Direct Anterior Approach for Hip Hemiarthroplasty.

PURPOSE: The purpose of this video is to demonstrate a surgical technique for a hemiarthroplasty of the hip joint through a direct anterior approach. We have found this technique to be a viable option for the treatment of displaced femoral neck fractures. METHODS: Multiple approaches to the hip are described for arthroplasty. Direct anterior approach for hemiarthroplasty is a safe, reproducible approach. RESULTS: The video displays our technique for performing a direct anterior approach to the hip joint for a hemiarthroplasty in the setting of a displaced femoral neck fracture. CONCLUSIONS:

Valgus Intertrochanteric Osteotomy for Femoral Neck Nonunion.

Valgus intertrochanteric osteotomy is an effective method of treating femoral neck nonunion by reducing shear forces at the fracture and correcting the neck-shaft angle. Good outcomes have been reported in the literature. Through careful preoperative planning and a precise operative technique, reliable healing of both the osteotomy and nonunion can be achieved.

Coxa Vara Deformity in Fibrous Dysplasia/McCune-Albright Syndrome: Prevalence, Natural History and Risk Factors: A Two-Center Study.

This study aimed to evaluate the prevalence of and risk factors for coxa vara deformity in patients with fibrous dysplasia/McCune-Albright syndrome (FD/MAS). This study was conducted at the National Institutes of Health and Leiden University Medical Center. All patients with any subtype of FD/MAS, FD involving the proximal femur, one or more X-rays available and age <30 years were included. X-rays were scored for the neck-shaft angle (NSA). Varus deformity was defined as NSA <110 degrees or >10 degrees below age-specific values. Risk factors for deformity were assessed by nested case-control a

Femoral Neck Non-Union - A Biomechanical Solution with Valgus Sliding Subtrochanteric Osteotomy: A Case Report.

Avascular necrosis and non-union are two most dreaded complications of femoral neck fracture fixations. Hip replacement seems to be a simple solution for this complex problem. However, the long-term efficacy of prosthetic replacement in the young population with higher functional demand is still questionable. Femoral head preserving valgus subtrochanteric osteotomies in properly selected cases have strong support from literature. The conventional technique of valgus subtrochanteric osteotomy involves lateral based wedge resection. Alternatively, a simpler sliding oblique subtrochanteric osteot

Dutch variant of Bellini metaphyseal dysplasia: report of two siblings.

Two sibling girls with cone-shaped knee epiphyses and metaphyses are described. Bone dysplasia with this rare, distinctive, radiographic finding, was first reported by Bellini and Bardare with only few cases reported thereafter. Velores et al. divided bone dysplasias with cone-shaped epiphyses and metaphyses of the knee in two entities which they named trichoscyphodysplasia and metaphyseal acroscyphodysplasia. Although the authors agree that there is more than one bone dysplasia that presents with these distinctive radiographic knee appearances, they consider that two few cases have been repor

The sagging rope sign: a critical appraisal.

Certain features of the sagging rope sign recently analysed by Apley and Weintroub (1981) are examined in detail. Evidence is presented to show that the line is a radiological shadow cast by the lateral edge of a severely deformed femoral head rather than a condensation of the spongiosa within the neck. An explanation is offered to explain the common association of the presence of this radiological sign with premature epiphysial fusion.

Pugh nail system as a form of treatment for fractures of the proximal femur.

A retrospective study of the results of treatment of 177 patients who had sustained a fracture of the proximal femur was undertaken. The social background, type of injury, operation performed, duration of stay after operation, mortality and morbidity were recorded and compared with the results published by other workers. It is concluded that the Pugh sliding nail system adequately fixes most fractures of the proximal femur. It allows early postoperative mobilization and discharge to the home environment for most patients. The overall mortality and morbidity compares favourably with results pub

Imaging of the Microstructural Failure Mechanism in the Human Hip.

Imaging the bone microstructure under progressively increasing loads allows for observing the microstructural failure behavior of bone. Here, we describe a protocol for obtaining a sequence of three-dimensional microstructural images of the entire proximal femur under progressively increasing deformation, causing clinically relevant fractures of the femoral neck. The protocol is demonstrated using four femora from female donors aged 66-80 years at the lower end of bone mineral density in the population (T-score range = -2.09 to -4.75). A radio-transparent compressive stage was designed for loa

Spontaneous femoral neck fracture resulting from osteonecrosis involving lateral femoral head-neck junction: a retrospective study.

BACKGROUND: Spontaneous femoral neck fracture is a rare condition that remains controversial due to limited reported cases. This retrospective study aims to provide further insights into the etiology and characteristics of the disease. METHOD: We conducted a retrospective review of data from 963 patients with femoral neck fractures. The data encompassed demographic information, medical histories, radiographic records, bone mineral density (BMD) measurements, and pathological examinations. Patients were categorized into two groups: spontaneous femoral neck fracture (SFF) group (30 cases) and c

Application of a novel osteotomy instrumentation as a substitute tool in total hip arthroplasty.

