[Less invasive fixation for the treatment of comminuted fracture of distal tibia].

OBJECTIVE: To study the result of less invasive fixation in treating comminuted fracture of distal tibia. METHODS: From 2002 to 2008, 48 patients with comminuted fracture of distal tibia were treated with surgery. The closed reduction and less invasive fixation were done to stable broken fibula, and the reduction by Kirschner wire to pick was used for relatively larger debris of tibia. The Kirschner wire or screw were used to fix fracture after the restore of the ankle cavity position and the alignment of the tibia. Partial weight loading and functional exercise of ankle joint were done at 6t

Torsional strength of the ankle in vitro. The supination-external-rotation injury.

Nineteen fresh-frozen anatomic specimens of the ankle were mounted in an experimental test device with the foot in supination. Torque versus rotation curves were recorded as each foot was rotated externally to failure. The mean failure torque was 45.3 Nm, the mean rotation to failure was 41.4 degrees, and the mean energy absorbed to failure was 10.6 Nm. Twelve specimens failed by fracture of the fibula, five by failure of lateral ligaments with no fibular fracture, one by a subtalar dislocation, and one by fracture of the calcaneous through a fixation screw hole. There were no statistically si

Proximal humerus fractures in the elderly: is there (still) a role for plate osteosynthesis?

INTRODUCTION: Proximal humeral fractures (PHFs) are among the most common fractures in the elderly population resulting from low energy trauma. The primary aim of this review is to assess the role of primary locking plate osteosynthesis in elderly patients with complex PHFs and highlight key surgical techniques to optimize outcomes. A literature overview and two illustrative cases are presented. CONCLUSION: Treating PHFs in elderly patients requires careful patient selection, adequate patient informed consent and shared decision making. Fit elderly (60-75 years) patients with complex 3- or 4-

Combined use of the Ilizarov method, concentrated bone marrow aspirate (cBMA), and platelet-rich plasma (PRP) to expedite healing of bimalleolar fractures.

Distal tibial and fibular fractures, particularly in patients with comorbidities, heal slowly and have a high incidence of postoperative nonunion and infection. Autologous concentrated bone marrow aspirate (cBMA) and platelet-rich plasma (PRP) increase osteogenic potential of demineralized bone matrix (DBM). The purpose of this case series was to evaluate the efficacy of cBMA, PRP, DBM in conjunction with the Ilizarov fixator as compared to DBM and the Ilizarov fixator alone in expediting fracture healing. Ten patients (mean age 52.9 years) were in the cBMA Group, and 10 patients (mean age 54

Malleolar fractures of the ankle. A comprehensive review.

Ankle injuries and associated malleolar fractures are common. The outcome of treatment depends on the treating physician's ability to properly identify the mechanism of injury and subsequently restore ankle function by accurate realignment of the normal ankle articulations. Particular attention must be paid to restoring the length and alignment of the fibula because it plays a major role in stabilizing the talar ankle component. The medial aspect of the ankle also is important. The vital role of the deltoid has been proved in many studies, and although controversy exists in the proper treatmen

Treatment of Recalcitrant Isolated Congenital Fibular Pseudarthrosis: Fibular Segment Transfer and Tibiofibular Synostosis with Unilateral External Fixator.

Isolated congenital pseudarthrosis of the fibula is a rare entity with a limited number of cases reported in the literature. Treatment is challenging because of recalcitrant nonunion and because no consensus about the best treatment plan exists. We report a case of isolated congenital fibular pseudarthrosis with valgus deformity of the ankle. The patient had a history of two failed operations. We used a novel surgical plan that combined tibiofibular synostosis with fibular segment transfer through a unilateral external fixator. The patient showed good early results with fibular union. We advoc

A Radiological Study for Assessing Syndesmosis Malreduction: Its Validity and Limitation.

This study assessed the diagnostic utility of different X-ray radiological methods on syndesmosis malreduction. Thirteen fresh ankle specimens were used to make a syndesmotic separation model. The specimen was fixed in the anatomic position and in malreduction positions, including internal rotation 10° (IR10°), IR20°, external rotation 10° (ER10°), and ER20°. The tibiofibular clear space (TCS), tibiofibular overlap (TFO) on the anteroposterior view, and anteroposterior ratio (A/P ratio) on the lateral view were measured. When the syndesmosis was fixed in IR20°, the sensitivity of the TCS, TFO,

Treatment of tibial defect with vascularized osteocutaneous pedicled transfer of fibula.

