The clinical differentiation of bacterial and fungal keratitis: a photographic survey.

PURPOSE: The purpose of this study was to determine whether clinical signs of infectious keratitis can be used to identify the causative organism. METHODS: Eighty photographs of eyes with culture-proven bacterial keratitis or smear-proven fungal keratitis were randomly selected from 2 clinical trials. Fifteen cornea specialists from the F. I. Proctor Foundation and the Aravind Eye Care System assessed the photographs for prespecified clinical signs of keratitis, and they identified the most likely causative organism. RESULTS: Clinicians were able to correctly distinguish bacterial from funga

Epidemiology and control of gonococcal ophthalmia neonatorum.

From a public health point of view gonococcal ophthalmia neonatorum (GCON) is important as it can rapidly lead to blindness. The frequency of GCON is determined by the prevalence of maternal gonococcal infection. In most industrialized countries the prevalence of gonorrhoea in pregnant women is less than 1%; in developing countries the rates are between 3% and 15%, more than 50% being due to penicillinase-producing Neisseria gonorrhoeae strains (PPNG). The rate of transmission from mother to newborn is between 30% and 50%. Strategies for the control of GCON include: (1) prevention of gonococca

Microbial keratitis in keratoglobus-associated vernal keratoconjunctivitis and atopic dermatitis.

PURPOSE: The authors describe a patient with keratoglobus and a history of vernal keratoconjunctivitis and atopic dermatitis who acutely developed corneal hydrops and severe microbial keratitis. The infectious keratitis responded poorly to medical management and resulted in enucleation of the eye. METHODS: A 25-year-old man presented with an acutely painful, red left eye. He had an ocular history of keratoglobus in association with vernal keratoconjunctivitis and atopic dermatitis. His visual acuity was light perception in the left eye and 20/40 in the right eye. Ocular examination showed a g

Posterior scleritis secondary to Pseudomonas aeruginosa keratitis.

PURPOSE: To report a case of Pseudomonas aeruginosa keratitis that progressed to posterior scleritis. METHODS: Observational case report. RESULTS: A 64-year-old woman was treated for P. aeruginosa keratitis. Twelve days later, she was diagnosed with posterior scleritis and treated with oral ciprofloxacin. Her condition progressed, ultimately resulting in enucleation. Histopathology confirmed posterior scleritis, a previously unreported complication of P. aeruginosa keratitis. CONCLUSION: P. aeruginosa keratitis may progress to posterior scleritis, a potentially devastating complication.

Six cases of scleritis associated with systemic infection.

Isolated scleritis (without keratitis) associated with infections is uncommon, and correct diagnosis and appropriate therapy for it are often delayed. Six patients with infection-associated scleritis were seen at our institution between May 1983 and May 1990 (these patients represented 4.6% of all patients with scleritis [six of 130 patients] in that period). Three of these cases were associated with systemic infections. One was associated with syphilis, one was associated with tuberculosis, and one was associated with toxocariasis. Three cases resulted from local infections. One was associate

Fulminant pseudomonal keratitis and scleritis in human immunodeficiency virus-infected patients.

Patients with human immunodeficiency virus infection are predisposed to fungal, parasitic, and viral infections. Bacterial infection can also be seen, although ocular bacterial infections have not been reported in patients with acquired immunodeficiency syndrome until recently. We present two cases of Pseudomonas corneoscleritis and one case of Pseudomonas keratitis in patients with human immunodeficiency virus infection that failed to respond to antibiotic treatment. Predisposing factors included extended-wear soft contact lens use in one patient and exposure secondary to Bell's palsy in anot

Sicca syndrome in patients infected with the human immunodeficiency virus.

PURPOSE: To investigate an association of the severity of the human immunodeficiency virus (HIV) disease with decreased tear production in a controlled setting. PATIENTS AND METHODS: Seventy-two patients (144 eyes) and 30 age- and sex-matched control subjects (60 eyes) were studied prospectively. Tear production was measured using the Schirmer I test (measured in millimeters after 5 minutes). Patients were classified into three clinical groups: asymptomatic HIV infection, lymphadenopathy syndrome (LAS) or acquired immune deficiency syndrome (AIDS)-related complex (ARC), and AIDS. Additionally

[No detection of mycoplasma in 205 conjunctival swabs].

