Temporal lobe disconnection in drug-resistant epilepsy associated with hippocampal sclerosis: how I do it.

BACKGROUND: Epilepsy surgery is the treatment of choice for drug-resistant epilepsy associated with hippocampal sclerosis. Patients over 50 years of age have an increased risk of postoperative complications after epilepsy surgery. METHODS: The authors present an operative video demonstrating right temporal lobe disconnection in a case of hippocampal sclerosis, along with an anatomical introduction and a section providing strategies to minimize complications. RESULTS: In selected cases, temporal lobe disconnection represents a viable alternative to resective procedures.

Does classical semiology ("5As") predict post-operative seizure outcome in patients with temporal lobe epilepsy?

OBJECTIVE: Temporal lobe epilepsy (TLE) accounts for 50%-70% of cases referred for epilepsy surgery. This study was designed to evaluate the distribution of the five hallmark seizure features-antecedent, aura, arrest, automatisms, and amnesia-considered collectively as the "5As," in individuals with temporal lobe epilepsy. We also analyzed the prognostic significance of 5As, if any, in predicting surgical outcome in drug-resistant TLE patients. METHODS: We included 684 patients who underwent clinical evaluation, neuropsychological assessment, high-resolution MRI, and VEEG monitoring followed

Cortical electrical stimulation modulates hippocampal electrophysiological activity in mouse brain slices of temporal lobe epilepsy.

To investigate structural and functional alterations in the prefrontal cortex-hippocampus (PFC-HPC) circuit in temporal lobe epilepsy (TLE) and the regulatory effects of cortical electrical stimulation on hippocampal activity. A kainic acid (KA)-induced TLE mouse model was used. Diffusion tensor imaging (DTI) analyzed PFC-HPC structural connectivity. Optimized PFC-HPC brain slices were prepared for electrophysiological recording via high-density microelectrode arrays (HD-MEAs), and cortical electrical stimulation at different intensities was applied. DTI revealed abnormal diffusion metrics fra

Distinct Theta-gamma network reorganization differentiates hippocampal sclerosis and MRI-negative mesial temporal lobe epilepsy.

INTRODUCTION: Temporal lobe epilepsy (TLE), particularly mesial temporal lobe epilepsy (MTLE), often presents with visual working memory (VWM) impairments, with potential heterogeneity between hippocampal sclerosis (HS) and MRI-negative subtypes. However, task-related electrophysiological (EEG) evidence regarding brain network alterations during VWM processing in MTLE remains limited. This study aims to identify shared brain network alterations and their behavioral correlates in patients with MTLE and to further characterize subtype-specific differences between HS-MTLE and MRI negative-MTLE.

Film recall reveals intact event memory but impaired sequence memory in temporal lobe epilepsy patients.

BACKGROUND AND OBJECTIVES: Patients with epilepsy (PWE), especially temporal lobe epilepsy (TLE), experience impaired memory for personally experienced events. However, current assessments of episodic memory are limited in their ecological validity with a potential to miss detection of subtle cognitive decline. We conducted an exploratory study to determine whether a naturalistic film-viewing task with open-ended spoken recall could detect memory differences between TLE patients and healthy controls (HCs). METHODS: TLE patients (ages 18-60, fluent in English, not legally blind) were recruited

Vagal afferents gate cortical excitability in a kainate-induced mouse model of temporal lobe epilepsy.

BACKGROUND: Vagus nerve stimulation (VNS) is an established therapy for refractory epilepsy, indicating that enhanced vagal activity can suppress excessive brain excitability. However, whether endogenous vagal tone itself serves as an intrinsic antiepileptic mechanism remains unclear. We aimed to determine if endogenous vagal activity restrains epileptic dynamics and to elucidate the neural pathways involved. METHODS: We employed an intrahippocampal kainate-induced mouse model of temporal lobe epilepsy and an optogenetic model of hippocampal excitability. We performed gain-of-function studies

Urinary Metabolic Profiling During Epileptogenesis in Rat Model of Lithium-Pilocarpine-Induced Temporal Lobe Epilepsy.

Background/Objectives: Temporal lobe epilepsy (TLE) often develops following an initial brain injury, where specific triggers lead to epileptogenesis-a process transforming a healthy brain into one prone to spontaneous, recurrent seizures. Although electroencephalography (EEG) remains the primary diagnostic tool for epilepsy, it cannot predict the risk of epilepsy after brain injury. This limitation highlights the need for biomarkers, particularly those measurable in peripheral samples, to assess epilepsy risk. This study investigated urinary metabolites in a rat model of TLE to identify bioma

Abnormal functional connectivity of paracingulate gyrus in patients with temporal lobe epilepsy-comorbid sleep disorders.

Abnormalities in paracingulate gyrus (PCG) were found in both patients with temporal lobe epilepsy and patients with sleep disorders. These abnormalities include reduced functional connectivity between PCG and other brain regions, abnormal activity and reduced volume of PCG. PCG may be associated with comorbid sleep disorders in patients with temporal lobe epilepsy (TLE). This study aimed to explore the relationship between abnormal PCG function and comorbid sleep disorders in patients with TLE. We used Pittsburgh Sleep Quality Index -8/9 to divide fifty-eight patients into a group with sleep

Brain Volume Loss After Stereotactic Laser Interstitial Thermal Therapy in Patients With Temporal Lobe Epilepsy.

