Shock may be difficult to recognize in pregnant women due to the physiological changes that take place in the cardiovascular system. The first symptom of shock may be foetal distress. Fetal oxygen saturation (FSpO2) was monitored with the Nellcor Puritan Bennett N400/FS14 system during 16 labors to establish whether FSpO2 was influenced by maternal bearing down efforts in the second stage of labor. Fetal SpO2 is reported for 16 fetuses where neonatal outcome was normal. One hour of continuous data was recorded: 30 min prior to the onset of maternal bearing down efforts and the first 30 min of pushing. The hour was divided into six epochs of 10-min duration. OBJECTIVE: To develop a nonpenetrating scalp electrode for intrapartum fetal monitoring.
DESIGN: Preliminary observational evaluation of the device.
SETTING: An urban academic hospital in Johannesburg, South Africa.
SUBJECTS: Fifteen women in labour.
INTERVENTION: Application of the electrode for intrapartum monitoring.

One hundred thirty-one postdate pregnancies were evaluated by the sonographic biophysical profile. Results of prenatal testing were compared to neonatal condition. Predictive values were calculated in order to determine whether this testing scheme could be used to detect the fetus at risk for neonatal distress. The results of this study show that a normal biophysical profile score is highly predictive of normal outcome, but an abnormal test has only a 14% predictive value of poor neonatal outcome.

The fetal heart rate (FHR) response to a controlled sound stimulus of 80 dB was registered in 152 at-risk pregnancies. Thirty-four of the infants showed signs of an intra-uterine pathological condition at birth, whereas 118 infants were clinically unaffected. A 31-year-old nulliparous woman developed vaginal bleeding after spontaneous rupture of the membranes at 40 weeks of gestation. Fetal heart rate monitoring showed checkmark and sinusoidal patterns.