Mycoplasma is known to cause pulmonary and urogenital infections in humans. A pathogenicity for the human eye never has been assessed, although they have been observed in the peripheral eye in a few cases of acute and non-specific conjunctivitis, in patients with Reiter's disease and in newborns. Due to the absence of cell walls, mycoplasma is only sensitive to a few antibiotics. Therefore, we presume that mycoplasma has a role in non-specific and chronic conjunctivitis. It is conceivable that in HIV-positive patients the risk of opportunistic colonization of the conjunctiva by mycoplasma is i

Acute anterior uveitis and optic neuritis as ocular complications of influenza A infection in an 11-year-old boy.

The authors describe an 11-year-old boy developing bilateral acute anterior uveitis, papillitis in one eye, and neuroretinitis in the other eye after an upper respiratory tract infection of influenza A virus, possibly H1N1. Steroid pulse therapy resolved these conditions. The authors recommend alertness for visual blurring and ocular inflammation after influenza A infection.

[Local prevention with interferon of "epidemic" conjunctivitis caused by a currently unidentifiable virus].

Topical Interferon application did effectively interrupt the further clinical spread of a highly contagious viral keratoconjunctivitis. A total of 130 endangered inpatients, still free from symptoms, were treated with one drop per eye daily for a period of 12 days. 123 patients stood free from any further infection. Only during the first two days after the beginning of the Interferon prophylaxis 7 inpatients showed an acute viral keratoconjunctivitis still. Obviously these patients were infected prior to the prophylactic therapy and were just in the incubation period. We conclude that Interfer

Experimental ocular toxocariasis: a mouse model.

Male mice, strain C57 black, were infected with Toxocara canis by a single intragastric dose of 1500 infective eggs. The eyes were studied at sequential time periods after infection (6 to 63 days) by conventional microscopic techniques, and the histological characteristics of the inflammatory response were recorded. In the majority of animals the disease was unilateral. Twenty-six larvae were found in the retina, in the retinal vessels, and in the subretinal space in 20 eyes, while in 29 eyes there were inflammatory changes which were not related to the presence of intact or fragmented larval

Hyperacute optic neuritis in a patient with COVID-19 infection and vaccination: a case report.

BACKGROUND: As scientific knowledge continues to grow regarding coronavirus disease 2019 (COVID-19) infection, several neuro-ophthalmological manifestations have emerged, including rare reports of optic neuritis. Optic neuritis is an inflammatory demyelinating condition of the optic nerve that typically presents as subacute, unilateral vision loss and pain on eye movement. Several cases of COVID-19 infection and COVID-19 vaccination related cases of optic neuritis have been reported. We present a case of hyperacute, unilateral optic neuritis after both recent COVID-19 infection and subsequent

Evidence for the involvement of interleukin-1α during development of experimental cytomegalovirus retinitis in immunosuppressed mice.

Interleukin-1α (IL-1α) is an alarmin involved in the recruitment of macrophages and neutrophils during tissue inflammation. IL-1α can undergo cleavage by proteases, such as calpain-1, that enhances IL-1α binding to its receptor, although proteolytic cleavage is not necessary for biological activity. Macrophages and neutrophils are involved in the retinal inflammation associated with development of AIDS-related human cytomegalovirus (HCMV) retinitis. We therefore performed studies to test the hypothesis that IL-1α gene expression is stimulated intraocularly during retinitis development using tw

Bacterial pathogens and antimicrobial susceptibility in ocular infections: A study at Boru-Meda General Hospital, Dessie, Ethiopia.

INTRODUCTION: The eye consists of both internal and external compartments. Several variables, including microbes, dust, and high temperatures can cause eye illnesses that can result in blindness. Bacterial eye infections continue to be a major cause of ocular morbidity and blindness, and their prevalence is periodically rising. The objective of the study was to detect bacterial pathogens and assess their susceptibility profiles to antibiotics in the ophthalmology unit of Boru-meda Hospital in Dessie, Ethiopia. METHODS: A hospital-based cross-sectional study was conducted from February 1 to Ap…

A viral model for corneal scarring and neovascularization following ocular infection of rabbits with a herpes simplex virus type 1 (HSV-1) mutant.

PURPOSE: Herpes simplex virus type 1 (HSV-1) remains a major cause of corneal scarring and visual loss. Although efforts have been made, no reproducible animal model is available to examine recurrent corneal disease. Here we propose a rabbit ocular model to study recurrent corneal disease using an HSV-1 mutant that reactivates with high efficiency. METHODS: Rabbits were ocularly infected with 2 x 10 PFU/eye of the parental McKrae, dLAT2903 (a LAT-null virus with a low-reactivation phenotype), or CJLAT (a high-reactivation virus). Acute ocular disease [days 2, 4, 7, and 10 postinfection (pi)],