BACKGROUND AND PURPOSE: Temporal lobe epilepsy is the most common form of focal epilepsy. MR-guided laser interstitial thermal therapy (LITT) of the amygdalohippocampal complex has become an established therapy option in case of drug resistance. Long-term anatomic network effects on the brain due to deafferentiation have not yet been evaluated. METHODS: We analyzed brain volumes of 11 patients with temporal lobe epilepsy before and 1-year after hippocampal LITT with FastSurfer segmenting T1-weighted data. Additionally, we performed visual ratings and measurements. RESULTS: A total of 11 pati

Resting-State Network Transitions in Temporal Lobe Epilepsy: Insights From MEG-Based Dynamic Functional Connectivity.

Temporal Lobe Epilepsy (TLE), a common form of focal epilepsy, is associated with recurrent seizures originating in the temporal lobe, often leading to cognitive and psychological impairments. This study explores dynamic functional connectivity (dFC) patterns in TLE patients compared to Healthy Controls (HC) using resting-state Magnetoencephalography (MEG) data. dFC, which captures the temporal variability of brain networks, was analyzed across eight frequency bands (delta, theta, alpha, beta, low gamma, mid gamma, high gamma, and broadband) in 21 TLE patients and 21 HC. Nine dFC metrics, incl

Excitation-inhibition imbalance as a common thread linking early Alzheimer's disease with temporal lobe epilepsy.

Growing evidence suggests that network hyperexcitability is a pivotal yet under-recognized pathology linking early Alzheimer's disease (AD) with mesial Temporal Lobe Epilepsy (mTLE). This narrative review synthesises pre-clinical and clinical data showing how disruption of excitation-inhibition balance, driven chiefly by the loss or dysfunction of parvalbumin- and somatostatin-positive GABAergic interneurons (INs), emerges early in AD and fosters subclinical epileptiform activity that hastens cognitive decline. We integrate findings that degeneration of Ventral Tegmental Area dopaminergic proj

Deficits in facial emotion recognition in temporal lobe epilepsy: Comprehensive understanding from cognitive neuroscience perspectives.

Facial emotion recognition (FER) is a key aspect of social cognition, crucial for social interactions. Recent studies have focused on deficits in FER among patients with temporal lobe epilepsy (TLE) and the factors influencing these deficits. Research suggests that brain damage caused by epilepsy may lead to specific impairments in recognizing emotions, with lateralization of epilepsy and the emotional intensity of stimuli playing important roles. Additionally, interactions between emotion type, emotional intensity, and the age of onset of epilepsy may contribute to these recognition deficits.

Dynamic and Static Functional Gradient in Temporal Lobe Epilepsy With Hippocampal Sclerosis Versus Healthy Controls.

AIMS: The gradient captures the continuous transitions in connectivity, representing an intrinsic hierarchical architecture of the brain. Previous works hinted at the dynamics of the gradient but did not verify them. Cognitive impairment is a common comorbidity of temporal lobe epilepsy (TLE). Gradient techniques provide a framework that could promote the understanding of the neural correlations of cognitive decline. METHODS: Thirty patients with TLE and hippocampal sclerosis and 29 matched healthy controls (HC) were investigated with verbal fluency task-based functional MRI and gradient tech

Relationship of left piriform cortex network centrality with temporal lobe epilepsy duration and drug resistance.

BACKGROUND: We investigated the relationship of piriform cortex (PC) structural network centrality with drug resistance and epilepsy duration as markers of sustained epileptic activity. METHODS: PCs were manually delineated on retrospectively collected 3D-T1-MRI images of patients with temporal lobe epilepsy (TLE). Connectomes were computed from diffusion MRI scans, including the PC as network nodes. Betweenness centrality (BC) and node degree were computed and compared across drug-resistant versus drug-sensitive patients. Correlations of centrality metrics with the duration of epilepsy were

Increased Dentate Gyrus Excitability in the Intrahippocampal Kainic Acid Mouse Model for Temporal Lobe Epilepsy.

The intrahippocampal kainic acid (IHKA) mouse model is an extensively used in vivo model to investigate the pathophysiology of mesial temporal lobe epilepsy (mTLE) and to develop novel therapies for drug-resistant epilepsy. It is characterized by profound hippocampal sclerosis and spontaneously occurring seizures with a major role for the injected damaged hippocampus, but little is known about the excitability of specific subregions. The purpose of this study was to electrophysiologically characterize the excitability of hippocampal subregions in the chronic phase of the induced epilepsy in th

Identification of hub genes significantly linked to temporal lobe epilepsy and apoptosis via bioinformatics analysis.

BACKGROUND: Epilepsy stands as an intricate disorder of the central nervous system, subject to the influence of diverse risk factors and a significant genetic predisposition. Within the pathogenesis of temporal lobe epilepsy (TLE), the apoptosis of neurons and glial cells in the brain assumes pivotal importance. The identification of differentially expressed apoptosis-related genes (DEARGs) emerges as a critical imperative, providing essential guidance for informed treatment decisions. METHODS: We obtained datasets related to epilepsy, specifically GSE168375 and GSE186334. Utilizing different
