Heterotopic ossification and peripheral nerve entrapment: early diagnosis and excision.

Heterotopic ossification can occur in neurologic disorders, burns, musculoskeletal trauma, and metabolic disorders. In addition to producing the complications of contracture, skin breakdown, and pain, it can cause peripheral nerve entrapment. Nerve entrapment due to heterotopic ossification may be misdiagnosed, and it is difficult to evaluate and treat without recurrence. Computed tomography is especially useful in localization before surgical release of the entrapped nerve. Resection of heterotopic ossification can be successful using disodium etidronate to decrease the risk of recurrence, an

[Severe methotrexate poisoning].

Methotrexate toxicity is rare but extremely severe. When complete, it consists of ulcerations of the gastrointestinal mucosae responsible for necrotizing enteritis, erythroderma, bone marrow aplasia, interstitial pneumonia, hepatitis and organic renal failure with diuresis. Toxicity is facilitated by pre-existing renal impairment, third sector and abstention or underdosage of foliculinic acid prescribed as antagonist. The diagnosis rests on serum assays, the results of which must be interpreted taking into account the assay method and the time elapsed between the injection of methotrexate and

Reconstruction of bone and soft tissue defects with free fibula transfer.

More and more it is recognized that for the reconstruction of extensive long-bone defects, especially in combination with soft tissue loss, revascularized bone transfers are preferable. For the reconstruction of larger bony defects, revascularized crista iliaca bone or fibula bone are the first choice. The fibula has certain advantages over the crista: It is a straight cortical bone, the length is almost always sufficient to reconstruct large defects, and soft tissue reconstruction is possible by including a skin island. A short description of the operation technique, pitfalls, and indications

Giant cell tumor with viral-like intranuclear inclusions associated with Paget's disease.

A case of giant cell tumor (GCT) of the distal femur in a 56-year-old man is reported in association with Paget's disease. The case is unique in that is is the first report of GCT arising in a patient with this disease in which the neoplasm arose in a bone that did not show radiologic evidence of Paget's disease. This case also represents the first example of such an association in which viral-like inclusion bodies are seen in the osteoclast of the tumor. The fact that similar inclusion bodies are seen in the osteoclast of patients with Paget's disease and in their GCT implies a cause-and-effe

Giant cell tumor of long bone: a Canadian Sarcoma Group study.

A multicentric retrospective study of giant cell tumor of bone was conducted among Canadian surgeons. The hypothesis was that no differences would be found in health status, function, or recurrence rate irrespective to the nature of filling material or adjuvant used in patients treated with curettage. One hundred eighty-six cases were collected. There were 96 females and 90 males. The mean age of the patients was 36 years (range, 14-72 years), the minimum followup was 24 months, and the median followup was 60 months. Sixty-two percent of the tumors involved the knee region. One hundred fifty-e

Treatment of giant-cell tumors of long bones with curettage and bone-grafting.

BACKGROUND: The use of curettage, phenol, and cement is accepted by most experts as the best treatment for giant-cell tumor of bone. The present study was performed to evaluate whether equivalent results could be obtained with curettage with use of a high-speed burr and reconstruction of the resulting defect with autogenous bone graft with or without allograft bone. METHODS: The prospectively collected records of patients who had a giant-cell tumor of a long bone were reviewed to determine the rate of local recurrence after treatment with curettage with use of a high-speed burr and reconstruc

Computed tomography in the preoperative evaluation of masses arising in or near the joints of the extremities.

Knowledge of the precise anatomical relationship of a mass lesion to a joint is helpful in the preoperative planning of surgery. We have evaluated this relationship in 23 patients with a variety of benign and malignant lesions related to the knee and other major joints. In 15 patients, the lesion-joint relationship was assessed by routine computed tomographic (CT) scans followed within 24 hours by arthrography. In nine of these 15 patients, CT demonstrated that the lesion either was remote from the joint or clearly invaded it, and arthrography could have been avoided in these patients. In six

Is Proximal Femur Reconstruction With a Vascularized Fibula and Allograft Successful at Reconstructing a Tumor Resection in Children 6 Years of Age or Younger?

BACKGROUND: Treating bone sarcomas in young children, particularly in the proximal femur, is challenging because of the need to preserve growth potential, maintain joint function, and conserve bone for future revisions. In 1997, we introduced a new technique combining a vascularized fibula that preserved the proximal epiphysis to substitute for the femoral head and physis with a massive bone allograft aimed at providing a supportive scaffold for the autograft during growth. This approach initially showed promising results in both anatomic and functional restoration, but because it was a comple

Clinical evaluations of diaphysis malignant tumors of femur and tibia treated with microwave ablation in situ.