BACKGROUND: Mechanical failure, power shortage, and inadvertent contamination of the oscillating saw occasionally occurs in actualizing femoral neck osteotomy during total hip arthroplasty (THA); however, no appropriate alternative solution is currently available. This study aimed to introduce a novel osteotomy instrumentation (fretsaw, jig, cable passer hook) as a substitute tool while the oscillating saw was unavailable during THA. METHODS: This study included 40 patients (40 hips) who underwent femoral neck osteotomy during primary THA using the new osteotomy instrumentation (n = 20) and t

Three-dimensional mapping of cortical porosity and thickness along the superolateral femoral neck in older women.

Although several studies have analyzed inter-individual differences in the femoral neck cortical microstructure, intra-individual variations have not been comprehensively evaluated. By using microCT, we mapped cortical pore volume fraction (Ct.Po) and thickness (Ct.Th) along the superolateral femoral neck in 14 older women (age: 77.1 ± 9.8 years) to identify subregions and segments with high porosity and/or low thickness-potential "critical" spots where a fracture could start. We showed that Ct.Po and Ct.Th significantly differed between basicervical, midcervical, and subcapital subregions of

No association between telomere length and osteonecrosis of the femoral head.

BACKGROUND: Telemore length (TL) shortening has been found in many diseases. However, clinical characteristics of TL shortening in osteonecrosis of the femoral head (ONFH) has not been investigated. Therefore, we studied whether TL changes have clinicopathological values in ONFH. METHODS: The TL in the synovial tissues of 36 ONFH and 127 control patients (femoral neck fracture) was examined by quantitative real-time PCR as relative length, Δ Ct value. In addition, the correlation between TL and clinical features of ONFH and controls was analyzed. RESULTS: The average TL in the femoral tissue

Qualitative and quantitative magnetic resonance imaging evaluation of bone tissue vaso-occlusive events in patients with sickle cell disease.

OBJECTIVE: To evaluate the association between bone changes due to vaso-occlusive events in sickle cell disease (SCD) revealed by conventional MRI sequences and the fat fraction obtained using a 6-point DIXON technique (FFdix), in an attempt to use quantitative data as a biomarker for bone complications. METHODS: Cross-sectional study, with 48 SCD patients, 26-homozygous (HbSS), and 22-compound heterozygous (HbSC). Forty-eight healthy individuals paired by age, weight, and sex with SCD patients. All participants underwent lumbar spine and pelvis MRI. Conventional sequences: bone complications

Avascular necrosis after femoral neck fracture in children and adolescents: poor prognosis and risk factors.

INTRODUCTION: Avascular necrosis (AVN) after femoral neck fracture (FNF) is a rare and severe paediatric condition, but only few studies described its prognosis and risk factors. The present study aimed to evaluate the outcomes and independent factors for poor prognosis of AVN after FNF in children and adolescents. METHOD: This retrospective study included children and adolescents with AVN after FNF who received conservative treatment (CT group) or non-vascularized bone grafting (NVBG group) between 2000 and 2018. The primary outcomes were the risk of hip arthritis (Tönnis grade) and hip defo

Femoral neck shaft angle is not correlated with femoral version: A retrospective study of computed tomography scans.

BACKGROUND: An understanding of femoral anteversion and neck-shaft angle (NSA) is essential to deliver optimal orthopaedic surgical care. Despite the importance, there is little research examining the relationship between femoral anteversion and the NSA in an adult population. This study sought to determine if there is a correlation between femoral neck shaft angle and version in skeletally mature adults using computed tomography (CT) scanograms. METHODS: Between January 2010 and June 2017, all skeletally mature patients who had received a CT scanogram of the lower extremity were reviewed. Ex

Bone endoscopy: direct visual confirmation of cannulated screw placement in slipped capital femoral epiphysis.

Intraosseous endoscopic examination of the femoral neck and head was performed during the course of percutaneous screw fixation in 12 patients with slipped capital femoral epiphysis (SCFE). Visualization was satisfactory in 13 hips of these patients. We were able to distinguish trabecular bone and physeal cartilage endoscopically. Evidence of articular penetration was documented endoscopically in two patients. One patient had been referred for persistent hip pain and chondrolysis 6 months after screw insertion. Chronic joint penetration was observed endoscopically at the time of revision opera

Recurrent Aneurysmal Bone Cyst Treated with Percutaneous Doxycycline Sclerotherapy: A Case Report.

CASE: A 7-year-old girl with an aggressive proximal femur aneurysmal bone cyst (ABC) who sustained a pathological fracture of the femoral neck required an open reduction and internal fixation with curettage and bone grafting. This was followed by early aggressive recurrence of the ABC, which was treated successfully with percutaneous doxycycline sclerotherapy. CONCLUSION: Doxycycline sclerotherapy seems to be an effective and relatively low-risk treatment for recurrent ABCs.

Prostatic adenocarcinoma with osseous metastases in a dog.

Bone scintigraphy was used to diagnose osseous metastasis of prostatic adenocarcinoma in a 10-year-old dog with neck pain and ataxia and a large, sensitive prostate gland. Although radiography revealed a normal spine, prostatic fluid cytologic and ultrasonographic findings were compatible with prostatitis or neoplasia. Scintigraphic hot spots were seen in the axial skeleton, ribs, pelvis, humerus, and femur and corresponded to sites of metastatic prostatic adenocarcinoma.