Free osteocutaneous fibular flap transfer has been used for long bone and soft tissue defects in both the lower and upper extremity. Since July, 1984, two cases of tibial defect, with severe scar formation on the anteromedial aspect, have been treated. An ipsilateral, vascularized, osteocutaneous, pedicled flap of the fibula was used to bridge the bony defect and to resurface the wound, after scar excision and bony debridement. The peroneal vessels were severed, leaving one side intact. The entire fibular flap was shifted anteromedially through a tunnel between the anterior and posterior tibia

Immediate tibiocalcaneal arthrodesis with interposition fibular autograft for salvage after talus fracture: a case report.

Treatment goals in the operative management of talus fractures include prompt, anatomic, open reduction with rigid internal fixation; functional outcome is measured by degree of arthrosis, pain, range of motion, limb length, cosmesis, and return to premorbid activities. If restoration of the articular surfaces is precluded secondary to comminution, immediate and/or staged reconstructive salvage procedures must be considered. This report describes an immediate reconstructive procedure for salvage after a comminuted talus fracture with an ipsilateral tibia fracture. A standard antegrade tibial n

Clinical significance of the free vascularized bone grafts in fractures with large bone defects and non-union.

With the rapid progress of microscopic techniques in surgery, the free vascularized bone graft has been widely applied in fractures with large bone defects, non-union, congenital pseudoarthrosis, bone tumors, osteomyelitis, aseptic necrosis of the bone and osteogenesis imperfecta. We obtained excellent results with the free vascularized bone graft in three cases. One was a case of non-union with large bone defects of the right femur that had not improved despite many attempts at treatment over nine years after an open fracture of the right femur. The other two cases were open fractures with ma

[Tibial fractures in children (author's transl)].

In a collective study of 22 surgical clinics 1235 closed diaphyseal tibial fractures in children from 0 to 14 years were analysed. The incidence of this type of fracture in relation to all fractures in childhood, was 6.2%. In pre-school children isolated tibial fractures and incomplete fractures predominated, while in children from 6 to 14 years tibial plus fibular fractures as well as displaced fractures occurred more frequently. Closed reduction and plaster cast were used as standard treatment. Traction was used only exceptionally. In only 3.0% osteosynthesis was performed under the followin

Identification of concomitant injuries associated with specific spine level fractures in polytrauma patients.

INTRODUCTION: Spine fractures are associated with high energy mechanisms and can lead to substantial morbidity and mortality in the trauma setting. Rapid identification and treatment of these fractures and their associated injuries are paramount in preventing adverse outcomes. The purpose of this study is to identify concomitant skeletal and non-skeletal injuries related to cervical, thoracic, and lumbar fractures. METHODS: A retrospective review of institutional American College of Surgeons (ACS) registry was conducted on 3,399 consecutive trauma patients identifying those with spine fractur

The Use of Optical Tracking to Characterize Fracture Gap Motions and Estimate Healing Potential in Comminuted Biomechanical Models of Surgical Repair.

Fracture healing is stimulated by micromotion at the fracture site, whereby there exists an optimal amount of strain to promote secondary bone formation. Surgical plates used for fracture fixation are often evaluated for their biomechanical performance using benchtop studies, where success is based on overall construct stiffness and strength measures. Integration of fracture gap tracking to this assessment would provide crucial information about how plates support the various fragments present in comminuted fractures, to ensure there are appropriate levels of micromotion during early healing.

Subtrochanteric femoral fracture with postoperative chronic osteomyelitis treated successfully by 1-stage operation: a case report.

Chronic osteomyelitis is a chronic infectious disease of bone tissue, which can cause necrosis of bone and surrounding soft tissue, and is a common complication of open fracture, internal fixation, diabetic foot and blood-borne bone infection. Traumatic osteomyelitis is caused by bone tissue infection after open fracture surgery or open reduction of fracture or other bone and joint surgery. The lesion is near the fracture end. Intramedullary infection is the most serious infection in acute stage, with high fever, chills and other toxemia symptoms, similar to acute hematogenous osteomyelitis. T

Ankle fractures reduction under direct visualization. Superiority of anatomical landmarks versus radiological evaluation- A randomised control trial.

Distal tibiofibular joint injury is quite common in rotational ankle fractures, with high malreduction rates reported. Although several intraoperative techniques evaluate the optimal tibiofibular reduction, they are critically debated due to high error rates and subjective interpretation of the results. We attempted to describe specific anatomical landmarks and anatomical relationships of the ankle joint through capsulotomy and inspection of the anterior incisura fibularis corner and evaluate their reliability regarding optimal tibiofibular reduction. Sixty patients with malleolar fractures an
