatic pressure measurement in Meniere's disease.

The aim of this study was to evaluate the perilymphatic pressure, by means of the MMS-10 Tympanic Displacement Analyzer (TDA), in patients w … ressure of the inner ear is not the physiological basis for the clinical triad of MD; or 3) perhaps endolymphatic hydrops does not produce changes in perilymphatic pressure. The results of this series indicate that the TDA is not useful in the evaluation of patients with MD.

[Electrocochleography and phase audiometry in diagnosis of Meniere disease].

The diagnosis of Menière's disease is uncertain when the typical symptoms do not occur completely and definitely. A reliable finding of an endolymphatic hydrops (EH) is the base for a correct prognosis and therapy. Electrocochleography is a proven diagnostic procedure but requires a lot of time and of technical know-how. The mobility of cochlear partition can be tested by low-frequency masking (LFM) recording the phase dependent subjective masked threshold of a short test tone. We performed electrocochleography and LFM in 29 patients with suspected Menière's disease at the same day. Both tests…

[Acute vertigo caused by cerebellar vascular accident].

At the beginning, small cerebellar strokes may present only with acute onset of vertigo, unsteadiness and unidirectional nystagmus, like a vestibular neuritis. In some cases, it is associated with tinnitus and hearing disturbance, like an endolymphatic hydrops. Other cases may mimic a benign cupulolithiasis, with only a paroxysmal positioning vertigo. Attention should be focused on transient associated symptoms: headache and blurred vision. One should not wait for classical cerebellar clinical signs: they are subtle and they appear late. Within a few days, the clinical picture will change: ver…

[Stapes decompression].

Footplate decompression, which has been described in Lyon in 1965, can be easily performed under local anesthesia. The surgical procedure consists of a 8/10 mm diameter stapedotomy followed by an obturation with spongel, gelfoam or a piece of thin fascia. The operation is not a labyrinthectomy or a sacculotomy but aims to decompress endolymphatic hydrops in Ménière's disease. It must be realised only in case of typical unilateral Ménière's disease with disabling vertigo and very poor residual hearing. More than two hundred cases have been operated on. Vertigo has been relieved in 90% of the op…

Delayed post gadolinium MRI descriptors for Meniere's disease: a systematic review and meta-analysis.

OBJECTIVES: Delayed post-gadolinium magnetic resonance imaging (MRI) detects changes of endolymphatic hydrops (EH) within the inner ear in Meniere's disease (MD). A systematic review with meta-analysis was conducted to summarise the diagnostic performance of MRI descriptors across the range of MD clinical classifications. MATERIALS AND METHODS: Case-controlled studies documenting the diagnostic performance of MRI descriptors in distinguishing MD ears from asymptomatic ears or ears with other audio-vestibular conditions were identified (MEDLINE, EMBASE, Web of Science, Scopus databases: update…

Intraoperative Electrocochleography in Subjects Affected by Vestibular Schwannoma and Ménière's Disease: Comparison of Results.

OBJECTIVES: Histologic reports of temporal bones of ears with vestibular schwannomas (VSs) have indicated findings of endolymphatic hydrops (ELH) in some cases. The main goal of this investigation was to test ears with VSs to determine if they exhibit electrophysiological characteristics similar to those of ears expected to experience ELH. DESIGN: Fifty-three subjects with surgically confirmed VS aged ≥18 and with normal middle ear status were included in this study. In addition, a second group of adult subjects (n = 24) undergoing labyrinthectomy (n = 6) or endolymphatic sac decompression an…

Electrocochleography in patients with Meniere's disease.

OBJECTIVE: The objective of this study was to evaluate the role of electrocochleography (ECoG) in the diagnosis of Meniere's disease. STUDY DESIGN: This study is a retrospective case review. METHODS: Patients undergoing ECoG for Meniere's disease between 1995 and 2003 were identified and segregated as having definite , probable , or possible Meniere's disease according to the 1995 American Academy of Otolaryngology-Head and Neck Surgery guidelines for Meniere's disease classification. Those determined to have probable and possible Meniere's disease were then combined to form a less-than-defi

The intravestibular source of the vestibular aqueduct. II: its structure and function clarified by a developmental study of the intra-skeletal channels of the otic capsule.

