Clinical, procedural and patient-reported outcomes of the Ultimaster Nagomi sirolimus-eluting coronary stent in complex percutaneous coronary intervention: the NAGOMI COMPLEX prospective cohort study protocol.

INTRODUCTION: Percutaneous coronary intervention (PCI) has evolved significantly over the past decade with approximately 30% of patients presenting with complex coronary artery disease, posing heightened procedural and long-term risks. Complexity in PCI is increasingly recognised as multifactorial, encompassing lesion, procedural and patient-related characteristics. The Ultimaster Nagomi sirolimus-eluting coronary stent system is designed to address the challenges of complex PCI. The NAGOMI COMPLEX study aims to assess its safety and clinical performance in a comprehensively defined complex PC

Suspended will: The ethical journey of paediatric stem cell donors.

BackgroundIn allogeneic haematopoietic stem cell transplantation, paediatric donors may have limited involvement in donation decisions, as their psychological experiences and ethical needs may receive less attention when prioritising the recipient's survival.ObjectiveThis study explored paediatric donors' experiences of autonomy, competence, and relatedness throughout the donation process, using Self-Determination Theory (SDT) as a sensitising interpretive lens to examine implications for ethically sensitive nursing practice.DesignInterpretative phenomenological analysis (IPA).Participants and

Psychological and pharmacological treatments of intermittent explosive disorder: a meta-analysis protocol.

INTRODUCTION: Intermittent explosive disorder (IED) is characterised by recurrent, sudden episodes of impulsive aggression that are disproportionate to the provocation. The condition's management remains challenging due to the variability in treatment efficacy and the absence of Food and Drug Administration-approved interventions specifically for IED. This meta-analysis aims to evaluate the effectiveness of existing treatments for IED. METHODS AND ANALYSIS: Adhering to Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines, a comprehensive literature search was conducte

Developing the supportive care bundle for mothers of infants hospitalised in the neonatal intensive care unit: protocol for a multimethod study.

INTRODUCTION: Neonatal admission to the neonatal intensive care unit (NICU) represents an acute crisis for the family, particularly for the mother. During this period, mothers face a broad spectrum of care-related, emotional, informational and supportive needs; when these needs are not adequately addressed, serious negative consequences for maternal mental health and the mother–infant bond may ensue. Despite the significance of this issue, a comprehensive and culturally adapted care bundle for this population remains notably absent within Iran's healthcare system. The present study aims to dev

Remote symptom assessment and management via mobile app for adults with chronic kidney disease in Vietnam (SAM-CKD programme): a randomised feasibility trial protocol.

INTRODUCTION: Adults with chronic kidney disease (CKD) experience a wide range of symptoms that significantly lower their health-related quality of life (HRQoL). Using mobile-based applications for symptom assessment and management has the potential to alleviate the symptom burden of CKD and improve patient outcomes. METHODS AND ANALYSIS: This is a randomised feasibility trial to assess the feasibility, acceptability, usability and potential effects of a remote symptom assessment and management (SAM-CKD) 6 week programme delivered via a mobile application. Adults aged 18 years or older with C

Efficacy and safety of glucocorticoids for acute drug-induced liver injury with hyperbilirubinemia: protocol of a multicenter randomized controlled trial.

BACKGROUND: Drug-induced liver injury (DILI) can lead to potentially fatal complications, such as acute liver failure and even death. In clinical practice, glucocorticoids have been considered in some cases of DILI, especially those with hyperbilirubinemia. However, the available evidence remains controversial and its quality is also very limited. OBJECTIVES: To explore the efficacy and safety of glucocorticoids in patients with acute DILI and hyperbilirubinemia. DESIGN: An open-label, multicenter, randomized controlled trial (RCT). METHODS AND ANALYSIS: Overall, 232 patients with acute DIL

Paediatric Resuscitation Outcome in Children with Heart Disease (ProCHD): protocol of a Germany-wide multicentre, prospective open registry.

INTRODUCTION: While paediatric cardiac arrest is a rare event, consequences for the patients are significant with a considerable risk of morbidity, disability and mortality. The risk of cardiac arrest is substantially increased in children with congenital heart disease. Nevertheless, there is a lack of data concerning this population. To close this knowledge gap, this multicentre, prospective, open registry aims to implement a standardised structure for data collection and follow-up of paediatric cardiac arrests associated with heart diseases in Germany. METHODS AND ANALYSIS: All paediatric p

Comparison of diagnostic yield and safety of three endobronchial ultrasound-guided transbronchial biopsy techniques in diagnosing patients with mediastinal/hilar lymphadenopathy: a protocol of multicentre randomised trial in China.

