Human fertility problems may be due to chromosomal aberrations in one or both partners. In couples asking for in-vitro fertilization (IVF) the question arises as to whether chromosomal analysis is necessary. We analysed the karyotypes of 72 couples attending our clinic for IVF. BACKGROUND: We report the possible association between in vitro fertilization (IVF) and recurrent acute pancreatitis.
CASE: A patient developed acute pancreatitis during each of two cycles of IVF. On a spontaneous cycle, serum triglycerides were as follows: early follicular phase 2.34 mmol/L, mid-follicular phase 4.17 mmol/L, and late follicular phase 6.6 mmol/L. During an episode of acute pancreatitis, the serum triglyceride level was 38.45 mmol/L.
CONCLUSION: Acute pancreatitis may occur in patients with a family or personal history of hypertriglyceridemia who are candidates for IVF.

In 33 patients treated with a combination of an LHRH agonist (LHRH-A) and gonadotrophin in a long protocol, a biological hyperstimulation occurred (E2 greater than 2500 pg/ml on the day of HCG administration and 4722 +/- 1190 pg/ml the day after, with greater than 10 follicles greater than 12 mm on each ovary). In vitro fertilization enabled the study of lethal (parthenogenesis) or sublethal (triploidy, monosomy and trisomy) chromosomal abnormalities in man. According to the literature, 23 to 71% of preimplantation embryos carry a chromosomal defect. Various factors, such as delayed fertilization, early embryo fragmentation or elevated maternal age (greater than 35 years) are related to an increase in the incidence of chromosomal aberrations. These data reinforce the debate on a preimplantation genetic diagnosis in order to select for transfer only viable and apparently normal embryos.

The chromosome constitution of 22 human preimplantation embryos from donor oocytes fertilized in vitro by donor sperm was studied to assess the contribution of lethal chromosome anomalies to the high failure rate of implantation of in vitro fertilized embryos after embryo transfer in infertile women. Evidence was found of nondisjunction, resulting in trisomy, monosomy, and nullosomy; structural abnormalities; haploidy; and triploidy. Despite the lethality of their chromosome complements, these embryos could not be distinguished morphologically from those with normal chromosomes.

A case of a triploid embryo (69,XXX) resulting from in vitro fertilization is reported. The pregnancy ended in spontaneous abortion 44 days after embryo transfer. The embryo showed anomalies considered typical of triploidy but molar changes were not noted. The origin of extra chromosomes was apparently maternal.