CONCLUSION: A developmental histologic study of the otic capsule indicates that it grows a system of lamellar bone with abundant interconnecting intraosseous channels. These include the 'cartilage canals' in the cartilage model, the chondro-osseous and Haversian-like (Volkmann's) canals in the ossified otic capsule, the fissula ante fenestram, which seems to function as a lifelong manufacturer of the latter two channels, and the inner layer (vestibular arch) of the vestibular aqueduct, which is a complex series of Volkmann's canals and microcanals. Chemical changes, possibly produced by breakd

The relationship between nutrition and Ménière's disease.

Ménière's disease is an inner ear disease with attacks characterized by ear fullness, tinnitus, fluctuant sensorineural hearing loss and vertigo. Although pathophysiology of the disease is not fully known, endolymphatic hydrops are believed to play a role. Although there is no certain treatment procedure for Ménière's disease, some treatments are applied to prevent attacks, to treat the symptoms that occur during the attacks, and to prevent the permanent effects of the symptoms on the hearing and balance system. Lifestyle changes, dietary modifications, diuretics, vasodilator corticosteroids, …

Endolymph magnetic resonance imaging: Contribution of saccule and utricle analysis in the management of patients with sensorineural ear disorders.

Endolymphatic hydrops features excess endolymph in the membranous labyrinth, and is a marker of Menière's disease. Between the early 1980s and late 2000s, MRI in Menière's disease aimed purely to rule out tumor or malformation as differential diagnoses for the pressure disorder. Progress in high-resolution MRI, however, now enables excess endolymph to be visualized in the membranous labyrinth, differentiating saccule and utricle in Menière's disease and in other clinical presentations such as cochleovestibular schwannoma. More recently, non-visibility of the saccule was demonstrated in a subgr…

MR imaging of Ménière's disease after combined intratympanic and intravenous injection of gadolinium using HYDROPS2.

Endolymphatic hydrops (EH) in Ménière's disease is currently evaluated by 3-dimensional (3D)-real inversion recovery (IR) sequence after intratympanic (IT) administration of gadolinium (Gd) or by heavily T2-weighted (hT2W)-3D-fluid-attenuated IR (FLAIR) sequence after intravenous (IV) injection of Gd. Unilateral IT injection is usually performed. We employed a method in which we simultaneously administered contrast into one ear intravenously (IV side) and into the other ear both intravenously and intratympanically (IT + IV side) to evaluate EH in 10 patients with Ménière's disease. We then com…

The effects of high stimulus rate on the electrocochleogram in normal-hearing subjects.

The use of high stimulus rates has the potential to improve the electrocochleogram's (ECochG) sensitivity and specificity in endolymphatic hydrops and Menière's disease, but is currently hindered by the absence of an acceptable normative database. In response, this study recorded click-evoked ECochG tracings from 51 normal-hearing subjects (102 ears), between 18 and 60 years of age, at 7.1, 51.1, 101.1 and 151.1 clicks/s using a tympanic membrane electrode. As stimulus rate increased, various statistically significant (p < 0.05) changes were observed. In general, summating potential (SP) laten…

Immune-mediated sensorineural hearing loss.

A review is given on the way our knowledge of pathways of immune responses inside and in the immediate vicinity of the inner ear has gradually developed over the past two decades. Immune reactivity plays a more important role in the etiopathogenesis and natural course of various inner ear disorders than was thought originally. They comprise certain forms of fluctuating or rapidly progressive sensorineural hearing loss (SNHL) with or without endolymphatic hydrops. Patients may present themselves clinically with symptoms resembling Ménière's disease or even with sudden deafness. Immune-mediated …