INTRODUCTION: Mediastinal and/or hilar lymphadenopathy (MHL) is increasingly identified owing to various underlying conditions. Minimally invasive biopsy techniques, including endobronchial ultrasound-guided transbronchial needle aspiration (EBUS-TBNA), transbronchial mediastinal cryobiopsy (TBMC) and transbronchial forceps biopsy (TBFB), are common diagnosis tools. However, their safety and diagnostic efficiency remain unclear. This trial aims to compare the diagnostic yield and safety of these three techniques. METHODS AND ANALYSIS: This study is a three-arm, parallel-design, randomised con

Effect of preoperative inspiratory muscle training combined with preoperative education on postoperative pulmonary complications in high-risk patients with lung cancer: protocol for a randomised controlled trial.

INTRODUCTION: Preoperative inspiratory muscle training (IMT) is recognised as an important component of the preoperative management of lung cancer, although there is limited evidence for the delivery of a home-based IMT programme combined with preoperative education. We developed a programme combining short-term home-based IMT and preoperative physiotherapy education ('the programme'). This study aims to evaluate the effectiveness of this programme in reducing postoperative pulmonary complications (PPCs) after lung cancer resection compared with standard care. METHODS AND ANALYSIS: In this ra

VaNoLaH trial: a study protocol-a multinational randomised controlled trial including two identical substudies comparing vaginal versus vNOTES (vaginal natural orifice transluminal surgery) hysterectomy or laparoscopic versus vNOTES hysterectomy.

INTRODUCTION: Hysterectomy is one of the most common surgeries performed in women. Minimally invasive methods are on the rise globally as they have been shown to decrease surgical morbidity compared with abdominal hysterectomy. Hysterectomy by vaginal natural orifice transluminal endoscopic surgery (vNOTES) is the latest innovation. It combines the vaginal approach and endoscopy via the vagina. Large pragmatic randomised controlled trials (RCTs) are lacking comparing outcomes after vNOTES, vaginal hysterectomy (VH) and laparoscopic hysterectomy (LH). METHODS: Multicentre pragmatic RCT aiming

Comparing blastocyst euploid rates between the progestin-primed and gonadotrophin-releasing hormone antagonist protocols in aneuploidy genetic testing: a randomised trial protocol.

INTRODUCTION: Progestin can inhibit the pituitary luteinising hormone (LH) surge during ovarian stimulation for in vitro fertilisation (IVF) and studies show progestin-primed ovarian stimulation (PPOS) is effective in blocking the LH surge in IVF. More and more centres are using PPOS because this regimen appears simpler and cheaper. This study aims to compare the euploidy rate of blastocysts following the PPOS protocol and the gonadotropin-releasing hormone antagonist protocol in women undergoing preimplantation genetic testing for aneuploidy (PGT-A). METHODS/ANALYSIS: This is a randomised tr

Harnessing Simple Animal Models to Decode Sleep Mysteries.

Whether it involves human subjects or non-human animals, basic, translational, or clinical sleep research poses significant ethical challenges for researchers and ethical committees alike. Sleep research greatly benefits from using diverse animal models, each offering unique insights into sleep control mechanisms. The fruit fly (Drosophila melanogaster) is a superior genetic model due to its quick generation period, large progenies, and rich genetic tools. Its well-characterized genome and ability to respond to hypnotics and stimulants make it an effective tool for studying sleep genetics and

Effects of low-dose intravenous heparin therapy in aneurysmal subarachnoid hemorrhage: a randomized controlled clinical trial protocol.

BACKGROUND: Heparin anticoagulation therapy is a widely used method to prevent cerebral vasospasm (CV) and venous thrombosis in patients with subarachnoid hemorrhage caused by ruptured cerebral aneurysms. Subcutaneous heparin injection is considered safe and effective, whereas continuous intravenous heparin infusion is still being debated due to the risk of bleeding. Although most retrospective studies have confirmed the safety and effectiveness of unfractionated heparin (UFH) after aneurysm embolization therapy and its ability to reduce CV, there is still no randomized clinical trial comparin