BACKGROUND: This study was designed to evaluate the clinical outcomes of patients with diaphysis malignant tumors of femur and tibia treated with microwave ablation (MWA) in situ. METHODS: Retrospective study of 32 patients with diaphysis malignant bone tumors of femur or tibia have been treated by microwave ablation. Instead of en bloc resection, hyperthermia ablation in situ was carried out followed by strengthen procedure. The patients were followed up for a period ranging from 36 to 180 months. RESULTS: Twenty-five patients survived over 3 years and all of the patients alive have a satis

[Spontaneous rupture of the femoral artery after radiotherapy: a case report].

Radiation-induced arteriopathy is a well-known disease whose incidence is not known and which usually arises chronically many years after radiation therapy. When it arises acutely, spontaneous rupture or, more rarely, thrombosis of the involved vessel may occur. Spontaneous rupture can occur within 4 to 32 weeks of radiotherapy, and usually affects the carotid artery involved in radiotherapy of the neck and head. Spontaneous rupture of the femoral artery is a very rare event and only a few cases have been reported in the literature. In this paper we report a case of spontaneous rupture of the

Joint-preserving limb salvage surgery under navigation guidance.

BACKGROUND: Recently, the navigation system has been introduced to orthopedic oncology. It can apply MRI and/or CT images to intraoperative visualization. We performed navigation-assisted limb salvage surgeries on patients with a malignant bone tumor of the metaphysis of the long bone or the iliac bone while preserving the adjacent joint. METHODS: When preoperative chemotherapy was estimated to be effective by imaging studies and the residual remaining epiphysis was expected to be more than 1 cm long after tumor resection with 1-2 cm of surgical margin, joint-preserving surgery was performed

Parosteal osteoma of bones other than of the skull and face.

BACKGROUND: Osteoma of the skull and facial bones is not uncommon, is usually asymptomatic, and may be associated with Gardner's syndrome. Osteomas involving other bones are unusual. METHODS: The authors describe 14 cases (eight men and six women from age 21-66 years) of parosteal osteoma of bones other than of the skull and face. RESULTS: Thirteen lesions involved long bones, and one involved the clavicle. A long-standing (18 months to 31 years) mass was reported in seven patients. Symptoms of pain, a mass, or both were present in all except three patients who were asymptomatic. Lesions ran

Prolapse of the mitral valve is described in two patients with the Ebstein's anomaly of the tricuspid.

Prolapse of the mitral valve is described in two patients with the Ebstein's anomaly of the tricuspid valve. This association has not been described previously. It is probable, however, that this association is not a rare one, but that clinical features of the prolapsing mitral valve are obscured by those resulting from the malformed tricuspid valve. Opportunity also was provided to study anatomically the mitral valve of a patient known to have a systolic click and a late systolic murmur (the Barlow syndrome). Although there have been several anatomic descriptions of floppy mitral valve at nec

Which implant is best after failed treatment for pathologic femur fractures?

BACKGROUND: Successful treatment of pathologic femur fractures can preserve a patient's independence and quality of life. The choice of implant depends on several disease- and patient-specific variables; however, its durability must generally match the patient's estimated life expectancy. Failures do occur, however, it is unclear which implants are associated with greater risk of failure. QUESTIONS/PURPOSES: We evaluated patients with femoral metastases in whom implants failed to determine (1) the rate of reoperation; (2) the timing of and most common causes for failure; and (3) incidence of

Mechanical strength of a femoral reconstruction in paediatric oncology: a finite element study.

In 1997 the proximal femur of a four-year-old child affected by a Ewing sarcoma was reconstructed using a massive bone allograft in conjunction with a vascularized fibula autograft. During the first three years of follow-up the reconstruction underwent important morphological changes. The aim of the present study was to evaluate the risk of fracture of the reconstructed proximal femur, once the physiological loads are restored, associated with a short, slow but unprotected level walk. Subject-specific finite element models of the operated femur, and of the intact contralateral one, were genera

Is there still a role for osteoarticular allograft reconstruction in musculoskeletal tumour surgery? a long-term follow-up study of 38 patients and systematic review of the literature.

AIMS: To assess complications and failure mechanisms of osteoarticular allograft reconstructions for primary bone tumours. PATIENTS AND METHODS: We retrospectively evaluated 38 patients (28 men, 74%) who were treated at our institution with osteoarticular allograft reconstruction between 1989 and 2010. Median age was 19 years (interquartile range 14 to 32). Median follow-up was 19.5 years (95% confidence interval (CI) 13.0 to 26.1) when 26 patients (68%) were alive. In addition, we systematically searched the literature for clinical studies on osteoarticular allografts, finding 31 studies sui
